FOREWORD

1. PURPOSE

This Advisory Circular has been revised to include current civil aviation safety
regulations and procedures. This AC provides information and guidance for Operators,
AMOs and Manufacturers to prepare Service Difficulty Reports (SDR).

2. REFERENCES

This Advisory Circular contains advisory material only and should be used in
accordance with the applicable regulations.

3. CANCELLATION

Advisory Circular 43-02, Service Difficulty Reports, dated June, 1999, is now
cancelled.

4. REVISIONS

Revisions of this Advisory Circular will be approved by the Director General Of Civil
Aviation.

1. PURPOSE

This Advisory Circular provides information and guidance to operators, aircraft


maintenance organizations, and manufacturers concerning the requirements and
procedures for making Service Difficulty Reports (SDR) related to Indonesian
registered aircraft and leased aircraft operated by an Indonesian operator. Also
included is a detailed list of reportable occurrences, and sample SDR forms.

2. RELATED CIVIL AVIATION SAFETY REGULATIONS (CASRs)

- CASR Part 121 and 135 require reporting by certificate holders or operators.

- CASR Part 145 requires reporting by approved maintenance organisations.

- CASR Part 21 requires reporting by manufacturers.

3. BACKGROUND

CASR Parts 21, 121, 135 and 145 define the requirements for reporting service
difficulties. The Service Difficulty Report information is required for the following
reasons :

a. To enable the DGCA Directorate of Airworthiness Certification to become aware


and participate in the analysis and prevention process of serious defects,
malfunctions, failure and occurrences experienced by operators while operating and
maintaining Indonesian registered aircraft or aircraft leased by an Indonesian
operators.

b. To enable the DGCA to comply with the provisions of ICAO Annex 8 with respect to
exchange of information with other contracting states related to the continued
airworthiness of aircraft.

4. DEFINITIONS

a. Service Difficulty (SD)


A malfunction, defect, failure or occurrence experienced during operation or
maintenance of an Indonesian registered aircraft or a leased aircraft operated by an
Indonesian operator or other entity that affects or could affect the airworthiness of
the aircraft or safety of the flight crew, or ground personnel.

b. Service Difficulty Report (SDR)


A mandatory notification submitted to the DGCA by an operator or approved aircraft
maintenance organization (AMO) or manufacturer containing specific information
about a Service Difficulty experienced or discovered.

5. MANDATORY REPORTING REQUIREMENTS

a. To comply with the mandatory SDR requirements, the operator or approved aircraft
maintenance organization or manufacturer should assign a senior staff person from
engineering or quality control to be responsible for submitting the reports.

b. A DGCA approved form must be used when submitting an SDR. Appendix B
provides such a DAC form 43-01.

c. The SDR form must be submitted within 72 hours of the time of occurrence or
discovery. If submission will be delayed, past the 72 hour limit the DGCA must be
notified by electronic mail or facsimile of the information available at the time. In no
case should the SDR be submitted later than 7 days after occurrence or discovery.

d. Any additional information that may not be available within 72 hours of the
occurrence or discovery and may take considerably more time to collect or develop,
e.g. tear down or test reports, previous AD or Service Bulletin related data,
recommendations, and conclusions. This additional information shall be submitted
as soon as it is available.

e. Operators or approved aircraft maintenance organizations or manufacturers must


use the same identification code number for each SDR. These codes are found in
the DGCA Civil Aircraft Register and Shop (Aircraft Maintenance Organization)
Registry. This coding includes a 5 digit operator identifier and a sequential individual
SDR number (refer to Appendix B-item 2). For example:
GA 001/89/3 is Garuda Indonesia 3rd SDR submitted in 1989.
58099/89/7 is Universal Maintenance Centers 7th SDR submitted in 1989.

f. Although an SDR is not required for simulation of system or component failure


conditions for testing or training purposes, if a failure, defect, or malfunction does
occur during such operations, an SDR is required. For example, if during training of
cabin attendants, an emergency evacuation slide fails to operate, an SDR is
required. Likewise, if during a check, the landing gear emergency extention system
fails to operate, an SDR is required.

g. Those SDs occurring or discovered during accomplishment of DGCA Airworthiness


Directive requiring special inspection shall be also reported.

h. Always enclose any original pilots reports, engineers reports with the SDR as first
hand information is essential to evaluation and analysis.
Often drawings, photos, photocopies of manuals, or other information can also help
to describe a service difficulty and should be enclosed with the SDR.

i. The SDR or notification of late submittal shall be mailed or sent by electronic mail or
facsimile to the DGCA at the address mentioned on the SDR Forms.

6. RETENTION OF SDR RELATED COMPONENTS AND PARTS

In the event the nature or cause of an SD is not clearly evident, the operator or
approved maintenance organization or manufacturer shall retain the item and keep it
protected and preserved from damage and deterioration until such time as agreed with
the DGCA.

7. RETENTION AND TRANSFER OR SDR RECORDS

Refer to CASR Parts 21, 121, 135 and 145 for certificate holders to keep records of
SDRs and transfer them under certain conditions.

APPENDIX A. DETAILED LIST OF REPORTABLE SERVICE DIFFICULTIES

A. Malfunctions or Failures of Aircraft Systems, Engines, Components, or


Equipment Concerning:
1. Engine in-flight-shut-downs (IFSD) including intentional shutdowns or significant
power loss;

2. Uncontrollable operation of propellers, main rotors, or tail rotors;

3. Malfunction of any flight control or engine control system;

4. Failure of landing gear to retract or extent, or to support the aircraft;

5. Failure of wheels, brakes or tires;

6. Failure or malfunction of an electrical, hydraulic, or pneumatic system resulting in


loss of that system to the aircraft;

7. Loss of control of cabin pressure;

8. Asymmetrical or uncommanded thrust reversal;

9. Multiple or serious malfunctioning of avionics, indicating, warning, radio or


navigation system resulting in hazardous flight information , e.g. false fire warning,
or excessive errors of instruments;

10. Taking emergency action during flight resulting in precautionary or forced landings,
rapid descent, use of emergency systems, equipment or modes of operation, or
emergency procedures as listed in the approved aircraft flight manual;

11. Fires (including engines), explosions, smoke or toxic gases during maintenance,
ground, or in-flight operation;

12. Significant leakage or flow interruption/exhaustion of fuel, hydraulic fluid, or oil;

13. Servicing with incorrect fluids;

14. Failure or malfunction of safety equipment to operate, i.e. slides, rafts, fire
extinguisher, oxygen breathing equipment and emergency locator transmitters.

B. Structural Damage:
1. Separation of any aircraft, engine, or propeller part during in-flight or ground
operation;

2. Failures, cracks, deformations, or corrosion of structural parts including accidental


damage which exceeds maximum allowable continued in-service limits of the
manufacturer.

C. Operational Occurrences - Conditional Inspections:
1. Lightning or bird strikes, hard or over-weight landings, turbulence, overspeed flight
conditions, and on-aircraft hazardous material spillage that may affect airworthiness
and require a conditional special inspection;

2. Rejected take offs for either actual or for false warning indications of malfunctioning
systems or components.

3. Return to base for any reason.

4. Any failure of Emergency evacuation system or component including exit doors that
fail to perform the intended functions during an actual emergency training testing,
maintenance, demonstration or inadvertent deployments.

APPENDIX B INSTRUCTION FOR COMPLETING THE SERVICE
DIFFICULTY REPORT DAC FORM 43-01

A. For Organizations submitting the SDR

1. Enter the name of Operator/Approved Maintenance Organization/


Manufacturer.

2. Enter the SDR Control Number as specified in section 5e.

3. Enter the aircraft system and subsystem, respectively by the numerical


symbols (ATA spec. 100).

Example: Engine Cylinder 7230 enter only 7230.

4. Enter the registration mark shown on the aircraft such as PK-GAC, N-


953AE.

5. Enter the stage of flight or ground operation the aircraft was engaged in
when the reported failure, malfunction, or defect occurred or was observed.

6. Place where the difficulty occurred such as Jakarta, Medan, Jeddah.

7. AIRCRAFT:
- MANUFACTURER: Enter the aircraft manufacturer's name. Any
meaningful abbreviation will be acceptable.
Example: Beech, Cessna
- MODEL/SERIES: Enter aircraft model as identified on the aircraft data
plate.
Example: 172A, 180
- SERIAL NUMBER: Enter the serial number assigned by the
manufacturer.
Example: 81RK, 94RK

POWERPLANT:
- MANUFACTURER: Enter the engine manufacturer's name. Any
meaningful abbreviation will be acceptable.
Example: Lyc., Cont.
- MODEL/SERIES: Enter engine model as identified on the engine data
plate.
Example: IO-540, O-470R
- SERIAL NUMBER: Enter the serial number assigned by the engine
manufacturer.

PROPELLER: Complete only if pertinent to the problem being reported.
- MANUFACTURER: Enter the manufacturer's name. Any meaningful
abbreviation will be acceptable.

data sheet/propeller specifications.
Example: DHCC2Y, M74CC
- SERIAL NUMBER: Enter the serial number assigned by the propeller
manufacturer.
Example: D800, D900

1. APPLIANCE/COMPONENT (Assembly that includes part):


- Comp/Appl Name: Enter the manufacturer's nomenclature for the
component or appliance of the specific part causing the problem.
Example: engine, starter
- Manufacturer: Enter the part manufacturer's name.
Example: Lyc., Lear
- Model or Part No.: If supplied by the manufacturer.
Example: 0-362YK-1, 0-473GH-2
- Serial Number: If supplied by the manufacturer.
Example: CH9693, DE8549

1. SPECIFIC PART (of component) CAUSING TROUBLE:


- Part Name: Enter the name of the specific part causing the problem.
The appliance or component is the assembly which includes the part.
For instance: When the part is a burned wire, the component would be
the system using the wire, such as VHF communication system. When
the part is a bearing, the appliance should be the unit using the
bearing, such as starter, alternator, generator, etc. When the part is a
stringer, the component name should be fuselage, wing, or stabilizer,
etc.
Example: bearing, wire, etc.
- MFG. Model or Part No.: Enter the manufacturer's part number.
Example: 14542, 23893
NOTE: If same as aircraft engine, or propeller, leave blank.
NOTE: If the aircraft, engine, or propeller manufacturer is the
component manufacturer, leave blank.
- Serial No.: Enter the serial number assigned by the manufacturer.
Example: W5489, etc.
- Part/Defect Location: Enter the location.
Example: left half, right wing

1. Part TT: Enter the service time of the part in whole hours. (If Part TT is
unknown, use aircraft, engine, propeller, or appliance/component total time,
whichever is applicable);
Example: 02756, 04278

2. Part TSO: Enter the service time of the part since it was last overhauled, in
whole hours. (If part TSO is unknown, use an aircraft, engine, propeller, or
appliance/component time since last overhaul, whichever is applicable.)
Example: 00351, 00427

3. Part Condition: Enter the word(s) which best describe the part condition.
Malfunction, Corroded, Failed, Cracked, Chafed, Missing, Misassembled,
Leaking, Short Circuit, Scored, Contaminated, Incorrect Part,
Disconnected, Open Circuit, Deflated, Burnt, Discharged, Jammed

4. Date Occurrence: Enter the actual date of the occurrence, day. Month,
year.

5. Enter name of who has submitted the SDR.

6. Enter title.

7. Enter phone or facsimile number.

8. Date Submitted: Enter the date of submission, day, month, year.

9. Comments The text should identify and describe the cause of the
malfunction, failure or defect. It should contain descriptive information
concerning the part/component that caused the difficulty, inspection
findings and corrective action taken to prevent recurrence. If additional
space is required to give the complete story use the reverse side of the
form. Make reference to any attachment such as pictures, parts, etc.

B. For DGCA use only

19. Enter the date the SDR was received.

20. Enter the DAC control number as follows:


First two digits - year that SDR is recorded
Second two digits - month that SDR is recorded
Third two digits - day that SDR is recorded
Four last digits - serial sequence number of the SDR system
i.e.: 0209150001.

21. Enter status of SDR Open or Closed


Open - Check this block if the report is not complete and there is more
information forthcoming.

Closed - Check this block if the report is complete and there is not
expected to be any further information on this particular
occurrence.
Supplemental reports are entered as Supp.#1 open if there is
more information to follow or Supp. Closed indicating that this is
the final report.

23. Sign the SDR form when the SDR is closed.
