1 Access work areas
Uneven surfaces, material in area

Use correct paths to access work fronts
Remove excess materials in area
Dispose of unwanted materials
Notify supervisor if remediation work is required
weather affected surface (erosion, water, dust) Avoid walking through excessively affected wet areas
Notify supervision if potholes and erosion requires rectification and make area safe before leaving
Stairs, varying ground levels Use three points of contact when using stairs
Use designated paths where provided
Ensure surface is in a satisfactory state to be used as access, if not notify supervisor
2 Load and Transport materials Manual Handling Injury Plan all manual handling lifts (path to be travelled, within personal capabilities)
Use the correct manual handling techniques
Dual person lift
Use mechanical aides
Other Traffic and Pedestrians Use spotter if required
Communication (hand signals, voice, radios, signage)
Forklift operator to have a current licence and site VOC
Site Conditions Inspect route and ensure clear and safe for access
Drive to the conditions
Unstable Loads Ensure loads are correctly packed for balance
Tie load down
Ensure loose material is guarded from dislodgement
Crush Injuries and Pinch Points Ensure that all personnel have their hands and feet clear of all loads
Plan how the load is to placed before commencement of lift, communicate this to all the work crew
Ensure that the load is landed safely without any part of body under any part of the load
3 Erect/Modifying Scaffolding Manual Handling Injury Plan all manual handling lifts (path to be travelled, within personal capabilities)
Use the correct manual handling techniques
Dual person lift
Use mechanical aides
Falls from Height All personnel MUST be trained, deemed competent and VOCed
Ensure that a WAH permit has been issued and the controls and restrictions are in place
Ensure that the harness is fitted correctly, quarterly inspected and tagged and inspected prior to use
100% hook up must be used whilst working at heights
Lanyards must be hooked on overhead where possible
Do not hook directly onto grid-mesh and handrails
Ensure anchor point is capable of withstanding 1.5ton for single person, 2.2ton for two people
Use beam straps with protection in place on sharp edges
Do not exceed 30 radius for inertia reels
Static lines and Inertia reels to be installed by trained personnel
Dropped Objects Tool lanyards are to be used
Barricading to be erected for drop zones and to isolate area
Ensure information tags are placed on barricading
Scaffold Collapse/Instability Competent and Licensed scaffolders a current site VOC to be completed
Scaffold to be built off stable structures
Scaffold to be built on level ground away from penetrations and excavations
Crush Injuries and Pinch Points Ensure that all personnel have their hands and feet clear of all loads
Plan how the scaffold components are to be placed before commencement
Gloves and PPE to be worn for all manual tasks
PPE must fit correctly and be appropriate for the task
Ensure housekeeping is maintained
