Notes: Peter had a little bit of a cold today, so he wasnt as focused as he usually is. Mom also mentioned they

have had a crazy day.

Impression:
This week, we had to reschedule our visit to Monday instead of Wednesday because of
Thanksgiving. When we arrived, Peters mom left a note at the door saying they would be
back soon, because they had to pick up Peters older sister. When they arrived home, Peter
came running into the room to see us. Peters mom mentioned that he wasnt able to go to
preschool because of his cold so he has had a crazy day running errands with his mom. For
this week, we decided to start working on a new goal, which is, following 1-2 step directions.
We brought some new activities for Peter to experiment with and to help keep him engaged.
Today, we noticed he had a harder time than normal with staying focused. The activities that
we had planned for today was, the ball/stairs game, and the Simons Says game.
Objective Description of Data and Observations:
For the first activity, we played the throwing the ball down the stairs game. Peter did a great
job at taking turns with the ball. We would say to him, Whose turn is it with the ball? and he
would let us know whose turn it was. Occasionally he would say it was Peters turn a few
times in a row, so we reminded him that it was someone elses turn. He loved rolling the ball
down the stairs and would say boink every time it bounced down a new stair. After we did
this activity several times, we told Peter we were going to play a new game.

As we went into the other room to play Simon Says, Peter ran outside and ran around the
house. After he ran around the house, he came back inside and we gathered up his family to
play Simon Says with. For this activity, we didnt play where you were out if you did
something that Simon Says didnt say. We did this activity, just so that Peter could follow
simple directions, and also stay on task in a circle time activity for several minutes. He did
great with this, and loved doing the actions and mimicking like all of us. After a long time of
playing this game, Peter started to wander around the room and lose interest. We tried to guide
him back to the circle, and he stayed for a few more Simon Says rounds.
As we played Simon Says, we were working on two goals of Peters. The first goal that we
were focusing on was, following directions. The goal that his mom wants him to work on is
following 1 to 2 step directions, so we found a way to get him engaged while following
directions. The other goal that we were working on while playing this game was, staying in an
activity area for 6-7 minutes. For this goal, we really want to see him in a circle time activity
where he is staying and participating.

Assessment and Evaluation:

For the first activity of playing ball on the stairs, Peter knew exactly what to do since we have
done this many times before with Peter. In the past with this activity, Peter has spontaneously
told us whose turn it was. However, today we could tell it was harder for him to stay focused,
as he needed the direction and guidance today to recognize it was time for someone else to
throw the ball. With this activity, he didnt stay as long as he normally does playing. At first I
thought this might be because we do this activity often with him, and that maybe he wasnt as
interested in it. However, as the day went on, we came to see that he had a hard time staying
focused on things that he normally loves to spend lots of time on. His mother noted that when
he is sick, he becomes less patient and has a hard time staying focused.
For the next activity of playing Simon Says, Peter was very excited to participate because his
siblings and his mom was also participating and excited to follow the directions. At the
beginning of the activity, we could give out a simple direction and Peter would follow it and
do the actions. However, by the end of the activity, Peter needed modeling and some physical
guidance to stay focused and to complete the direction that was given out. Peter did a great job
staying in the center for as long as he did, and so by the end he was just ready to move on to a
new activity.
Today with the activities, we had to be a little more intrusive than we have in the past just
because gaining his attention was a little trickier today. We only had to use partial physical
today about twice and that was enough for him to be able to complete the task. I dont think
that we ever used something that was too intrusive for Peter as we helped guide him to finish a
task.
We made sure that the activities were appropriate for Peter, and even made adjustments to
Simon Says to ensure that he would understand what is being asked and would be able to
follow along with how you play.
This week, we decided to add on his goal of following 1-2 step directions. We did this because
we felt that he was making great progress on his other goals, and that it would be good to add
something else related for him to work on.
As a team, we work really well together and always discuss goals and activities that we need
to work on with Peter during his intervention. Peter has become comfortable around us and is
excited to play with us when we go to his home.

Plan:
This next week as we visit Peter, we will continue to work on all three of his goals, which are,
taking turns, staying in an activity area for 6-7 minutes, and following 1-2 step directions.
Next week we will do the throwing the ball down the stairs game, we will incorporate a snack
for Peter, we will have a circle time activity- possibly a book where he can be engaged, or a
song. Depending on the weather, we will also go outside for Peter to jump on the trampoline
to give him time to move around and to incorporate taking turns. For a taking turn activity, we
will bring another activity incorporating letters somehow. For his goal on working on
following 1-2 step directions, we will incorporate more games to help him understand
following directions. We will do games such as Simon Says, and hide and go seek. As we
work with Peter this next week, we will try to focus on using verbal prompts to help guide
Peter, and will move to other prompts if he needs it.
