WELCOME TO McCARTHY!
We are excited to have you here! This guide was designed to help you learn about the benefit options available to you and your family. We hope the
information that follows allows you to make the most of McCarthys benefits package. All full-time salaried employees are eligible for McCarthys
health, dental, vision, and HSA/FSA plans as well as company-provided basic group life insurance, Accidental Death and Dismemberment (AD&D),
insurance, and short-term and long-term disability insurances (STD/LTD). Voluntary life, critical illness and accident insurances, legal and identity
theft protection, and pet insurance are also available for enrollment. McCarthy also offers many free programs and tools that add extra value and
provide additional support for employees.

Benefits Eligibility
For eligible employees, benefits begin on your 1st day of employment. You have 31 days to make your elections. All full-time salaried
employees and their families are eligible. This includes your spouse or domestic partner and dependent children. Additional details
on dependent eligibility/ coverage and tax implications are available online. Attempting to insure someone who is ineligible may result
in cancellation of coverage.

You will only be able to make benefit changes during your new hire enrollment period or annual open enrollment period.
Changes are not permitted outside of those two events, unless you experience an IRS- approved change in family status which includes
the following:

The addition of a dependent through birth, adoption,


or marriage

The loss of a dependent through divorce or death, or if your


child reaches the maximum age limit for coverage

A change in you or your spouses employment status from


full-time to part-time (or vice versa)

A spouses loss of employment

A change in your spouses employer provided coverage


(ex: open enrollment)

IMPORTANT NOTE:

If youre enrolling your spouse/domestic partner and/or dependent


child(ren), you will be required to produce documentation verifying
your dependents eligibility for medical, dental, and vision benefits
under the McCarthy plans. If youre enrolling a domestic partner,
please ensure that you review the Domestic Partnership Tax
Implications Notice available on the McCarthy benefits website.

Enrolling for your benefits is easy. Simply access the benefits
website BenefitFocus at mccarthy.HRinTouch.com from any PC, laptop, tablet or smartphone device.

If you experience an issue after entering your username as specified above, there may already be someone in the system with
a similar login, so please enter the first two letters of your last initials.

PASSWORD: If this is your first-time logging in, the password is your Date of Birth in the following format (01Jan1970). Once this

Our medical benefits are designed to help you and your

You may choose to designate a Primary Care Physician,
although you are not required to do so. All covered
services must be provided, prescribed, authorized, or
directed by a Kaiser Plan Physician and services must
be provided inside your Home Region.

REDUCE MEDICAL PREMIUMS


If you choose to enroll in our medical plan,
you can reduce your annual medical plan premiums
by up to $400for yourself, plus up to another $400
for your spouse/domestic partner through Vitality.

MEDICAL PLAN DEFAULT ENROLLMENT


You have 31 days from your start date to complete benefit
enrollment online. If you do not make an enrollment
election within 31 days, you will be default enrolled
into our Cigna Minimum Value plan with employee
only coverage. The Cigna Minimum Value plan is
the only health plan in which default enrollment will
be processed.

If not eligible for HSA, the contribution will be added


to a Health Reimbursement Account (HRA).

Flexible Spending Account (FSA) Plan 2017 IRS Maximum Limit

FSAs are tax favored accounts that allow you to pay


for eligible medical and dependent care expenses

with pre-tax dollars. Some examples of Healthcare
FSA expenses include deductible and coinsurance,
and some examples of Dependent Care FSA expenses Dependent Care FSA $5,000
include nursery school or day care tuition, or before

accounts are a use it or lose it benefit meaning that
unused funds are not rolled over and are forfeited at
the end of the year. The Healthcare FSA does offer
a grace period where you may incur claims through

previous years funds. All claims must be submitted for
reimbursement by March 31st of the following year.

You are eligible to sign up for Healthcare FSA if you


have waived medical coverage, if you enrolled in
Kaiser medical plan, or if you are eligible for Health
Reimbursement Account (HRA). All employees are
eligible to sign up for Dependent Care FSA regardless
of medical plan participation.

The purpose of our retirement program is to assist you in meeting your savings goals and retirement needs. McCarthy offers a retirement program
comprised of two qualified defined contribution plans: the Employee Retirement Savings Plan (401k) and Employee Stock Ownership Plan (ESOP).

LIFE INSURANCE
BASIC TERM LIFE AND AD&D INSURANCE
Basic Term Life and AD&D insurance provided by Securian protects your familys financial future. Term Life insurance pays a benefit to your
beneficiaries if you die. AD&D insurance pays an additional benefit if you are seriously injured due to an accident or you die as the result of an
accident. Full-time salaried employees are provided with Basic Term Life insurance at 2.5 times your annual base salary up to a maximum of
$750,000. AD&D insurance is also provided at the same coverage level. These benefits are free and begin on your first day of employment.

OPTIONAL TERM LIFE AND AD&D INSURANCE OPTIONAL TERM LIFE


You have the opportunity to purchase additional Term Life and Employee and Spouse/Domestic Partner
