SUMMARY
The objective of this work was to investigate the role of cytokines in rheumatoid
arthritis (RA) and to compare cytokine profiles, observed in RA, with those in allergic
diseases, such as allergic asthma, atopic dermatitis and hymenoptera allergy.
The intracellular flow cytometric method can identify the cytokine producing cell type
but gives no absolute quantitative measurement of the produced cytokine. In
Chapter III, we investigated whether the technique that determines intracellular
cytokine production is well correlated with extracellular cytokine determination. First,
a flow cytometric technique based on the single cell method was used to identify
intracellular cytokine production in T-lymphocytes and monocytes. In parallel, a
microsphere based flow cytometric immunoassay was performed on the same
samples of RA patients to evaluate cytokine secretion. The data of this study
demonstrated that semi-quantitative flow cytometric detection of cytokines (IL-1, IL-
6, IL-10, IL-12) in monocytes is well correlated with extracellular cytokine secretion.
Measurement of IL-2 and IFN- production performed by enumerating cytokine-
producing T-lymphocytes or by quantification of cytokine in culture plasma, yielded
results that showed a good correlation. For IL-4, moderate correlations between
intracellular and extracellular cytokine levels were observed. Therefore, in RA
patients, there is no secretion deficit of intracellularly produced cytokines, since good
correlations are observed between intracellularly blocked and extracellular cytokine
detection.
These results stressed the need for further investigation of the influence of therapy
on cytokine profiles. Prospective studies were set up to investigate the influence of
therapy on cytokine profiles in T-lymphocytes and monocytes in RA and allergic
patients (Chapter V-VII).
In Chapter VI, the influence of therapy with methotrexate in combination with low
dose corticosteroids on T-cell cytokine profiles was evaluated in RA patients. Before
therapy, RA patients showed a predominance of type 1 cytokines, in CD4+ T-cells
and CD8+ T-cells, compared to healthy controls. Long-term therapy with
methotrexate in combination with low dose corticosteroids reduced the predominance
of type 1 cytokines towards normalisation of the cytokine balance between type 1
and type 2 cytokine production, both in CD4+ and CD8+ T-lymphocytes.
In the next chapter (Chapter VIII), the direct influence of pro-inflammatory (IL-1, IL-
6, TNF-, IFN-) and anti-inflammatory (IL-4) cytokines on chondrocyte survival (cell
proliferation/necrosis/apoptosis), proliferation and nitric oxide production was
investigated in an in vitro model. No effect of IL-6 could be demonstrated on
chondrocyte function with respect to chondrocyte apoptosis, viability, proliferation and
nitric oxide (NO) production.
Pro-inflammatory cytokines such as IL-1 and TNF- but not IL-6 were able to induce
NO-mediated apoptosis, whereas only high levels of IL-4 could inhibit the effect of IL-
and TNF- on NO production and proliferation of chondrocytes but not apoptosis.
These changes in chondrocyte survival, induced by pro-inflammatory cytokines (IL-
1 and TNF-), might be of pathogenetic significance in the development of cartilage
degradation in rheumatoid arthritis.
In summary, this thesis provides further evidence for the role of cytokines in the
pathogenesis of rheumatoid arthritis and allergic diseases. Cytokine profiles can be
influenced by therapy as well in RA as in hymenoptera allergy. Furthermore,
evidence is presented that pro-inflammatory cytokines and immune complexes, able
to induce apoptosis in chondrocytes, can be involved in the development of cartilage
degradation in RA.
SAMENVATTING
Het doel van deze thesis is de rol van cytokinen en de pathogenese van reumatode
artritis (RA) te bestuderen en het cytokinenprofiel in RA te vergelijken met het
cytokinenprofiel in allergische aandoeningen.
In een volgend deel (Hoofdstuk IV) van deze thesis worden type 1 (IL-2, IFN-) en
type 2 (IL-4, IL-5) cytokinen intracellulair bepaald bij verschillende immunologische
aandoeningen: in een transversale studie worden simultaan in CD4+ and CD8+ T-
lymfocyten cytokinenprofielen bepaald bij patinten met reumatode artritis,
behandeld met salazopyrine of methotrexaat, bij patinten met allergisch astma, bij
patinten met atopisch eczeem en controlepersonen. Er wordt aangetoond dat
klinische stabiele RA patinten onder therapie toch een overwicht aan type 1
cytokine producerende T-lymfocyten vertonen in vergelijking met IgE gemedieerde
aandoeningen (astma, atopisch eczeem). Een positieve correlatie wordt gevonden
tussen type 1 cytokine productie en ziekteactiviteitsparameters bij RA patinten.
Alhoewel een overwicht aan type 2 cytokine wordt verwacht bij de IgE gemedieerde
aandoeningen, wordt enkel bij patinten met actief atopisch eczeem een verhoogde
productie aan een type 2 cytokine, namelijk IL-4, vastgesteld.
Uit de resultaten van de vorige studie kan men afleiden dat er mogelijks een invloed
is op cytokinenprofielen door ziekteactiviteit en therapeutische interventie. Daarom
wordt vervolgens nagegaan of therapeutische interventies veranderingen in het
cytokinenprofiel van enerzijds type 1 en anderzijds type 2 aandoeningen kunnen
veroorzaken.
In een tweede prospectieve studie (Hoofdstuk VI) wordt het cytokinenprofiel van RA
patinten gevalueerd voor en na behandeling met zogenaamde disease-modifying
anti-rheumatic drugs (DMARDs), waarvan men aanneemt dat ze het verloop van het
ziekteproces in gunstige zin kunnen benvloeden en niet enkel de symptomen
behandelen.
In deze prospectieve studies (Hoofdstuk V en VI) wordt duidelijk dat type 1 en type 2
cytokinen productie benvloed wordt door behandeling van patinten, enerzijds door
toediening van disease-modifying anti-rheumatic drugs (DMARDs) bij reumatode
artritis patinten (type 1), anderzijds door wesp immunotherapie bij patinten met
wesp allergie (type 2). Een normalisatie en verschuiving van de cytokinen balans
treedt op door behandeling van type 1 en type 2 aandoeningen.
In een volgend gedeelte (Hoofdstuk VII) wordt de invloed nagegaan van anti-TNF-
therapie op het cytokinenprofiel in T-lymfocyten en monocyten bij RA patinten.
Tumor necrosis factor alpha (TNF-), een cytokine dat vooral door monocyten en
macrofagen wordt geproduceerd, speelt een belangrijke rol bij auto-immuunziekten.
Er werd in muizenmodellen aangetoond dat anti-TNF- therapie de activiteit van
TNF- inhibeert en de ontwikkeling en inflammatie bij collageen genduceerde artritis
reduceert. Klinische studies rapporteren dat anti-TNF- therapie bij patinten met
actieve reumatode artritis, die niet reageerden op methotrexaat behandeling,
aanleiding geven tot een duidelijk verlaagde ziekteactiviteit. In hoofdstuk VII wordt de
invloed nagegaan van anti-TNF- therapie op de ziekteactiviteit (in vivo) en op het
cytokinen profiel (IL-2, IL4, IFN-, IL-10, IL-12, IL-1, IL-6, TNF-) in T-lymfocyten en
monocyten van patinten met reumatode artritis (in vitro).
We kunnen besluiten dat dit werk aantoont dat cytokinen een rol spelen, enerzijds in
reumatode artritis en anderzijds in allergische aandoeningen. Verder kan de
kraakbeendestructie bij reumatode artritis mogelijks verklaard worden door apoptose
van chondrocyten, genduceerd door pro-inflammatoire cytokinen en
immuuncomplexen.
