STATEMENT OF CERTIFICATION
I certify under penalty of law that the Stormwater Pollution Prevention Plan (SWP2 Plan) has
been completed, retained on site and implemented at this facility. The SWP2 Plan and
attachments were completed under my direction or supervision in accordance with a system
designed to assure that qualified personnel properly gather and evaluate the information
contained in the SWP2 Plan. Based on my inquiry of the person, or persons, who manage the
site, or those persons directly responsible for gathering the information, the information
contained in the SWP2 Plan is, to the best of my knowledge and belief, true, accurate and
complete. I am aware that there are significant penalties for providing false information,
including the possibility of fine and imprisonment.

from this site.

4. FACILITY DESCRIPTION
The facility is a permanent dry batch concrete plant. Makeup water is from the city of Anywhere =s potable water
supply system. Wastewater is generated by the washing of the inside of the truck mixer drums and the outside of the
trucks. The wash water is collected in an earthen basin. There is generally no discharge from the basin. Settled
solids are periodically removed and used as road / subgrade base material. Domestic wastewater is routed to a septic
tank / lateral field.
Process water will be contained in the centrally located pond located in the area east of the batch plant facility. The
process water pond is dependent upon both evaporation and percolation to maintain current conservation level. As
can be expected, the accumulation of fines in the process water pond will have to be excavated from time to time and
the resulting material will be hauled to an acceptable landfill. The pond is complete with a berm to allow for a two-
foot freeboard in case of excessive rainfall.
Storm water has been analyzed and one potential discharge point has been considered. The point is located in the
extreme Northwest corner of the property. This location takes advantage of natural drainage and will cause the least
amount of disturbance to the site. The improvement in the Northwest property corner consists of a grass barrier strip
bounded on the downstream side by an earthen berm. The outfall consists of an 18 reinforced concrete pipe with a
4 weir installed for sampling purposes. The barrier strips will be sprigged with drought tolerant Northern Bermuda
grass with a straw/hay cover until the grass is established. Monitoring will be as required by the permit.
The base drainage map with proposed improvements along with the process water pond is complete. The log of
admixture usage is ongoing and is logged monthly.
5. MAPS

buildings, entrances/exits, berms to keep stormwater off the site, water pollution
control structures, Best Management Practices needed to implement the SWP2 Plan,
waste concrete piles, warehouses, any areas where left over concrete is dumped,
where trucks are washed, etc and the normal stormwater flow direction across the site
is provided.
