Use of Explosive meter on board vessel should become a regular practice, while

taking soundings of fuel oil tanks located in confined space or in a space not
well ventilated or fitted with a diesel engine.
All the oil tank sounding pipes located in the engine room must be provided
with self closing devices.
Bunker tank sounding pipes are not to be located in the vicinity of any diesel
engines.

1 Ship staff responsible for the maintenance and upkeep of the engine must always
involve themselves in analyzing the problem and ascertain the root cause of the
problem.

2. Proper running-in of the engines / Gearbox must be strictly followed as per the
maker?s recommendations.

3. Proper investigation must be carried out, whenever metal particles noticed in the
filters of the L.O.system
