Assignment:

Figure shows a diagram of offshore reactor which consists of with high temperature alarm and
temperature controller used to control the process. The frequency of loss of cooling of the
reactor is once in a year. Construct an Event Tree Analysis (ETA) to quantify the following
four steps of safety functions;

i. High temperature alarm alerts operator with the failure rate of 1%


ii. Operator notices high temperature with the failure rate of 25%
iii. Operator re-starts cooling system with the failure rate of 25%
iv. Operator shuts down reactor with the failure rate of 10%

From the analysis, show whether the operation should be continued or shutdown or runaway.
