SUMMER APPLICATION INSTRUCTIONS

The types of financial assistance available for the summer term are limited compared to the fall and spring terms.
Additionally, amounts for some types of assistance available for the summer will depend upon your annual and aggregate
levels. Please keep in mind that the majority of funding available for the summer term will be LOANS. When processing your
financial aid application, the Office of Student Financial Aid (OSFA) will indicate on your Financial Aid Award Notification all
aid for which you are eligible. You will need to indicate which awards you would like to “accept” or “decline.” You must then
return the signed Financial Aid Award Notification to OSFA by the appropriate deadlines.

Below are more-detailed instructions regarding required documentation, deadlines, degree status, enrollment status and
types of assistance as they pertain to the summer 2010 term.

Required Documentation
• The 2009-10 Free Application for Federal Student Aid (FAFSA) must be on file in the OSFA by June 30, 2010 to be
considered for financial assistance for the summer 2010 term. UIC’s federal school code is 001776. Note: In some
situations a 2010-11 FAFSA must be on file in the OSFA to be considered for summer assistance.
• If you are selected for Verification, you will receive notification from the OSFA indicating the additional documentation
that must be submitted before your financial aid file is reviewed and processed.
• You must also submit this Summer 2010 Financial Aid Application to the OSFA. Keep the Instructions.

Summer 2010 Deadlines (There will be no exceptions)


Summer application Priority Deadline 4-week & 12 week: Friday, April 2, 2010
8-week: Friday, April 2, 2010
Last date Summer Application will be accepted 4-week & 12 week: Friday, May 21, 2010
8-week: Friday, July 9, 2010

Documentation 8-week: Friday, July 9, 2010
Last date to accept a Federal Perkins Loan and to 4-week & 12 week: Friday, May 28, 2010
complete the Perkins Master Promissory Note (MPN) 8-week: Friday, June 18, 2010
Last date to accept any other aid and to complete 4-week & 12 week: Friday, May 28, 2010
appropriate promissory note(s) 8-week: Friday, July 16, 2010

Degree Status
In order to be eligible for financial assistance, you must be a degree seeking student at UIC. If you are not a degree seeking
student, you may be eligible for a private loan. If you are interested in such a loan, please review the list of private
lenders/loans via the following link: http://www.finaid.org/loans/privatestudentloans.phtml. Approval of private loans is at the
discretion of the lender, not UIC.

Enrollment Status
In order to be eligible to receive loan assistance, you must be enrolled for at least 6 credit hours if you are an undergraduate
or professional student. Graduate students must be enrolled for at least 5 credit hours. If you are eligible to receive a
Federal Pell Grant and have not used all your eligibility in the Fall09/Spring10 terms, you may be eligible to receive a Federal
Pell Grant if you are enrolled for at least 3 credit hours. NOTE: If you register for fewer hours than you report to us on your
Summer Application, it may result in a reduction or cancellation of awards.

Types of Assistance
The most common types of assistance available during the summer term are the following: Federal Pell Grant, Illinois
Veteran Grant, Illinois National Guard Grant, Federal Stafford Loan, Federal Perkins Loan and Federal PLUS loan. If you are
eligible for the Illinois National Guard Grant and/or Illinois Veterans Grant, please make sure to indicate on the application if
you wish or do not wish to use either of these programs for the summer term. If you do plan to use either of the programs for
the summer term, a copy of your ISAC eligibility letter must be on file in OSFA.

1. Review the instructions before completing this application. You must complete this entire worksheet and the form must be
SIGNED. Be sure to neatly print your UIN within the boxed areas in the bottom right-hand corner of this form.
2. Please submit this document, along with any other requested documentation to the address above. Please do not FAX.
