I am writing this letter with regret to inform you that the father of Meilanie Joy B. Pascual, a
4th year student under BSBA Marketing Management, Joseph C. Pascual passed away last
April 20, 2017.

The loss that they are facing cannot be fulfilled by anything, but since we are accorded with
the death aid policy, the request for financial assistance can at least help with the mourning
without any difficulty. I have attached all the required documents, Form 5 and School ID of
Ms. Pascual and the Death Certificate of his Father for your perusal.

Kindly issue the claim amount on Ms. Meilanie Pascuals name, and if possible kindly let us
know when we can come to your office to collect the check for the compensation amount.
