Answer :
There is a patient named Mr Peter Cathcart , he was admitted because of excessive ketone
production leading to suspected ketoacidosis and this is potentially life threatening condition.
the symptoms he showed are abdominal pain, vomiting, rapid breathing, extreme tiredness,
drowsiness. Me as a nurse needs to take action such as I need to monitor blood sugar levels give
insulin injections if the blood sugar levels in two high so the glucose can be decrease, and then
dietician to discuss diet so he can control his food intake. After that, patient will be shown how
the self-monitoring of blood glucose. (SMBG) so when the blood glucose increases he can call
nurse or doctor and ask then to take action. And then, demonstrate to patient how to read SMBG
(self monitoring of blood glucose) stripe so he can understand what is shown on the monitor.
After that, Link SMBG to diet so he knows what food can be eating normally and the other
which he cannot eat because it will make impact to the SMBG and his blood sugar levels. In the
end, I need to Emphasizes the importance of monitoring blood sugar levels every morning and
make a note to take to the GP results.

When I start a new shift, I listen to the nursing report from the previous shift. The report
is about the patients condition and nursing interventions that has been done for each patient. I
observed a case of Mrs Anna Rumangit, she is 65 years old. In the morning, I was assigned in
Policlinic on June 17th 2014, 09.30 a.m. she came with headache her primary complaint since a
week before she came to the hospital. She was felt at the bathroom and got injury on behind her
head since a day before she had headache. Patient looked pale on face, weak of the body, swollen
on her head, and hold the pain on her head. So that, her pain got 1-3 scale (mild pain). Doctor
diagnosed that she had mild head injury. I did vital signs examined. After that, I got blood
pressure 130/90 mmHg, Pulse 72 x/minute, respiration rate 20 x/minute, temperature 37,5o C.
The patient looked swollen on behind her head. I suggested Mrs. Anna to do relaxation
treatment. I helped Mrs Anna lay down on the bed with semifowler position. And then, I did to
collaborate with doctor for gave analgetic drug that is Na Diklofnil.

gastritis. I did vital signs exams I got blood pressure 110/70 mmHg, respiration rate 21 x/minute,
Pulse 80 x/minute, temperature 37,5o C. I suggested the patient to manage his food system so
that he could heal faster. I did to collaboration with doctor for gave the drugs to his.

2014 with mandrill pain on feet and hand as her primary complain. She looked pale and weak,
she said the pain mostly happen in night, and she cant slept finely, she had paralyzed on right
side. Then I did vital signs examined. i got blood pressure 149/90mmHg, respiration rate
22times/minutes, pulse pressure 90times/minutes, temperature 37oc. Doctor diagnosed that her
had Gouth Atritis , I suggested she may not ate peanuts contain, so gouth atritis can occur her
again, she must bed rest exactly, and I did to collaborate with doctor for gave the drugs.
