Location for Case Study:

Location we have chosen is Machine workshop which is under Department of


Mechanical Engineering (JKM). The Machine workshop along with other workshop
such as Welding workshop, GEGAS and automobile workshop located near the
Department of Mechanical Engineering(JKM) and Department of Science, Mathematics
and Computer(JMSK).

Introduction

Machine workshop is built to enhance knowledge of student to handle machine and


know equipment that are used in the workshop. Likewise, machine has been associated
with many activities that may causes injury and maybe death due to many kind of
improper activities when handling the machine.

There are many kind of equipment that is involved in this workshop for student
to learn. Basic machine such as rolling machine that has been used for shaping tool bit
so that it can be used in machine. In this workshop there are only four use able rolling
machine. Rolling machine in this workshop consist of two type which are foot rolling
machine and hand rolling machine. Both of this type consists of two parts which are
smooth and rough surface.

There is also lathe machine where student used it to manufacture or crafting item
so that the workpiece can be used for many purpose. Lathe machine that are commonly
used here is Pinacho. For each type of lathe machine, it can be differentiate by number
of jaw chuck of machine. In some machine there are 4 jaw chucks while other has only
3 jaw chuck. However the functions of the machine are the same which is for lathering
workpiece. Lathe machine can undergo process of turning, milling, chamfering, and
drilling and tapering.Another type of machine that use here is drill machine. This
machine is commonly used for drilling workpiece. In this workshop there are only 4
drill machine available.

In this workshop the lecturer in charge is Pn.Nur Asyiqin Binti Idris ,En Khairil
Asyraf Bin Ahmad Maliki, and En Azrul Affandi Bin Eliah. Whereas, the Chief
Department for JKM is En Zairi Bin Osman. This workshop open from 8.00am to 12.00
afternoon and reopen again from 1.00pm to 5.00pm.

In this case study we are going to observe different type of hazard and grouping
the hazard according to their categories in machine workshop. After that, we are going
to use semi quantitative assessment method where matrix table is involved in the
process to analyse the hazard. From the matrix analysis we are going to determine step
should be taken to control risk based on hierarchy of risk control.

1) Hazard Identification

From investigation we have done, we have found out different type of hazard in this
workshop. Different type of hazard can be shown in diagram below.

Rolling Machine

This is a type of hazard that can be found at machine workshop. Rolling machine can
cause several types of hazard at once. Rolling machine can be associated with hazard
related to ergonomic where person need to bent when handle the machine. This may
cause back pain to the person. Besides that, the machine causes physical where the
machine causes vibration when in contact with the machine. From safety aspect , the
machine can cause injury to a person if there is no one guarded the machine or
accidentally to the machine.

Lathe Machine

The second hazard is from lathe machine. This machine can cause several type of
hazard such as physical hazard where the movement of chuck of machine can produce
vibration. Besides that, the machine can also cause ergonomic type hazard where if a
worker work for long period of time they may cause back strain or injures because poor
postures. Since the machine is dangerous thus it involve safety type hazard because it
use electricity and metal chip that produce while using the machine can cause serious
injury to a person if in contact. Moreover, lathe machine cause lot of noise without any
coolant or mechanical problem from the machine.

Coolant

Besides that, coolant also can cause hazard in this workshop. Since machine need
coolant to function effectively, coolant can be found abundantly in the workshop.
Improper way handling coolant can cause hazard such as chemical hazard and safety
since coolant spill on floor can cause tripping and falling of worker. Coolant that spill
on electrical hazard can cause death if a person steps on the water.

Power Supply

The next hazard is power supply of workshop. Since all the machines here are generated
by electricity, it can cause serious problem if any accident happen. Hazard that cause
from power supply is safety hazard where short circuit can cause electrical hazard when
contact with water.

Drill Machine

Besides that there is also drill machine that may cause hazard. Drill machine can cause
hazard such as physical hazard because vibration from the machine. Beside that it can
also cause ergonomic hazard since poor postures from working at machine can cause
back strain. Since there is chance for injury from drill it can cause safety type hazard for
the worker.

3) Semi-Quantitative assessment

Based on hazard that we found we have come up with semi quantitative assessment as
shown based on formula below

Rolling Machine

Rolling machine hazard can be reduces by isolating the machine into a small place.
Since the friction between workpiece and rolling machine causes vibration and heat, it
can be reduce by engineering control in which coolant can be added to reduce risk.
Worker should also wear Personal Protective Equipment(PPE) such as goggle and
safety coat to reduce debris from the machine.

Lathe machine

Lathe machine Hazard can be reduce by isolating the machine by giving spaces between
the machines. Then it can be reduces by engineering control by placing barrier and
redesigning the procedure so that chip from workpiece wont hit worker when handling
machine. After that, we can use administrative control where all the work are being
supervision and training from lecturer, job rotating between worker and housekeeping,
repair and maintenance programme to reduce hazard. Worker also should wear Personal
Protective Equipment such as goggle and safety boots to reduce hazard.

Coolant

Hazard from coolant can be reducing by using elimination method by throwing the
substance after using it.

Power Supply

Power supply risk can be reduce by isolating the power supply from any type chemical.
Power supply also should be built at higher place and no wire exposure by involving
engineering control. Administrative control should be involving wear all personal
should follow safety procedure, following scheduling maintenance and always do
housekeeping from any coolant or chemical nearby.

Drill Machine

Hazard from Drill Machine can be reduce by using engineering control. Machine should
be redesign so that the height can be adjusted for worker to handle the machine.
Administrative control should be done by involving safety work procedure when
handling the machine. The machine also should be always undergoing maintenance
because the machine use for long period of time. Personal Protective Equipment should
be wearing all the time to prevent worker from hit by chip from workpiece.
