Madam:

The SJCPRISAA Meet 2016 will be holding on October 25-27, 2016. This sports competition will be
participated by all Private Elementary Schools here in San Jose City. Being one of the members of the
said sports event, I am requesting for our school participation on this competition.

This is to promote our school and to expose our athletes into a greater challenge and to develop their
ability in the sports of their best.

In line with this, we the coaches and students athlete extend our sincerest appreciation and profound
gratitude for our financial support and valuable contribution toward the success of our school in this
annual event.

Attachments herewith, are the itemized expenses and its resources together with the list of players
expenses for the said event.
