Helen G. Abdon, MBA

Student Discipline and Formation Office
La Salle University

Dear Maam Abdon:

I, Clifford Jan Tabada Atig, a 4th year architecture student (11356836), am writing to you with
respect to my student identity card which I lost yesterday morning around 6:30 a.m. when I was
on my way to campus. I do not know exactly how or where I lost it since I was in a hurry for my
7 a.m. class.
I have been keeping that I.D. for the rest of the school year until yesterday. Without it, surely, I
will be facing a lot of difficulties since that I.D. offered me a lot of student privileges especially
during my public bus travels. I already asked the guards if someone saw my I.D. and
surrendered it to them. But unfortunately, they informed me that no one reported the missing
card.
Since I lost my I.D., I humbly ask for a promissory pass even until this Monday (Feb. 27, 2016) so
that I can enter the school without question. In that given span of time also, maybe I will find it,
hopefully. I claim responsible for my actions. If ever my I.D. will not be found, I am ready for the
consequences. Thank you.

Respectfully,

Clifford Jan T. Atig
