installation system set to manual mode at the local and remote
activation stations, contrary to the manufacturer's
operating instructions. This arrangement would prevent
the system from operating automatically as designed
and approved. The Chief Engineer stated that the main
fire fighting system for the engine room was the foam
system which would discharge automatically; however,
the foam fire fighting system's operating manual
indicated that they system must be activated manually.

2525 - Masters responsibility and The U.S. Coast Guard attended the vessel on October 6
authority to determine the Masters condition after receiving a
report from pilot onboard that the Master and
crewmembers appeared to be intoxicated. The Coast
Guard met with the Master to discuss the purpose of the
visit. Upon investigation by the Coast Guard an
alcohol test (Breathalyzer) was performed by the Coast
Guard, which indicated a blood alcohol content (BAC)
of .1 (U.S. legal limit .04/STCW & SMS .05 BAC). The
Coast Guard has determined the Master to be
intoxicated and unfit for duty to perform essential
shipboard procedures as it pertains to the Masters
Position and responsibilities. Based on the Masters
intoxicated state, the Coast Guard does not consider the
vessel to be sufficiently and efficiently manned in
accordance with the Safe Manning Document.
Additionally, the PSCO reviewed the vessel's safety
management system documentation and found Form
OA-O4B where the Master signed that he was aware of
Companies Drug and Alcohol Policy. Prior to
movement, the vessel shall be provided with a
certificated Master to the satisfaction of the
Administration.

1720 - Control of discharge of oil PSCO discovered evidence indicating that a flexible
hose arrangement was fitted between the discharge side
of the marine diesel oil pump and the oily water
separator (OWS) three way valve, bypassing the
installed OWS and oil content meter (OCM) and
allowing the oil and/or oily water mixtures to be
discharged directly overboard into the sea. PSCO also
discovered the presence of heavy oil in the overboard
discharge valve downstream of the OWS/OCM sampling
supply connection. This unapproved method does not
permit the crew to monitor the content of effluent to
ensure the overboard discharge does not exceed 15 parts per
million.

0999 - Other (Safety In General) The Master failed to report to the Administration repairs
made to the #4 starboard fuel oil tank in way of frames
72-73.

2550 - Maintenance of ship and Objective evidence discovered in an expanded ISM


equipment exam revealed the following major non-conformities: the
intentional deviation from the company's SMS
environmental protection policy as evidenced by the
bypass of oil filtering equipment and subsequent
discharge of oil directly overboard, an improperly
maintained Oil Record Book Part I as required by
MARPOL Annex I and the vessel's SMS, and failure to
report to the company the hazardous condition
discovered with the #4 starboard fuel oil tank.

excessive wear on the fan belt for the cooling system
causing the generator to overheat. It cannot provide
sufficient power to supply all those services that are
essential for safety in an emergency for the required
amount of time.

1420 - Cleanliness of engine room PSCO discovered severe oil leaks throughout machinery
in the engine room (including the starboard side of the
main engine, the heavy fuel oil pump, and the
bunkering lines into the engine room) leading to oil
soaked lagging and creating a significant fire hazard.

2510 - Safety and environmental Evidence discovered during an expanded ISM exam
policy revealed the company/vessel failed to implement
applicable requirements of the Safety Management
System (SMS). The emergency generator is inoperable,
the port side engine room ventilation fan is inoperable,
the #2 heavy fuel oil purifier is inoperable and the
heavy fuel oil pump is leaking a significant amount of
oil. The #2 and #3 generators will not operate unless
additional means of cooling is provided. There is also
oil soaked lagging on machinery throughout the engine
room creating a significant fire hazard. Request an
external audit of the vessel's safety management system.

protection. The ship's crew also does not properly test
the water spray system prior to cargo operations as
detailed in the vessel's Cargo Operations Manual section
15.15.5

starting during three separate operational tests during
the exam.
