This letter is in regard to my VUL application in your company. I was told by my agent, John Lester M.

Arcipe that you are requiring me to undergo full medical examination regarding my Asthma. I want you

to know that the last time my Asthma attacked was 7 years ago and I forgot already the name and

location of my attending physician. I am currently in a good condition, not even taking maintenance,

visiting hospital and I avoid activities that would trigger the asthma.

Rest assured that all the details I declared in RESPIRATORY DISORDER QUESTIONNAIRE are honestly

correct.

I am willing to undergo medical examination but I cant accommodate it immediately due to my busy

schedule in office.

I am asking for your consideration to approve my application without medical examination if it is really

possible to avoid hassle in our part.
