de la pression ventriculaire est beaucoup plus prolonge aprs linjection de sang
quaprs linjection de srum physiologique (redessin daprs Kroin et al.[17]).
Par ailleurs, en accord avec la loi de Monroe-Killie, qui impose que le volume
des trois compartiments reste constant, si le volume du LCR diminue, le volume
vasculaire augmente; la baisse du volume de LCR entrane une vasodilatation
crbrale rflexe, mdie par le trijumeau, entranant une augmentation linaire
du dbit sanguin crbral visant restaurer le volume intracrnien normal[20].
Cette augmentation compensatoire de volume se fait sur le versant artriel et
surtout veineux (aspect de pachymningite, tmoignant de la veinodilatation).
De plus, la gne la circulation veineuse au niveau de la face infrieure de
lencphale, entranant une hypertension au niveau des circulations veineuses
drainant les structures situes sur la base du crne, ophtalmique ou auditive par
exemple, pourrait expliquer une partie de la symptomatologie associe, comme
les altrations de la circulation veineuse ophtalmique[21].

Enfin, la traction sur les vaisseaux mnings de la convexit peut tre respon-
sable de ruptures vasculaires, particulirement veineuses. Il pourra se produire un
hmatome intracrnien, sous-dural, ou extra-dural exceptionnellement (Figure2).

4. Diagnostic diffrentiel
Devant un tableau typique dans les jours qui suivent une brche mninge,
si lexamen neurologique est normal, le diagnostic est a priori vident et aucun
examen complmentaire nest indiqu pour le confirmer. Quelques trs rares
situations posent problmes et justifient dune dmarche diagnostique prcise,
car, toute cphale aprs une brche mninge nest pas obligatoirement lie
une hypotension du LCR[30,31].
4.1. Mningite
Lexistence dune hyperthermie impose dliminer une mningite iatrogne.
Il peut tre parfois ncessaire de refaire une ponction lombaire pour totalement
carter cette hypothse.
4.2. Thrombophlbite crbrale et clampsie
Chez une parturiente, la moindre atypie clinique doit faire voquer une
thrombophlbite crbrale qui sera diagnostique par une IRM[30,31]. Dautres
situations diagnostiques difficiles peuvent tre lies la prsence de cphales
dans le cadre dune (pr) clampsie. Une vritable enqute diagnostique doit
alors tre ralise (Figure 4).
4.3. Pneumencphalie
La pneumencphalie est secondaire linjection dair au cours dune anes-
thsie pridurale ou dune injection intrathcale [32]. La cphale est prcoce
en 24h, intense, non posturale, souvent associe une mydriase unilatrale.
Les pisodes convulsifs ne sont pas exceptionnels, souvent prcds par une
altration de la vision. Liconographie met en vidence les bulles dair, qui peuvent
tre sous-durales ou ventriculaires. La gurison spontane est la rgle dans les
48h, aide par lapport doxygne par voie nasale.
4.4. Hmatomes
La brche mninge peut se compliquer dun hmatome sous-dural, sous-
arachnodien ou intracrbral. La cphale nest alors pas lie lhypotension
du LCR. Le diagnostic est indispensable[14,15].
5. Traitement prventif
5.1. Aiguille et biseau
Sil nest plus ncessaire en Anesthsie de parler du diamtre et du design
de lextrmit de laiguille, ce problme nest pas encore rsolu dans les
services de Mdecine et dUrgences. Il existe des relations entre diamtre de
laiguille, design de son extrmit et incidence des cphales[33]. Les aiguilles
biseau de Quincke, tranchantes, ralisent une brche nette, nentranant pas
de raction inflammatoire ni de dbris de dure-mre, alors que les aiguilles non
tranchantes ralisent une vritable explosion de la dure-mre, gnrant une
raction inflammatoire intense, avec des fragments de tissu qui obstruent la
brche, acclrant la cicatrisation.
5.2. Orientation du biseau
Avec une aiguille de Quincke, il faut que le biseau soit parallle et non
perpendiculaire laxe du rachis. Cette recommandation, tablie sur une hypo-
thse errone de lorganisation histologique des fibres de la dure-mre [34],
reste cependant cliniquement valide. Une mta-analyse rcente en rappelle la
pertinence[35]. Le mcanisme serait en relation avec les modifications de la
taille de la brche en fonction des mouvements du rachis[36].
5.3. Rinsertion du mandrin
Avant le retrait de laiguille, la rinsertion du mandrin permet de rduire
lincidence des cphales de faon importante (16,3% versus 5,0%, p<0,005).

dose maximale. Des accidents convulsifs ont t rapports avec des doses de
500mg par 24h. En pratique, la cafine ne constitue pas un traitement adapt
de la cphale.
6.6. Tramadol et triptans
Il nexiste pas de justification du tramadol dans ce contexte, et ses effets
secondaires possibles (nauses et vomissements) le contre-indiquent.
Plusieurs cas cliniques rapportent lutilisation du sumatriptan dans le trai-
tement des CSBM. Aprs ltude contrle de Connely (versus srum sal) ne
montrant aucune amlioration de la cphale aprs administration sous-cutane
de sumatriptan[41], nexiste pas dindication aux triptans dans cette pathologie.

7.4. Gabapentine
Une publication de 2cas cliniques de cphales post-analgsie pridurale
rebelles au traitement antalgique classique, rapporte lefficacit de la gabapen-
tine (400 mg x 3jours pendant 2jours). La cphale stait amliore dans les
24heures, et le recours au blood-patch na pas t ncessaire[54].

Ces modifications des pressions entranent des modifications immdiates
de lhmodynamique intracrnienne, comme montr chez lanimal (Figure6)[17],
et mesurable sur lhmodynamique de la veine ophtalmique chez lhomme[21].
Cet effet entrane la disparition rapide de la cphale, parfois au cours mme de
linjection du blood-patch.
