1.

Work method for launching girder shall be revised by the contractor

and submitted for the approval of supervision consultants. The work method
must include as minimum, the preventative control measures and procedures of
the contractor / sub contractor. the responsibility of each person on work site ,the
step by step procedures that will be employed including the technicalcalculations
(e.g for the lifting of girders) , the testing and adequacy of the lifting gear
including tensile strength and diameter of steel wire ropes (SWR) to be used the
mechanical fitness of the Winches snatch blocks. etc clear zone areas , detour
plans , communication methods, emergency action plans, etc.

2. Contractors shall prepare an equipment and work gear testing and

maintenance control methodology and submit for the approval of supervision
consultants. The methodology will include as a minimum, inspection
frequency , testing frequency , maintenance frequency service life etc.

3.Contractors communication method shall be improved specifically when

heavy machinery is at work, as well as when persons are working at height
and at underground and demolition works. The current practice of using
hand signals is not effective. The Contractor shall upgrade their
communication methods to include modern tools, equipments , including the
use of walkie talkie etc

4.The Contractor must ensure its responsibility that all its sub-contractors fully
comply with the applicable health and safety requirement under the contract
provisions and the law of the land .In addition , the Contractor must also
ensure that its sub-contractors fully adhere to the approved work
methodology.

5.Supervision Consultants will ensure that no work is carried out without the
approved methodology and no work is executed unsupervised. The
Supervision Consultants will also ensure that the approved work
methodology is continuously monitored for the compliance during execution
. The Contractor will give an undertaking that the approved methodology
will not be changed or modified at site without the Supervision Consultants
approval.

6.Supervision consultants will ensure that contractor obtain prior clearance to

execute high risk works e.g work on heights , work on launching of girders
,installation of scaffolding and formwork underground and demolition works
etc.
7.Contractor will increase the number of qualified health and safety officers
with the requisite trainings / certifications construction work site safety to at
least 1 person for every 5km intervals and adhere to Clause 6.7 Health and
safety of the general conditions of the Contract.

8.Supervision Consultant will increase the number of Occupational and Health

and Safety Experts with experience in highway construction sites for
effective and safe supervision of the works.

9.Both the contractor and Supervision Consultants will provide assurance in

writing to NHA that the overall health and safety practices on the E-35
Project have adequately improved and the instructions hereunder stand
implemented.
