0740 - Pumps The fire main is damaged within the #5 ballast water
tank causing the ballast water tank to overfill and drain
through the tank vents and sounding tubes on the main
deck.
0940 - Ballast fuel and other tanks The vessel's emergency generator fuel shut-off valve is
inoperable, meaning the fuel supply is not capable of
being closed from a safe position outside of the
emergency generator space in the event of a fire.

made to the #4 starboard fuel oil tank in way of frames
72-73.
1420 - Cleanliness of engine room PSCO discovered severe oil leaks throughout machinery
in the engine room (including the starboard side of the
main engine, the heavy fuel oil pump, and the
bunkering lines into the engine room) leading to oil
soaked lagging and creating a significant fire hazard.
2510 - Safety and environmental Evidence discovered during an expanded ISM exam
policy revealed the company/vessel failed to implement
applicable requirements of the Safety Management
System (SMS). The emergency generator is inoperable,
the port side engine room ventilation fan is inoperable,
the #2 heavy fuel oil purifier is inoperable and the
heavy fuel oil pump is leaking a significant amount of
oil. The #2 and #3 generators will not operate unless
additional means of cooling is provided. There is also
oil soaked lagging on machinery throughout the engine
room creating a significant fire hazard. Request an
external audit of the vessel's safety management system.

0635 - Launch arrangements for Upon inspection of the davit and winch system for the
rescue boats rescue boat, the PSCO was informed by the Chief Mate
that the davit system was not operational. The vessel
was unable to safely lower or recover the rescue boat
during an operational test.
0615 - Rescue boats Rescue boat was not in working order and ready for
immediate use. During operational test of the rescue
boat, the crew was not able to start the outboard motor.
The rescue boat is required per the Safety Equipment
Certificate.
