globales, no slo han generado pocas iniciativas sino que, adems, han sido
poco eficaces cuando han intentado hacerlo. El liderazgo de la poltica
sanitaria est en manos del Gobierno y el papel de las organizaciones de
intereses queda limitado a su reaccin poltica.
Esto confirma la hiptesis ya expuesta en pginas anteriores. El sistema
poltico espaol otorga una legitimidad poltica parcial a las organizaciones
de intereses. Les permite reaccionar ante la legislacin, pero no las legitima
para presentar iniciativas legislativas. No son representantes de la colectivi-
dad (de la sociedad) y, por tanto, no tienen legitimidad para la creacin
legislativa. Pero, como representantes de sectores afectados, s tienen le-
gitimidad para presentar sus planteamientos ante las iniciativas de los
tericos representantes de la colectividad (partidos polticos, sistema parla-
mentario, Gobierno). Al dominio poltico del PSOE tenemos que aadir la
especial situacin de liderazgo de la administracin sanitaria en los pasa-
dos aos. A principios de los ochenta, el sistema sanitario necesitaba de
una reorganizacin para evitar su colapso y para adaptarlo tanto a las
nuevas necesidades sanitarias como a las limitaciones financieras. Dado el
gran protagonismo del Estado en el sistema sanitario (como principal
proveedor de cobertura sanitaria y servicios), su liderazgo en los cambios y
reformas sanitarias no es slo lgico, sino tambin necesario. La combina-
cin del papel dominante del Estado en el rea sanitaria con una legitimi-
dad poltica parcial de las organizaciones de intereses hace que su partici-
pacin e influencia poltica vaya a remolque siempre de las iniciativas
gubernamentales y del sistema poltico formal.
En trminos de todo el perodo (1977-1990), tanto la OMC como los
Colegios Provinciales han destacado por su efectividad frenando legisla-
cin desfavorable y, en mucha menor medida, consiguiendo la aprobacin
de medidas legislativas favorables. Su incidencia administrativa y sobre la
orientacin global de la poltica sanitaria ha sido muy pequea. La CESM
es especialmente efectiva frenando medidas polticas desfavorables (tanto
legislativas como administrativas) y su influencia en la orientacin poltica
general ha sido mayor que la de sus aliados polticos (OMC/CPOM). La
efectividad poltica global de la CESM parece muy superior a la de la OMC
y a la de los Colegios Provinciales. La corta vida poltica de la CESM se
caracteriza por un incremento rpido de su influencia poltica y capacidad
de representacin en detrimento de la capacidad de la OMC y especialmen-
te de los Colegios Provinciales. La progresiva importancia de los aspectos
laborales (en detrimento de los de tipo profesional), la mayor aceptacin
por parte del PSOE de los planteamientos y racionalidad de tipo sindical y
la oposicin entre PSOE y OMC favorecen no slo el desarrollo organizativo
y poltico de la CESM, sino la sustitucin progresiva de la OMC en la
representacin poltica de la profesin. La CESM est consiguiendo am-
pliar el mbito y las reas de representacin del colectivo profesional a
costa de la representacin de la OMC. La racionalidad de tipo sindical (y
