of Canvasser and represent me during the conduct of the voting and canvassing
of votes. You are finally required to furnish me a certified copy of the certificate
of votes of candidates signed and thumb marked by all the members of the Board
of Election Teller/ Special Board of Election Teller/ Baranggay Board of
Canvasser. Lastly, you also confirm that you are not disqualified under the
second paragraph of Section 178 of the Omnibus Election Code.
