Questionnaire

Respected Respondent,

I am student of PRESTON UNIVERSITY, ISLAMABAD. I am doing a research on Role


Stressors, Employee Performance and Supervisor support in public and private health sector.
I would greatly appreciate if you spare some time to fill the enclosed questionnaire,
please be as honest as you can your answers are confidential and will not be shared with others
you work with.
Through this questionnaire I intend to have only your views on the issue and there are no
right or wrong answers. So, please respond to all questions as requested.
