Summary

Hatfield rail crash took place on 17 October, 2000 at Hatfield, UK.


4 people died and more than 70 got injured.
Cause of accident was metal fatigue of rail which induced derailment.
Major changes in UK railway system took place after investigation of
accident. These include speed limit reductions and tightening of
health & safety procedures.
2005, Network Rail and contractor Balfour Beatty were found guilty of
breaching health and safety laws and were penalized.

Incident Details (When, How and Where)

Incident took place on 17 October 2000 at 12:23(UTC) at Hatfield,


Hertfordshire, UK.
This accident was due to mental fatigue and derailment.
Reports says that there was a lack of communication and some staff
were not aware of maintenance procedure.
Aftermath of accident saw:
Widespread speed limit reduction throughout the rail network.
Tightening of health and safety procedures.
Network Rail and Balfour Beatty were found guilty of breaching health
and safety rules.

Main Cause

Preliminary investigation found a rail had fragmented as trains


passed .
Rolling Contact Fatigue.
Multiple surface-breaking cracks.
Such cracks are caused by high loads where the wheels contact the
rail.
Repeated loading causes fatigue cracks to grow. When they reach a
critical size, the rail fails.

Other Causes & Facts

Underlying cause identified by the HSE investigation:


The maintenance contractor, Balfour Beatty Rail Maintenance Ltd
(BBRML) failed to manage effectively the inspection and maintenance of
the rail at the site of the accident.
Infrastructure controller, failed to manage effectively the work of BBRML
and failed to implement an effective rail renewal operation.
The speed and manner in which the train was driven were not factors
in the derailment.
No evidence that:
Signals in the vicinity were disobeyed
The signaling system had failed.
Vandalism or an act of terrorism contributed to derailment.

Consequences (Casualties)

Robert James Alcorn, of Auckland, Newzeland


Four passengers died Steve Arthur, from Pease Pottage,
in the accident. Leslie Gray, of Tuxford Nottinghamshire
Peter Monkhouse, of , Leeds
Those who died were all in the restaurant coach.
33 were initially reported as injured.
The number of injured was later revised to over 70.
03 were seriously injured.
Two of those who seriously injured were GNER staff working in the
restaurant coach at the time of the accident.
Emmerdale (British TV Series) actress Anna Brecon was travelling on
the train, and suffered minor cuts and bruises.

Consequences (Revenue Loss)

Train operating companies were adversely affected by the disruption.


Lose an estimated 19% of revenue in the year following the crash.
Freight operator EWS (English, Welsh & Scottish Railway) was
cancelling up to 400 trains per week as a result.
Estimated resultant losses at 1 million per month.
The cost to the entire UK economy of the disruption was estimated at
6 million per day.

Court Case

In 2003 five managers, Network Rail and Balfour Beatty that


maintained the rail,charged with manslaughter and breach of health
& safety charges but managers denied the charges.
In 2004 man-slaughter charges were dropped but other charges
stood.
In 2005 trial began and judge acquit all on manslaughter charges.
Afterwards Balfour Beatty changed its plea to guilty on health and
safety charges.
Network Rail were fined 3.5 million while Balfour Beatty were fined

Remedial Measures - I

To improve maintenance and reliability of assets, efforts to eliminate


fish-plated joints by welding single rails into long welded rails
There should be a progressive use of Tie Tamping and ballast
cleaning machines for track maintenance.
All rails and welds must be ultrasonically tested as per laid down
periodicity. Self Propelled Ultrasonic Rail Testing (SPURT) should
be used for Rail flaw detection.
Design of passenger coaches with features of crashworthiness to
absorb most of the impact energy, should be developed and coaches
are being manufactured accordingly.

Remedial Measures - II

To enhance electrical safety of coaches, only halogen free, fire


retardant, low smoke e-beam irradiated cable should be provided in
new coaches
unmanned level crossings should be closed by elimination,
merger/diversion to the nearby level crossing and construction of
Subways.
Vigilance Control Device (VCD)should be used it is a system where if
the driver does not perform a certain set of actions over a period of
time, sound alarms, flashing light and brake systems start operating
automatically.

Remedial Measures - III

The Rails and other materials of Rolling Stock should be used upto
Design Life. These components should be replaced or may be used
after proper rehabilitation after the Design Life.
The Components of Rolling Stock should be maintained using Proper
Maintenance Procedures.
The Operating Speed of Rolling Stock should limit according to Design
Speed using Strict Laws and Regulations.
Overloading of Rolling Stock should be prevented. Overloading can
cause Rail Track components to fail quite early before the design life.
The Communication and Maintenance system should not be confusing
so as create any ambiguity that can cause accident.
