Information JD has been wandering the hall, leaving class, and
Communicated expressing desire to attend a different school. To fix this,
JD will be able to see Mr. Wallace in the mornings when
she comes to school late. Once in class, JD will have the
option to ask for breaks and will specify a reason why
she needs to take them. This is to help her learn how to
identify and express her emotions, and also self-
advocate. She may start to meet with Shannon once a
week as well. We also want JD to consider after school
activities.

Information We have done additional testing since the last meeting:

Articulation and expressive and receptive language:
expressive and receptive vocabulary set GP in the low
average.

Clinical Evaluation of Language Fundamentals: GP
testing as average

The language memory test showed GP having average
abilities.

Screening test for apraxia shows GP having the apraxic
language disorder muscle articulation
GP struggled the most with verbal sequencing

This is the second testing the GPs mother has done. She
had an intensive neurodiagnostic test that identified GP
as having dyslexia from Dr. C

GP continues to qualify for speech and language services


AR issues with executive function (good test with
teacher observations)


Outcome and Follow up This is the follow up meeting for the evaluation of
existing data and the meeting for GPs IEP reevaluation
eligibility.
