RE: ORAL SCREENING

This serves to inform you that an oral screening will carried out by a dentist as follows:

Date: 8 April 2016

Organisation: Tooth Fairy Dental Clinic, Kuching.

This screening will be done in stages of 2 classes per slot to minimize disruption to lessons. A briefing will
be carried out and an oral chart given to each student at the end of the screening.

We hope to seek your support on this matter. Kindly approach the respective class teachers should there be
any queries.
