Retake Form

Students who score below a 66% on a test have the opportunity to retake the test
for a higher score. Students have 3 school days to complete a retake from the day that
the test is returned to them.

Instructions: Decide on a date and time outside of regular class time (before/after
school or study hall) to complete the test and tell Mrs. Hibbler what you decide by email
or phone (ehibbler@lansingchristian.org or 810 627 8280). Return this form to Mrs.
Hibbler on the day of the retake with the original test.

This form needs to be signed and returned on the day of the retake. A signature
on this form indicates that you have studied with your student to prepare them for this
retake opportunity.
