WELCOME TO McCARTHY!
We are excited to have you here! This guide was designed to help you learn about the benefit options available to you and your family. We hope the
information that follows allows you to make the most of McCarthys benefits package. All full-time salaried employees are eligible for McCarthys
health, dental, vision, and HSA/FSA plans as well as company-provided basic group life insurance, Accidental Death and Dismemberment (AD&D),
insurance, and short-term and long-term disability insurances (STD/LTD). Voluntary life, critical illness and accident insurances, legal and identity
theft protection, and pet insurance are also available for enrollment. McCarthy also offers many free programs and tools that add extra value and
provide additional support for employees.
Benefits Eligibility
For eligible employees, benefits begin on your 1st day of employment. You have 31 days to make your elections. All full-time salaried
employees and their families are eligible. This includes your spouse or domestic partner and dependent children. Additional details
on dependent eligibility/ coverage and tax implications are available online. Attempting to insure someone who is ineligible may result
in cancellation of coverage.
You will only be able to make benefit changes during your new hire enrollment period or annual open enrollment period.
Changes are not permitted outside of those two events, unless you experience an IRS- approved change in family status which includes
the following:

Single Sign On: Access the website via single sign-on through Oracle PA. By accessing the site via Oracle, you will have direct access to

The BenefitFocus website requires a unique username and password for login.
SETTING UP USERNAME/PASSWORD
USERNAME: Your first name, the first initial of your last name and the last four digits of your Social Security Number.
Example: MichaelT1525 (username is not case sensitive)
Exceptions:
Enter only the first 10 letters of your first name
Example: Christopher becomes Christophe.
If you experience an issue after entering your username as specified above, there may already be someone in the system with
a similar login, so please enter the first two letters of your last initials.
Example: John Smith will likely have to enter JohnSm1234 instead of JohnS1234.
PASSWORD: If this is your first-time logging in, the password is your Date of Birth in the following format (01Jan1970). Once this
information is entered, you will be prompted to change your password.

Vitality participants are rewarded for making healthier choices by earning gift cards from great retailers, fitness trackers, and gym
membership rebates you will even reduce McCarthy medical premiums by participating in Vitality.

I RS Limits In 2017, the maximum employee contribution to a qualified retirement plan is $18,000, plus an additional $6,000 if age 50 or over. Additionally, theres also a total retirement plan
contribution limit (employee & employer) that can be contributed in single year for qualified retirement plans. For 2017, the limit is $54,000. This applies to both 401(k) and ESOP.

Health Advocate
McCarthy partners with Health Advocate to provide employees
and their families with a Personal Health Advocate, typically a
registered nurse, supported by medical director and benefits and
claims specialists to help handle healthcare and insurance-related
issues. Services are covered at no cost to employees and include
assistance with:
Diabetes, Weight Loss and Nutrition Written lesson plans and printed materials
Navigating the US Health System Data and progress tracking that can easily be shared with
your childs care team
Grand Rounds
Livongos diabetes management tool is currently provided free
of charge to McCarthy employees and their dependents covered
by a Cigna medical plan. Services include:
