AFFIDAVIT FOR OBTAINING INCOME CERTIFICATE

I M.Bal Raju S/o Late G.Hari, aged about 40 years, occupation Daily Wage worker, R/o Adikmet, Vidya Nagar, 1-9-395/D Jandagalli,
Musheerabad Mandal, Hyderabad, 500044. Ph.No 9392443182, do hereby solemnly affirm and declare as under:-

I am the Deponent here in and as such acquainted with the facts deposed below.

I submit that my forefathers and family members, all belong to B. C. Community sub-caste is Mudiraj, Group D, No 19. I am a daily
wager and my monthly income is Rs. 4000/-

I submit that I am residing at the above said place, for the last 25 yeas with my family. That I have been resident at the above said
address since 1990. The property is rented & I am paying rent of Rs.1500/- per month. I am the tenant of the said property. The said
property contains two rooms.

Income Details :-

Yearly Income :- 48,000/-

Monthly Income:- 4000/-
Daily Income:-160/- for 25 days.

No. of Rooms:- 2 Rooms

That Income Certificate is required for submission in office of Board Of Intermediate Examination as My Son G. Shiva Prasad aged 16
years, is pursuing Inter 1st year, in Jahnavi Junior College, Narayanaguda, Hyderabad.

I therefore request the MRO, Musheerabad Mandal to please issue Income certificate in the name of my self for the Intermediate
education of my son G. Shiva Prasad, for the purpose of education/Scholarship , to submit in his College.

What all stated above is true and correct to the best of my knowledge and belief and hence this affidavit.
