Scope:
Timely discharge of patient;
To provide patient and family's satisfaction with the care they receive; and
Project To provide effective and efficient systems for multidisciplinary referral, treatment and
description support.
Out of Scope:
Patient who require other consultation and other reasons of current admission before
and after discharge;
Discharge Against Medical Advice (DAMA), Out-on-Pass, and Patient Transfer
Availability of In-patient Discharge Policies and Procedure;
Structured and process of discharge planning system;
Requirements Completion of patient discharge plan and checklist;
75% of team members will attend the bi-monthly meeting (every 2nd and last Tuesday)
Patient and family are active partners of the care process; and
Effective use of hospital resources
By the end of April 2018, healthcare providers (doctors, nurses and allied health
professionals) will ensure timely discharge of patients by adhering to the Expected
Project Objectives Date of Discharge in TrakCare
By the end of April 2018, there will be a safe process of patient discharge by
implementing an Electronic Discharge Journal and following the Administrative Policy
on In-Patient Discharge
Policy and Procedures on In-Patient Discharge Planning;
Discharge Monitoring Evaluation Tool;
Discharge Journal/Electronic Journal/Baseline data
Deliverables Morning doctors' discharge rounds;
Regular discharge assessment by nurses;
Bed utilization reporting system;
Multidisiplinary Collaboration;
Culture-sensitive and effective Discharge Planning Program
Lack of standardized policy-driven discharge planning program
