1. Introduction
1.1. JobDescription
This method statement has been prepared by Coffey Water Ltd. (CWL) to outline the steps required to
performinspectionsontwoSouthDublinCountyCouncil(SDCC)pressurevessels.Theinspectionsarebeing
carried out to satisfy the statutory inspection requirement for these types of vessels. The two pressure
vesselsarelocatedatthefollowingsites:
1. PressureVessel1SaggartBoherboyWaterPumpingStation(WPS),Saggart,Co.Dublin.
2. PressureVessel2NewcastleDrainagePumpStation(DPS),Newcastle,Co.Dublin.

ThisMethodStatementwilloutlinetheproceduresforeachstageoftheinspectionprocessaswellasthe

1.7. ScopeofWorks
ThescopeofworksrequirestheinspectionoftwosurgevesselsatwoseparateSDCCsites.Thefirstvessel
isa17m3vesselinstalledonapotablewatertransferpumpstationatSaggartReservoirandtheotherisa
6m3vesselinstalledonawastewatertransferpumpstationatNewcastleDPS.
TheinspectionworksarebeingcarriedouttosatisfytherequirementsoftheSafety,HealthandWelfareat
Work(GeneralApplicationAmendment)Regulations2012forstatutorypressurevesselinspections.
Theinspectionworkswillrequirethefollowingsteps:

Themainpurposeofthepressurevesselinspectionrequirementsistoensurethateachpressurevesselis
safelyoperatedandmaintained.Someofthebenefitsthatresultfromregularlyscheduledpressurevessel
inspectionsarelistedbelow:

x Improvementoffacility,personnel,andpublicsafety;
x Preventionofdamagetotheenvironment;
x Improvementofreliability;
x Reductionofoperationandmaintenancecosts;
x Minimizationofunscheduledoutages;and
x Minimizationofliability.

1.8. FrequencyofInspection
To satisfy the inspection requirements called for by the Safety, Health and Welfare at Work Regulations
2012forpressurevesselsofthetypesinstalledatSaggartandNewcastle,inspectionandtestingmustbe
carriedoutasfollows:
1. Beforethevesselisfirstplacedinservice;
2. Afteranymajorrepairoralterationtothevessel;and
3. The next inspection must be performed within 26 months, and the interval of subsequent
inspectionsmustnotexceed26monthsthereafter(providingdeteriorationisshowntobelowand
atapredictablerate).

1.9. Standardsandcodes
The engineering inspector may take account any construction standards used for the vessel being
inspected.

5. SequenceofWorks

5.1. PreInspectionActivities
ThefollowingpreinspectionactivitieswillbecarriedoutforeachofthevesselsatSaggartandNewcastle
5.1.1. SiteVisit
AsitevisitwillbecarriedoutbyCWLforaninitialassessmentofthesurgevesselsinstallation.
Asubsequentsitevisitmayberequiredbythevarioustradestoassessthesiteareabeforemobilisationto
sitefortheworks.CWLwillliaisewithSDCCtoarrangethesitevisitsandtoensurethatsitevisitattendees
areescortedandthecorrectaccessandinductionproceduresarefollowed.
5.1.2. PressureVesselDocumentation
Areviewoftheknownhistoryofthepressurevesselwillbeperformedfromdateofmanufacturetothe
presentday.Thiswillincludeareviewofinformationsuchas:

x Designdetailsincluding;
- Length;
- Diameter;
- Dishedendsshapeandradii;
- Capacity;
- Designpressure;
- Nozzleconnectiontypesandsizes;
- Vesselancillaryequipment(safetyvalves,pressuregauges,sightglassetc.);and
- Internalandexternalcoatingsystem.

x Operatingconditions;
x Normalcontentsofthevessel;
x Dateoflastinspection;
x Serialnumberandmaterialsofconstruction;
x Codeofconstruction;
x The type of connections used during fabrication of the vessel to determine the proper joint
efficiencytobeusedduringstressanalysisofthepressurevessel;and
x Shellthicknesssurveyrecords.
5.1.3. ScaffoldingInstallation
Scaffoldingwillberequiredateachofthevessellocationsandwillbeinstalledpriortotheinspectionteam
mobilisingtosite.Scaffoldingwillbeinstalledtoprovideaccessto:

x Theupperhalfofeachvesselexternally;
x Thevariousvalvesandappurtenancesmountedontopofthevessels;and
x Theaccessmanwayattheendofeachvessel.
Scaffoldingwillalsobeinstalledinawaythatdoesntprecludeaccesstoanyoftheexteriorsurfacesofthe
vessel.
ScaffoldingwillbeinstalledbyasuitableSOLASCSCScertifiedscaffolder,inaccordancewiththerelevant
legislation and HSA code of practice.  The scaffolding used will be fit for purpose, in good condition and
properworkingorder.Thescaffoldingwillbeinspectionaftererectionandbeforeuse.Itisnotexpected
thatthescaffoldingwillbeinplaceinexcessofthe7dayinspectionrequirement.
Thefollowingsectionswilldetailtheinspectionprocedureforthevesselateachlocation.

5.2. SaggartBoherboyWPSSurgeVesselInspection
The SaggartBoherboy surge vessel is installed on a pump station that supplies water to the Kiltalown
reservoir. The pump station operates at night between the hours of 9pm and 6am. The pump station is
offlineduringtherestofthedayandthistimewillbeusedtoconductthesurgevesselinspection.
TheinspectionofthesurgevesselattheSaggartBoherboywaterpumpstationwillbebrokendowninto
thefollowingstages:
1. Vesselisolation,lockoutandtagout;
2. Vesseldeenergisationanddraindown;
3. Manwayopeningandinternalaccess;
4. Externalinspection;
5. InternalInspection;
6. NonDestructiveTesting(NDT);
7. Safetyvalveandpressuregaugetesting;
8. Vesseldisinfection;
9. VesselSealingandRecommissioning
5.2.1. VesselIsolation,LockOut&TagOut
In general, the surge vessel isolation and access procedure should follow the SDCC procedure for vessel
isolationshowninSection7.2inAppendixC.
TheSaggartBoherboyWPSsurgevesselwillbetakenoutofservice,lockedoutandtaggedout.Thiswillbe
theresponsibilityofSDCC.CWLpersonnelwillsightandconfirmthephysicalequipmentisolationsaswell
astheaccompanyingdocumentationbeforeproceedingwithanyworksonthesurgevessel.Thefollowing
itemsofplantandequipmentwillbelockedoutofservicetoallowtheinspectiontotakeplace:
1. Pumpsdischargingintothepressuremainthatthesurgevesselisconnectedto;
2. 300mmwaterinlet/outletisolationvalveunderneaththevessel;
3. Aircompressorfeedingthesurgevessel;and
4. Compressedairlineisolationvalveatopthesurgevessel;
SeeAppendixDforamarkedupcopyofthesystemP&IDshowingthe3pointsofisolation.
5.2.2. VesselDeEnergisation&DrainDown
OncethevesselisolationisconfirmedbyCWLstaffthenthedeenergisationanddraindownprocedurewill
begin.
Thevesselwillbedeenergisedbyventingtheresidualcompressedairthroughthemanualventvalveon
topofthesurgevessel.Oncetheinternalpressurehasbeenequalisedwiththeatmospherethenthedrain
downofthevesselcantakeplace.A3inchdrainhosewillbeattachedtothedrainvalveonthebaseofthe
vessel and run to the nearest foul sewer approximately 20 metres away, see Appendix E for details. The
drainvalvewillbeopenedandthevesselallowedtodraindownuntilempty.
5.2.3. ExternalInspection
Athoroughexternalexaminationofthepressurevesselincludinginspectionoftheweldedconnectionsand
associated equipment will be undertaken. The external inspection will provide information regarding the
overallconditionofthepressurevessel.Thefollowingitemswillbereviewed:
1. Ifitisfoundthatexternalcoveringssuchasinsulationandcorrosionresistantcoatingsareingood
conditionandthereisnoreasontosuspectanyunsafeconditionbehindthem,itisnotnecessaryto
removethemforinspectionofthevessel.
2. Anyleakageofwaterorairwillbeinvestigated.Leakagecomingfrombehindcoverings,supports,
or settings or evidence of past leakage will be thoroughly investigated by removing any covering
necessaryuntilthesourceisdetermined.
3. The pressure vessel mountings will be checked for adequate allowance for expansion and
contraction. Adequate allowance may be provided by slotted bolt holes or unobstructed saddle
mountings. Attachments of legs, skirts, or other supports should be examined for distortion or
cracksatwelds.
4. Vessel connections including manholes, reinforcing plates, nozzles, or other connections will be
examinedforcracks,deformations,orotherdefects.Boltsandnutswillbecheckedforcorrosionor
defects.Weepholesinreinforcingplateswillremainopentoprovidevisualevidenceofleakageas
wellastopreventpressurebuildupbetweenthevesselandthereinforcingplate.Accessibleflange
faceswillbeexaminedfordistortionandtodeterminetheconditionofgasketseatingsurfaces.
5. The surfaces of shells and heads will be examined for possible cracks, blisters, bulges, and other
evidence of deterioration, giving particular attention to the skirt and to the support attachment
andknuckleregionsoftheheads.Thesurfacesofthevesselwillbecheckedforerosion.
6. Weldedjointsandtheadjacentheataffectedzoneswillbeexaminedforcracksorotherdefects.

5.2.4. ManwayOpeningandRemoval
When the vessel draindown is complete, the manway at the end of the vessel can be unbolted and
removedtoallowaccesstotheinsideofthevessel.ThemanwayisattachedtoasmallDavitarmthatwill
swingthemanwaycovertoonesideandsupportitsweightonceremoved.TheDavitarmandconnection
to the manway cover should be thoroughly inspected prior to unbolting to ensure they are in good
conditionandcapableoffunctioningcorrectly.
5.2.5. InternalInspection
APermittoWorksystemwillbeputinplacetocontrolaccesstotheinsideofthesurgevessel.Thissystem
willensurethattheworksiteissafetoaccessandonlyauthorisedpersonscanentertheconfinedspace
workareasubjecttosuitablesafetyprecautionsinplace.
Avisualinspectionoftheinternalsurfacesofthesurgevesselwillbecarriedout.
Gas monitoring, lighting and forced ventilation equipment will be installed inside the vessel. The gas
monitor will be left in the tank for continuous monitoring of gases prior to entry. The forced ventilation
equipment will consist of an air blower (set up outside the vessel at ground level) and flexible ducting
runningfromtheoutletoftheblowertotheinsideofthevessel.Lightingwillbe110voltwhererequired.
Personnelenteringthevesselwillwearasafetyharnessconnectedtoamanwinchmountedoutsidethe
vessel.PersonnelenteringthevesselwillwearPPEincludingdisposableoveralls,bootcoversandglovesto
preventcontaminationofthevesselinternals.
A rescue kit including breathing apparatus, will be on standby for the duration of the works inside the
vessel.
Thevisualinspectionofthesurgevesselinternalswillfocusonthefollowing:
1. Theconditionoftheoriginalpaintsystemthatwasusedtocoattheinsideofthevessel;
2. Theconditionandcorrosionofanyexposedbaremetalsurfaces;
3. The condition of welded joints and the adjacent heat affected zones and whether any cracks or
otherdefectsarepresent;and
4. Vessel connections including manholes, reinforcing plates, nozzles, or other connections will be
examinedforcracks,deformations,orotherdefects.

5.2.6. NonDestructiveTesting
Spot checks of the pressure vessel shell and dished heads will be performed and documented from the
outside of the surge vessel by the inspector using ultrasonic testing equipment. The ultrasonic testing
equipment should be properly calibrated and have a waveform display with peaktopeak measurement
modecapabilityandanaccuracyof+/1%.Thewallthicknessdataforeachsubsequentinspectionwillbe
usedforcomparisonstodetermineifanywallthinningmaybetakingplaceduetocorrosion.
Inadditiontothethicknessspotchecks,theinspectorwillcarryoutmaterialthicknesschecksusingNDTon
any areas of corrosion on the vessel that are identified as being of concern during the internal/external
inspections.
5.2.7. SafetyValveandPressureGaugeTesting
Athoroughinspectionofthesafetyvalveandothersafetydeviceswillbecarriedouttoensurethevesselis
operatingwithinitsspecifiedpressurerangeandisbeingadequatelyprotected.Functionaltestingofthe
safety valve and pressure gauge will be performed by removing the devices from the vessel and testing
themwithasmallportablepump.Thesafetyvalvewillbepressurisedtojustaboveitsreliefsetpointto
ensure proper operation. The pressure gauge will be pressurised across its operating range with a
calibratedreferencegaugebeingusedtoconfirmthevesselpressuregaugeisoperatingcorrectly.
5.2.8. VesselDisinfection
SDCCtoadvise.
5.2.9. VesselSealingandRecommissioning
Uponcompletionoftheinspection the surgevesselmanwaywill beclosed,boltedandsealed.Thedrain
hosewillberemoved,thedrainvalveclosedandthedrainvalveblankflangereinstalled.Newgasketswill
beprovidedforthemanwayanddrainvalveflangeconnections.Thesafetyvalveandpressuregaugewill
bereinstalledandthevesselrecommissioned&reinstatedintoservice.Ingeneral,therecommissioningof
the surge vessel will be carried out in accordance with Section 7.3 of the Vessel Isolation and Access
Procedure,seeAppendixC.

5.3. NewcastleDPSSurgeVesselInspection
The Newcastle surge vessel is installed on a waste water forwarding pump station. The pump station
operates throughout the day based on inflows into the plant. There is also a storm overflow tank onsite
thatisusedtocapturehighinflowsduringstormeventsforforwardingbythepumpslateroncethehigh
inflowrateshavesubsided.
Thestormoverflowtankswillbeusedtocapturetheinflowstotheplantduringthetimethepumpstation
isofflinetoallowtheinspectionofthesurgetank.SDCChaveadvisedthatthereisenoughsurgecapacityin

TheinspectionofthesurgevesselattheNewcastleDrainagePumpStation(DPS)willbebrokendowninto
thefollowingstages:
1. Vesselisolation,lockoutandtagout;
2. Vesseldeenergisationanddraindown;
3. Manwayopeningandinternalwashdown;
4. Externalinspection;
5. NonDestructiveTesting(NDT);
6. InternalInspection;
7. Safetyvalveandpressuregaugetesting;
8. VesselSealingandRecommissioning.
5.3.1. VesselIsolation,LockOut&TagOut
Ingeneral,thesurgevesselisolationandaccessprocedureshouldfollowtheprocedureshowninSection
7.2inAppendixC.
Thesurgevesselwillbetakenoutofservice,lockedoutandtaggedoutbySDCCoperators.CWLpersonnel
willsightandconfirmthephysicalequipmentisolationsaswellastheaccompanyingdocumentationbefore
proceedingwithanyworksonthesurgevessel.Thefollowingitemsofplantandequipmentwillbelocked
outofservicetoallowtheinspectiontotakeplace:
1. Wastewaterforwardingpumpsdischargingintothesurgevesselpressuremain;
2. 300mmwaterinlet/outletisolationvalveunderneaththevessel.
3. Aircompressorfeedingthesurgevessel;and
4. Compressedairlineisolationvalveatopthesurgevessel;
SeeAppendixDforamarkedupcopyofthesystemP&IDshowingthe3pointsofisolation.
5.3.2. VesselDeEnergisation&DrainDown
OncethevesselisolationisconfirmedbyCWLstaffthenthedeenergisationanddraindownprocedurewill
begin.
Thevesselwillbedeenergisedbyventingtheresidualcompressedairthroughthemanualventvalveon
top of the surge vessel. Once the internal pressure has been equalised with atmosphere then the drain
downofthevesselcantakeplace.A3inchdrainhosewillbeattachedtothedrainvalveonthebaseofthe
vesselandruntothepumpstationsuctionWetWell1approximately15metresaway,seeAppendixEfor
details.Thedrainvalvewillbeopenedandthevesselallowedtodraindownuntilempty.
5.3.1. ExternalInspection
Athoroughexternalexaminationofthepressurevesselincludinginspectionoftheweldedconnectionsand
associated equipment will be undertaken. The external inspection will provide information regarding the
overall condition of the pressure vessel. The external inspection will be carried out in accordance with
Section5.2.3ofthisdocument.
5.3.2. ManwayOpeningandInternalWashDown
When the vessel draindown is complete, the manway at the end of the vessel can be unbolted and
removedtoallowaccesstotheinsideofthevessel.ThemanwayisattachedtoasmallDavitarmthatwill
swingthemanwaycovertoonesideandsupportitsweightonceremoved.TheDavitarmandconnection
to the manway cover should be thoroughly inspected prior to unbolting to ensure they are in good
conditionandcapableoffunctioningcorrectly.
Oncethemanwaycoverhasbeenremovedthesurgevesselinternalswillbewasheddowntoexposethe
originalpaintfinish,ifstillintact,orthebaremetalofthevesselshellifthepaintfinishisnotintact.
Lighting will be installed at the vessel manway to light the inside of the vessel, facilitate the washdown
procedureandtoallowvisualexaminationofthevessel.
Lighting and forced ventilation equipment will be installed inside the vessel once the vessel has been
washed out. The forced ventilation equipment will consist of an air blower (set up outside the vessel at
groundlevel)andflexibleductingrunningfromtheoutletoftheblowertotheinsideofthevessel.Lighting
willbe110volthalogensitelightingrunfromgroundlevelfromthesitepowersupply.
AccesstotheinsideofthevesselshallfollowtherequirementsoftheSDCCConfinedSpaceProcedure,see
AppendixFfordetails.
5.3.3. InternalInspection
Avisualinspectionoftheinternalsurfacesofthesurgevesselwillbecarriedoutbyenteringthevessel.
APermittoWorksystemwillbeputinplacetocontrolaccesstotheinsideofthesurgevessel.Thissystem
will ensure that only authorised persons can enter the confined space work area, under very controlled
laiddownconditions.
Gasmonitoring,lightingandforcedventilationequipmentwillbeinstalledinsidethevessel.Multiplegas
monitorswillbeused,attheentrancetothetankbeforeentryandontheinspectionoperative.Amonitor
maybetemporarilyplacedinsidethetankforcontinuousmonitoringofgasespriorandduringentry.The
forcedventilationequipmentwillconsistofanairblower(set upoutside the vesselatgroundlevel)and
flexibleductingrunningfromtheoutletoftheblowertotheinsideofthevessel.Lightingwillbe110volt
sitelightingrunfromgroundlevelfromthesitepowersupply.
Personnelenteringthevesselwillwearasafetyharnessconnectedtoamanwinchmountedoutsidethe
vessel. Personnel entering the vessel will wear PPE including safety glasses, FFP3 particle filtering mask,
disposableoveralls,bootcoversandgloves.
Anemergencyescapeunit,willbeonstandbyforthedurationoftheworksinsidethevessel.
Thevisualinspectionofthesurgevesselinternalswillfocusonthefollowing:
1. Theconditionoftheoriginalpaintsystemthatwasusedtocoattheinsideofthevessel;
2. Theconditionandcorrosionofanyexposedbaremetalsurfaces;
3. The condition of welded joints and the adjacent heat affected zones and whether any cracks or
otherdefectsarepresent;and
4. Vessel connections including manholes, reinforcing plates, nozzles, or other connections will be
examinedforcracks,deformations,orotherdefects.

AnysignificantareasofcorrosionwillbeinvestigatedfurtherusingNDT.
5.3.4. NonDestructiveTesting
Spot checks of the pressure vessel shell and dished heads will be performed and documented from the
outside of the surge vessel by the inspector using ultrasonic testing equipment. Nondestructive testing
shallbecarriedoutinaccordancewithSection5.2.5ofthisdocument.
Inadditiontothethicknessspotchecks,theinspectorwillcarryoutNDTmaterialthicknesschecksonany
areas of corrosion on the vessel that are identified as being of concern during the internal/external
inspections.
5.3.5. SafetyValveandPressureGaugeTesting
Athoroughinspectionofthesafetyvalveandothersafetydeviceswillbecarriedouttoensurethevesselis
operatingwithinitsspecifiedpressurerangeandisbeingadequatelyprotected.Safetyvalveandpressure
gaugetestingwillbecarriedoutinaccordancewithSection5.2.7ofthisdocument.
5.3.6. VesselSealingandRecommissioning
Uponcompletionoftheinspection the surgevesselmanwaywill beclosed,boltedandsealed.Thedrain
hosewillberemoved,thedrainvalveclosedandthepressuregauge/levelinstrumentassemblyreinstalled.
New gaskets will be provided for the manway and drain valve flange connections. The safety valve and
pressuregaugewillbereinstalledandthevesselrecommissioned&reinstatedintoservice.Ingeneral,the
recommissioning of the surge vessel will be carried out in accordance with Section 7.3 of the Vessel
IsolationandAccessProcedure,seeAppendixC.

5.4. AirCompressorMaintenance
The surge vessels are provided with top up air supply from a compressor located at each site. The
compressorsareasfollows:

andtagoutproceduretoallowtheinspectionandservicingtooccur.CWLpersonnelwillsightandconfirm
theisolationsandthelockout/tagoutbeforethecompressorservicingworkscommence.
CompressorservicingwillbecarriedoutinaccordancewiththeprocedureoutlinedinAppendixG.
6. QualityControls
TheSiteSpecificHSQEPlanwilldetailallQualityControlsfortheProject.
See Section 1.9 for details of the standards and specifications that will be complied with throughout the
courseoftheworks.
UltrasonicNDTequipmentusedduringtheinspectionworkswillbeinproperworkingorderandcalibrated
correctlyinaccordancewiththemanufacturersrecommendationsforthematerialbeingassessed.

7. EnvironmentalImpacts
The Site Specific HSQE Plan will detail all Environmental Controls for the Project. Provide details of any
potentialenvironmentalimpactrelatedtotheworks.Whataretherisksandhowwilltheybecontrolledin
relationtothefollowing;theimmediateandsurroundinghabitat,floraandfauna,vegetationandadjacent
watersources.
Thefollowingsectionswilloutlinetheenvironmentalimpactrisksforeachsite.

7.1. SaggartBoherboy
The main environmental impact risk at the Boherboy site will be the potential for the discharge of
chlorinatedwatertotheenvironmentduringthedraindownofthesurgevessel.Thisriskwillbemitigated
byensuringthatthemanholeusedtodisposeofthedraindownwaterisafoulsewermanhole.
Inadditiontotheabove,measureswillneedtobetakentoensurethataftertheinternalinspectionthe
vesselisreturnedtoserviceinasterile/disinfectedstate.SDCChavesuppliedtheprocedurefordisinfecting
thesurgevesselpriortoitreenteringservice,seeSection5.2.8fordetails.

7.2. Newcastle
ThemainenvironmentalimpactriskattheBoherboysitewillbethepotentialforaccidentaldischargeof
foul water to the environment during the draindown of the surge vessel. This risk will be mitigated by
ensuringthatthedraindownlocationusedwillbepumpstationWetWell1.Thehosewillalsobesecurely
attachedto thesurgevesselandsecuredat the dischargelocationtoensurefoulwaterisnotaccidently
dischargedtothesurroundingarea.
8. SafetyControls
The Site Specific HSQE Plan will detail all Health & Safety Controls for the Project. Any changes to the
project method statement that are required on site will be assessed using a Safe Plan of Action, see
AppendixHforanexampletemplate.
CWL will provide a stand by man who will fill out confined space vessel entry permits. CWL will
witness/actionthevesselentryprotocoli.e.Lockoffaircompressor,closeandlockofftheinletvalve,use
gasdetectortocheckoxygenlevels,insuresafetygeariscorrectandindate,checkscaffoldingandladderis
inproperworkingorder.Ablowerandductingwillbeusedtocirculatefreshairwithinthesurgevessel.On
completionofallchecksCWLpersonnelwillenterthevesselwearingasafetyharness.Oncompletionand
exitfromvesselpersonnelwillsignoffthevesselentry.
CWL personnel will review the scaffolding certificate and sign the GA3 Working at Heights Inspection
form/tagsbeforeuse.

Once the lockout/tagout procedure has been completed by SDCC personnel for the surge vessel and
ancillaryequipment,SDCCwillauthoriseCoffeyGrouppersonneltotakeoverresponsibilityfortheworks

8.2. HazardIdentification/RequiredControls&PPERequirements
The hazards identified and control measures to be implemented are outlined in the Risk Assessments
attachedinAppendixJ.

1. Policy Statement

It is policy so far as is reasonably practicable to eliminate the hazards that give


rise to safety, health and welfare (SH&W) risks for employees and those who
may be affected by their activities, where this is not possible, implement control
measures that will manage the hazards and risks, that may arise, so as to
prevent injuries at work.

It is policy to protect employees and others who may be affected from or by the
hazards and risks associated with working in confined spaces.

2. Purpose

To provide guidance and a series of actions, on how to prevent injury and/or ill-
health associated with working on confined spaces and to mandate all employees
to demonstrate compliance with all applicable legislation in respect of confined
space working.

3. Scope

Applicable to all employees and those who may be affected by working in


confined spaces.

Confined Spaces are found in a range of locations within a local authority and
could be a trench / excavation used for construction purposes, a tank / manhole
in a water or wastewater pumping station, a basement within a depot or office
complex, a bridge void / culvert etc.

4. Legislation and Guidance

6. Definition
The term confined space means any place, including any vessel, tank, container,
vat, silo, hopper, pit, bund, trench, pipe, sewer, flue, well, chamber, compartment,
cellar or other similar space which, by virtue of its enclosed nature creates
conditions which give rise to a likelihood of accident, harm or injury of such a
nature as to require emergency action due to

(a) the presence or reasonably foreseeable presence of

(i) flammable or explosive atmospheres,

(ii) harmful gas, fume or vapour,

(iii) free flowing solid or an increasing level of liquid,

(iv) excess of oxygen,

(v) excessively high temperature,

(b) the lack or reasonably foreseeable lack of oxygen.

The following are key characteristics of a 'confined space' which are considered
as part of these procedures,

x The space must be substantially enclosed.

x There must be a risk of at least one hazard of the type, listed in the
definition above, occurring within the space.

x The risk of serious injury from the hazard must be created by virtue
of the enclosed nature of the space.

x The potential injury must be serious and be such as to require


emergency action to rescue the person involved.

7. Key Hazards

7.1 Toxic Atmosphere

A toxic atmosphere can cause various acute effects, including impairment of


judgement, unconsciousness and death. It may occur in the confined space due
to the presence or ingress of hazardous substances. These substances may
remain from previous processing, or be present as a result of previous storage, or
arise from the disturbance of sludge or other deposits (e.g. during cleaning).
Sometimes hazardous substances may be trapped under scale or in brickwork
only to be released as a result of the work process.

Hazardous substances may also enter the space from adjoining plant that has
not been isolated effectively. Gas and fumes can build up over time in, or can
travel along sewers, cable tunnels or culverts. Hazardous substances may be
produced by a work process being carried out inside or in the environs of the
confined space, for example, by welding, flame cutting or use of plant or
machinery.

A fire within or in the environs of a confined space could also result in the
formation of a toxic atmosphere within the space.

7.2 Oxygen Deficiency

A deficiency of oxygen in the atmosphere can result in impairment of judgement,


unconsciousness and death. It can be caused by displacement of air by another
gas or by various biological processes or chemical reactions (e.g. rotting of
organic matter, rusting of metal, burning, etc) which scavenge or consume
oxygen from the air. Oxygen can also be removed from the air by absorption onto
steel surfaces, especially where these are damp.

7.3 Oxygen Enrichment

An excess of oxygen in the atmosphere, in the presence of combustible material,


results in an increased risk of fire or explosion. Oxygen enrichment, even by a
few percent, considerably increases the risk of fire. Materials, which do not burn
in air, may burn vigorously or even spontaneously in enriched air. The
flammability of clothing may be increased. Oxygen enrichment may be caused,
for example, by a leak of oxygen from an oxygen cylinder used in gouging or
flame cutting operations.

7.4 Flammable or Explosive Atmospheres

A flammable atmosphere presents a risk of fire or explosion. Such an


atmosphere can arise from the presence in the confined space of flammable
liquids or gases or of a suspension of combustible dust in air. If a flammable
atmosphere inside a confined space ignites, an explosion may occur, resulting in
the expulsion of hot gases and the disintegration of the structure.

7.5 Flowing Liquid or Free Flowing Solids

Liquids or solids can flow into the confined space, drowning or suffocating the
occupants or causing other injuries such as burns. Solids in powder form may
also be disturbed in a confined space resulting in an asphyxiating atmosphere.

7.6 Excessive Heat

The enclosed nature of a confined space can increase the risk of heat stroke or
collapse from heat stress, if conditions are excessively hot. The risk may be
exacerbated by the wearing of personal protective equipment or by lack of
ventilation.

7.7 Other Potential Hazards

Other hazards, which may be present and need consideration are associated:

x with the place of work (e.g. slippery floors, falls from a height, moving
parts of mechanical equipment, corrosive surfaces, non-life
threatening levels of toxic fumes, etc) or

x with the work activity (e.g. noise, electricity; mechanical equipment,


dust etc), must be identified and evaluated in the normal manner.

Indeed some of these other hazards are often associated with confined spaces in
particular industry sectors. For example, in the water / sewage, physical contact
with mechanically-powered apparatus is a hazard commonly associated with
confined spaces due to the presence of rotary agitators in process vessels, the
hazard of exposure to micro-organisms / hazardous chemicals is synonymous
with work in sewers.

Appropriate control measures should be put in place for these hazards, as for any
others not mentioned above, which may be present.

8. Roles and Responsibilities

The following roles and responsibilities are specific to this document and are in
addition to the other roles and responsibilities assigned to the post-holder.

8.1 The Director of Service (DOS) and analogous grades shall ensure:

8.1.1 that the objectives specified in Section 5 are delivered.

8.2 The Senior Engineer (SE) and analogous grades shall ensure:

8.2.1 that the policies and procedures identified are being implemented throughout
the local authority

8.2.2 that inspections and confined space audits are undertaken on a regular basis

8.2.3 any necessary action plans arising from these outputs are integrated into the
local authority safety management system.

8.3 The Senior Executive Engineer (SEE) and analogous grades shall,
working with all relevant Line Management, ensure:

8.3.1 arrange for the implementation of the policy and procedure and will undertake
inspections and participate in audits to establish the overall level of
compliance.

8.3.2 The SEE will ensure that a list of the potential areas that could be defined as
a confined space to be assessed and be classified into high, medium and low
risk, recorded on a confined space register and be labelled in accordance
with Section 9 below. The Confined Space Register will be available on a
County / City wide basis and will also be included within any site-specific
safety documentation.

8.3.3 The SEE will also appoint Confined Space Authorised Person(s) (CSAP)
within each such area of command.

8.4 The Confined Space Authorised Person (CSAP)

8.4.1 The persons appointed shall be competent to complete a Confined Space


Risk Assessment and any associated control documents e.g. method
statements, permit to work systems etc.

8.4.2 All such personnel shall have completed the FS Course CS/MA/11 and
shall be listed in a register recording all known and assessed confined spaces
as referred above. The listing shall be revised each year and it shall give the
date to which each person retains their CSAP rating.

8.4.3 No person shall be permitted to enter a confined space to complete work


unless it is not reasonably practicable to achieve the work without such entry
and they have trained to enter the specific category of confined space as
outlined within Section. All confined space entries must be planned and
completed in compliance with the relevant confined space procedure.

8.5 The Area Engineer / Line Manager / Supervisor / Inspector person in


charge of each location where there is a confined space shall ensure:

8.5.1 That the access to that location is secure from the public and other persons
and that the confined space itself is closed off and properly signed at all
times.

8.5.2 That entry into confined spaces is in compliance with the procedures outlined
below in Section 9

8.5.3 That all necessary checks and inspections, including maintenance, will be
undertaken on all equipment used in confined operations. See list below
showing non- exhaustive range of equipment that can be used in confined
space entries.

8.5.4 Line Managers will also be expected to review confined space entry
procedures and ascertain additional controls that may reduce occupational
risk with other future work in this area.

8.5.5 The following records shall be maintained by Line Management:

x Register of Confined Space Authorised Personnel (CSAP).

x Register of all personnel who are competent (trained and medically


fit) to work in a confined space including the risk type (L/M/H) of
confined space and the date up to which they are permitted to do this
work. I.e. Confined Space Competent Person (CSCP)

x Records shall be maintained in each Section of all of the Confined


Space Entry Permits issued for a period of 5 years.

x Details of all training records for confined spaces. Training records


maintained by South Dublin County Council Human Resourses

8.6 Employees and Confined Space Competent Persons ( CSCP) shall:

8.6.1 not enter a confined space unless they are competent to undertake this work.
Competency includes medical assessment and also the knowledge, skills and
experience and supported by FAS approved training course as outlined with
Section 11 Training

8.6.2 follow and implement the appropriate safe system of work and execute
effective emergency arrangements to ensure safe working in confined
spaces.

8.6.3 Employees must also make proper use of any equipment provided including
personal protective equipment supplied by their employer, where required as
part of the safe system of work, for example, use of safety harnesses,
respiratory protective equipment and reflective clothing.

8.6.4 Employees must inform line management of any change to the personal
health that may affect their ability to work safely in confined spaces.

8.7 Health and Safety Officer shall:

8.7.1 Ensure the co-ordination of the Safety Management System and will advise in
the implementation of all procedures where necessary. The H & S officer will
also undertake inspections and audits and report findings through line
management.

8.8 Contractors.

Contractors working in Confined Spaces on behalf of a local authority shall


not be permitted to enter a confined space unless the following is in place:

8.8.1 The contractor has a site specific CS risk assessment and associated
documentation;

8.8.2 The contractor has sufficient personnel with the relevant FS


( LR/MR/HR/ER/MA) training to undertake the confined space entry safely;

8.8.3 The contractor has been advised by a Local Authority CSAP of the hazards
that could potentially exist in the confined space where work will be
undertaken;

8.8.4 The contractor has the required H&S equipment to undertake the confined
space entry, and that this equipment has being subject to the required
periodic safety inspections (copies of inspection certs to be made available
upon request) and is in safe working condition.


9. Procedures
(A course of principles of action adopted or proposed by a group or individual.
(Concise Oxford Dictionary 2010). This procedure provides an overview of the key
activities to be undertaken to support the successful implementation of safe working.

The type of procedure to be adopted when working in any confined space will
depend on the outcome of a site-specific risk assessment. Potentially any
substantially enclosed structure, in which people work, could be or could become a
confined space. Some places, which fall within the definition of a confined space,
may do so only occasionally due to the type of work to be undertaken.

A CSAP will undertake an initial Confined Space Risk Assessment using their Local
Authority Risk Assessment procedure or Confined Space Safe System of Work Plan
(SSWP) see Appendix 1

The generic procedure to follow with any category of confined space is as follows
and is subject to personnel being medically fit to undertake the work, being
competent (relevant FS approved training course) and having all the necessary
safety equipment.

x Identify work to be undertaken within confined space e.g. inspection,


maintenance, cleaning etc.
x Establish if the work can be done without making an entry.
x Undertake site-specific risk assessment identify hazards and assess risk
levels.
x Categorise the Confined Space into Low or Medium or High Risk
Classification.
x Subject to Risk Levels implement control measures and determine
emergency arrangements as summarised within the attached table.

The following are examples of working environments where the initial risk
assessment has determined the generic risk level and the procedures to follow are
generic as each location will require site specific procedures that is dependant on a
number of factors including hazards present, existing controls and the availability of
competent people (both authorised personnel and operators) as well as equipment.

It should be noted that this list is not exhaustive and risk levels can change depending
on new hazards being introduced into the confined space environment and / or the
activities undertaken therein e.g. entering a chlorine house to inspect pipework can be
classified as low risk but entering the same location whenever a house is filled with
chlorine gas due to a leaking pipe or valve will become a high risk and will require
significant controls in place before the leak can be repaired.

9.1 Low Risk Confined Space

A low risk CS exists where there is easy access / egress and natural ventilation. A
low risk entry with adequate natural or mechanical ventilation, where access is
simple (e.g. staircase or < 2metres deep if vertical lift) and unobstructed and there is
no likely risk of flooding.

Dry well entry for the purpose of short duration inspections or pump choke callouts,
where adequate mechanical ventilation exists, when all lines are sealed, where
permanent working gas detectors are present and they are only being entered for the
purpose of work which will not crack open any lines and when there is no obvious
evidence of any leaking pipe work then these dry wells are also considered Low Risk.
The above also is applicable to valve chambers.

Lone workers can work safely in this environment provided that they are trained in
low risk confined spaces e.g. FAS Course CS/LR/10 See Section 11, use a gas
detector to check the atmosphere before ensuring it does not alarm and adherence
to lone working procedures.

Confined spaces which pose few hazards of low impact and can be easily controlled
or removed. Typically these are spaces where natural or artificial ventilation can
easily be achieved and maintained, where entry and exit are straightforward and any
possible rescue can be performed adequately and safely by the entrant as a self
rescue.

Any workplace where the depth is > 2 metres and has an obstructed vertical lift e.g.
straight ladder into a dark chamber, then using a two man team and remaining
connected to a lifeline with tripod etc then entry can be made without the use of
Escape Breathing Apparatus. A safe system of work will be the main method of
control which may include adherence on occasions (2 person job) require a method
statement.

LOW RISK Confined Space Procedures

Following the completion of the Risk Assessment and before any Confined Space is
entered the space must be ventilated and the atmosphere must be tested and
certified as safe to enter in accordance with the control measures to be employed.

1/2 No. operative(s) for short duration work such as taking samples, reading meters,
un-choking pumps or undertaking inspections of low risk working environments. This
should also include valve operation whereby all fittings including sluice valves,
butterfly valves, hydrants, air valves and meters that are operable without the need to
enter a chamber or other confined space.

Only people trained to work in low risk environments should undertake this work and
a CSCP will be able to identity when a two person team is required or due to a
specific work activity will cause the risk level to increase identifying the need for
further controls to be introduced.

General specific hazards such as road traffic, manual handling etc. should also be
included within the site-specific risk assessment.

Min of Two-person operation required (where the depth is > 2 metres and has
an obstructed vertical lift).

I. Erect barriers and signs where necessary.

II. The no smoking prohibition should be enforced both in the confined space
and the immediate vicinity of the entry and ventilation points. (2 metres
radius)

III. Remove covers, open doors for access and ventilation.

IV. Check the gas detector then lower it into the confined space for at least five
minutes prior to entry. When testing gas in a chamber, you must check at
various levels in the chamber as different gases are heavier (e.g. H2S) &
lighter (e.g. CH4) than oxygen.

V. During this gas test note any unusual smells and carry out a visual check of
the stability of the structure, including ladders, step irons etc. assemble the
winching system, put on safety harness and make ready the resuscitator.

VI. If a gas or significant structural hazard is identified, abandon the entry, make
the area safe by replacing the covers etc., and immediately report the
conditions to the site or area supervisor.

VII. Minor structural problems can be reported at the end of the working day.

VIII. The CSCP in charge of the operation, having established that there is no gas
hazard and that the structure appears sound, shall work in accordance with
the training provided or in accordance the Checklist / Method Statement
prepared by the CSAP.

IX. The CSCP Topman will attach the winch cable to the safety harness worn by
the other CSCP person who will make the entry. In the cases where direct
lowering without the use of ladders or step iron is proposed, or if their
condition is suspect, an additional safety line shall also be attached and
manually played out as a supplement to the winch cable. This will require
either a second Topman to handle the safety line or an approved inertia
clutch device must be used to automatically control the safety line.

X. The entry may now proceed with the winch cable being controlled through the
clutch mechanism as the man climbs or is lowered into the confined space.
Under no circumstances may the winch cable be disconnected from the
safety harness whilst in the confined space.

XI. Whilst in the confined space, the gas detector must remain with the operator.

XII. In the event of the gas detector alarming or the operator man feeling unwell or
detecting unusual odours, the winchman will winch the CSCP from the
confined space. Escape breathing apparatus should be donned. The
Topman/winchman shall continuously observe the man in the confined space

and immediately winch him out should he appear to be in difficulty and unable
to communicate.

XIII. Evacuation of the confined space due to gas alarm shall be immediately
reported to the the Senior Onsite CSCP/CSAP. No further entry shall be
made until the situation has been positively re-assessed and confirmed with
the the Senior Onsite CSCP/CSAP. Re-entry in these circumstances must
start from procedure (ix) above.

XIV. Emergency evacuation of a casualty by winching only. Under no


circumstances should you enter the confined space to affect a rescue. Having
winched the casualty out, observe his condition and if necessary send for an
ambulance (e.g. unconsciousness).

XV. If the casualty is not breathing, dial 112/999 and ask for emergency services.

XVI. If the casualty has no neck pulse, apply CPR.

XVII. Egress under normal circumstances will require careful control of the winch
and particular care will be necessary when manoeuvring at the head of the
chamber.

XVIII. If when working in a low risk confined space there is gas alarm or the Topman
gives the instruction, the confined space should be evacuated immediately.
Do not stop to recover equipment. The CSAP CSCP in charge of the
underground party should account for all personnel identified within the
checklist / method statement.

XIX. At the completion of the operation all covers etc. must be replaced, barriers
removed and the site left in a safe condition. Where it is necessary to leave
covers off for ventilation purposes, the opening must be effectively and
securely fenced to prevent unauthorised entry.

XX. At the end of the day:

a. Check and stow safety equipment.

b. Report any equipment defects, operational problems or faults to the


site supervisor and log them into the checklist or method statement.

XXI. Report the finish of work to the site supervisor.

After testing the atmosphere the CSCP shall note the results of the test on the
Checklist.

Testing of atmosphere in the breathing zone shall continue throughout the time that
the space is occupied and any alarm shall cause an immediate evacuation to take
place unless full BA is being worn and suitable control measures have been put in
place.

A proper safety harness (complying with EN 361) must be worn at all times when in a
Confined Space below ground level

9.2 Medium Risk Confined Space

A medium risk confined space exists when there are access issues, a realistic
expectation of encountering a specified risk, possible introduction of specified risks
during the work activity, e.g. flammable, toxic or an oxygen deficient environment etc.

CSCP shall remain connected to a lifeline, monitored by a top man, and the use of
escape breathing apparatus is also required in these circumstances.

A Checklist / Method Statement will be the key management control system for
reducing risks as shown within Appendix 2.

When it is not possible to have persons permanently attached to a lifeline e.g. when
a team has to move away from the entry point in man entry sewers, complex sewage
wet wells, working on pumps in a drywell that has an obstructed vertical lift, e.g.
straight ladder, then this type of work will involve a bottom man (bottom of ladder)
and the use of escape BA. Note; in a drywell it is of paramount importance to avoid
the risk of a lifeline getting caught in the rotating parts of the pumping infrastructure.
In drywells fitted with a staircase, the workers in the drywell can perform a
self rescue when they don their beathing escape sets. Even if a staircase can
be accessed within 30 seconds at any point in the drywell, it is still advisable that a
tripod is setup and rescue line lowered to near basement floor level in case required
for rescuing an operative who has collapsed. Operatives should still however don a
rescue harness when working in a drywell considered a medium risk confined space
area.

IX. The CSCP in charge of the operation, having established that there is no gas
hazard and that the structure appears sound, shall work in accordance with
the Method Statement prepared by the CSAP will leave it with the designated
Topman.

X. Only one person maybe on the ladder or series of steps at one time. All tools
and materials should be lowered and raised by line and never carried by hand
on ladders.

XI. The CSCP in charge of the party will establish a system of visual or audible
contact with the designated Topman posting intermediate relay men if
appropriate.

XII. At least one gas detector will be carried with the working party and one gas
detector will remain at the entry point being monitored by the designated
Topman

XIII. Where the entry involves a long walk-through operation a further gas detector
and Topman will be stationed at the exit point.

XIV. Where relay men are stationed for communications and safety monitoring
they too should be equipped with gas detectors and escape breathing
apparatus.

XV. Standards for electrical safety and approved lighting apparatus for use in
confined spaces are contained in section 9 below.

XVI. If when travelling through a sewer, tunnel or similar confined space a


blockage is discovered, and it is not possible to determine with certainty
conditions beyond the blockage, the party should return to the point of entry
and obtain instructions from the CSAP.

XVII. Any work in clearing or investigating the blockage should be subject to High
Risk procedures (permit to work) as identified below.

XVIII. If when working in a medium risk confined space either the water level begins
to rise significantly, there is gas alarm or the Topman gives the instruction,
the confined space should be evacuated immediately. Escape breathing
apparatus should be donned. Do not stop to recover equipment. The CSAP
CSCP in charge of the underground party should account for all personnel
identified within the checklist/method statement. Where the place of work
within the confined space is greater than 10 minuites away from the cofined
space entry point, full breathing apparatus should be used for the duration of
the work.

XIX. Under no circumstances should anyone enter a confined space to rescue a


gas casualty. In such an event, a member of the crew at ground level must
dial 112/999 and ask for emergency services.

XX. At the completion of the operation the CSAP in overall charge shall account
for all personnel, ensure that the underground work area is either clear of
tools and materials or is otherwise in a safe condition and that all covers are
replaced, barriers removed and that the site is left in a safe condition.

XXI. Where entry points are left open for ventilation purposes they must be
effectively and securely fenced with grills and barriers to prevent unauthorised
entry.

XXII. At the end of the day:

a. Check, clean and stow safety equipment.

b. Report any equipment defects, operational problems or faults to the


supervisor and record them in the checklist / method statement.

XXIII. Report the finish of work to the site supervisor

9.3 High Risk Confined Spaces

A high-risk confined space exists where there is a known specified hazard that
cannot be eliminated or controlled without significant effort e.g. gas leak, mechanical
and electrical hazards, enhanced intrinsic hazards.

Circumstances, which might lead to a high-risk classification, may include the


following non-exhaustive list:
I. Fluid, mechanical or electrical isolation arrangements are complex
II. Structural systems are particularly deep or complex
III. Record drawings are in doubt
IV. There is a history of serious atmosphere hazard
V. Industrial waste discharges are insufficiently diluted to control harm from
chemicals, biological or radioactive agents.
VI. The risk of drowning is only controlled by pumping or tide.
VII. The job involves hot work or a fume producing process.
VIII. The job requires electrical apparatus operating above Safety Extra Low
Voltage (SELV) 25 volts or which is not explosion protected .
IX. The work in hand is unfamiliar, complex or inherently hazardous.

A permit to work system will be the key management control mechanism to reduce
risk. Specific rescue procedures are also required for this type of work activity.
Personnel levels will be determined by the outcome of a site-specific risk assessment
by a CSAP as described above and the completion of a permit to work.

There may or may not be access issues and the employee is expected to carry out
their function in the presence of this specified risk e.g. unable to completely isolate
water. Employees must wear emergency escape breathing apparatus and will on
occasions wear self contain breathing apparatus during working or will be available
for rescue purposes.

9.4 High Risk Operations Requiring a Permit to Work

A Permit to Work must be completed by a Confined Space Authorised Person


(CSAP). See Appendix 3. Once the Permit to Work is completed the CSAP shall
ensure that all of the following are completed / in place prior to any work actually
commencing:

x All of the safety equipment required, including rescue equipment and


PPE, is on site.

x All of the required trained personnel are on site.

x All of the personnel have been trained in and understand the specific

9.5 Assessment

Every location and operation is likely to be different and therefore every case
will require specific consideration. A number of aspects will be common to all
entries, whilst others will be peculiar to special operations.

Factors for consideration include:

9.6 Identify the System Layout

Schematic drawings and plans, or sewer maps, isolation, connection and


diversion points, valving, trade effluent factors, dimensions (depth, bore,
access, distances) other services, movements and gas hazards in the vicinity.
Electric circuits and Mechanical pipelines and isolation points.

9.7 Access and Structure

Ventilation, method of access and egress, site security, structural stability,


obstructions, inspections and temporary supports

9.8 Materials

Use of paints, mastics, adhesives and chemicals processes using gases, or


producing dust or fumes; chlorine or other chemical dosing etc. a Chemical
risk assessment will always be required in such circumstances.

9.9 Equipment and Special Operations

Hotwork (flame cutting, grinding, welding, cartridge tools) C.C.T.V., flash


photography and surveying instruments, non-BASEEFA approved
electrical/electronics apparatus.

x Environmental factors noise, temperature, dust, sewage spray etc.

x Is lifting and winching equipment necessary is there room?

x Lighting standards and equipment.

x Power requirements and electrical safety standards.

x Where the place of work within the confined space is greater than 10
minuites away from the cofined space entry point, full breathing
apparatus should be used for the duration of the work.

9.10 Operating Factors

Establish any operational constraints upon the work. Identify methods and
sequence of isolations or flow diversions necessary and the need for any
other permits to work (Mechanical, Electrical, Working at Height etc.

Consider the need for liaison with other operational groups both within and
outside the specific Directorate. This will include Contractors and Customers /
Clients who may effect or be affected by the work.

9.11 Personnel

Skills required consultants, contractors, direct labour, experience, training,


and qualification, manning levels. Physical fitness and specialist skills
supervision. Specialist scientific or engineering advice.

9.12 Planning and Preparation

Before the permit to work is issued for the commencement of operations the
CSAP will obtain all necessary ancillary permits to work, atmospheric test
safety clearance certificates, confirmation of isolations, flow diversion notices
and inspection certificates for plant and equipment etc.

Having thus established that the necessary engineering and environmental


controls have been applied, the CSAP confirms that manpower aspects have
been met with regard to trained, experienced personnel, adequately equipped
and competent to apply both the designated safe system of work and the
standard confined space high risk.

The CSAP will ensure that the facilities required for the emergency and
contingency planning, rescue and first aid, welfare and communications,
aspects covered by the documented system of work are available and in good
order.

9.13 Issue of Permit

The CSAP having considered that all potential hazards have been identified,
safety controls implemented and equipment provided will issue the permit to
work to an approved person CSCP The permit will be held available at the
worksite.

9.14 Variations

In the event of significant changes to the conditions or the operation of the


system of work the CSCP holding the permit will promptly notify the CSAP.
Whilst waiting further instructions the CSAP is authorised and expected to
take any necessary action to prevent danger. This includes authority to
suspend work.

9.15 Cancellation

At the completion of the operation or in the event of its abandonment the


CSAP holding the P.T.W. will complete the appropriate part of the permit
confirming that the confined space is evacuated and the site left safe.

10. Emergency Procedures

Emergency Procedures are referred to within the Procedures Section 9 as


part of the procedure and are also covered in the new FS training course
programmes as outlined in Section 10 below.

Prior to any work taking place, there must be suitable arrangements in place
for contacting the emergency services (fire and ambulance). Such
arrangements should be communicated to all of the rescue personnel.
Landline or mobile phone must be confirmed to be in working order, with
adequate reception in the case of mobile phones.

11. Training

FS have phased out the former 3 confined space courses that address
many of the training requirements as set out by the legislation. These courses
where as follows:

C1 - 2 Day training course is for Managers and Engineers,

C2 4 Day training course is for staff who at any level are required to
supervise, plan, work in or enter a Confined Space, and

C3 4 Day training course is for employees at any level who are


required to work in or enter a confined space.

11.1 Confined Space Training Courses

New FS Confined Space Course system was introduced in 2012 and is based on a
modular risk based approach and candidates who complete various modules will be
awarded Construction Skills Certification Scheme Cards (CSCS) and dependant on
the course taken a FETAC Level 5 or Level 6 Award as highlighted below. Staff who
obtain a a CSCS will be able to function as a CSCP for the specific category of risk
that they have been trained in and staff who been trained and awarded the CSCS
card for the CS/MA/11 can undertake the role of the CSAP.

CS/LR/11 Working in Low Risk Confined Spaces 1 Day

This FS National Training Programme is for employees at any level who are
required to work in or enter a LOW RISK confined space. If successful they qualify
for a FS CSCS Registration Card which enables them to work safely in low risk
confined spaces.

The purpose of the training event is to provide the participants with training in the
necessary skills and knowledge to operate successfully in LOW RISK confined
spaces utilising a safe systems approach to organising and controlling the work and
thereby comply with the Confined Space Code of Practice of 2010.

CS/MR/11 Working in Medium Risk Confined Spaces 1 Day

This FS National Training Programme is for employees at any level who are
required to work in or enter a MEDIUM RISK confined space. If successful and
having completed the Low Risk CS Course CS/ LR/11, within the preceding calendar
year, they will qualify for a FS CSCS Registration Card which enables them to work
safely in low risk confined spaces.

The purpose of the training event is to provide the participants with training in the
necessary skills and knowledge to operate successfully in MEDIUM RISK confined
spaces utilising a safe systems approach to organising and controlling the work and
thereby comply with the Confined Space Code of Practice of 2010.

CS/HR/11 Working in High Risk Confined Spaces 1 Day

This FS National Training Programme is for employees at any level who are
required to work in or enter a HIGH RISK confined space. If successful and having
completed both the Low Risk CS Course CS/ LR/11 and the Medium Risk Course
CS/MR/11 within the proceeding calendar year, they will qualify for a FS CSCS
Registration Card which enables them to work safely in low risk confined spaces.

The purpose of the training event is to provide the participants with training in the
necessary skills and knowledge to operate successfully in HIGH RISK confined
spaces utilising a safe systems approach to organising and controlling the work and
thereby comply with the Confined Space Code of Practice of 2010.

CS/RE/11 Search & Rescue for Working in High Risk Confined Spaces 1 Day

This FS National Training Programme is for employees at any level who are
required to work in confined spaces and require rescue teams. If successful they
qualify for a FS CSCS Registration Card which enables them to work safely in high
risk confined spaces.
On completion of this course along with the Low Medium and High training courses a
FETAC Level 5 certificate will be issued.

CS/MA/11 The Managing of Risk During Confined Space Working 1 Day

This FS National Training Programme is for line managers, supervisors and


engineers who are required to establish organisational policy, practice and
procedures as the outcome of risk assessments, design confined spaces and who
undertake the management of confined space entry.

On completion of this course participants will be awarded a CSCS card for the
management of risk during confined space working. It will not authorise participates
to enter any confined spaces. However, participants who have completed all 4
courses of Low, Medium and High Risk working including Search and Rescue along
with the completion of the Managing of Risk During CS working will be awarded a
FETAC Level 6 certificate.

Purpose of the training event is to provide an in-depth understanding of the


implications of the Confined Spaces Regulations, to comply with The Confined Space
Code of Practice 2010 and to develop the skills necessary for accurate assessment
and classification of risk for confined space working.

*Refresher training to be inputted when agreed with FAS.

Note: If staff still have a valid confined space C1/C2/C3 card (i.e. based on the old
system as outlined at the start of section 11) that has not yet expired, this will still be
valid and recognised under the new training procedures until the expiry date of the
card has elapsed.

Appendix 4
Procedures for Entering a Pump Station Drywell (for Pump Station Inspectors/Mechanical Crew
Pump Choke Callout).
1. Upon arrival at pump station, report immediately to the control room (never enter drywell immediately upon
arrival!).

2. Separate Key must be in use for control room & drywell. Keys for the drywell must be stored within the control
room, thereby forcing the inspector to access control room first before the drywell.

3. Check the Fixed-Wall Gas Detection System control panel (located in the control room) for gases in the
drywell.

4. Ensure drywell ventilation system is activated.

5. If gas alarm exists, open access doors of drywell and activate ventilation system (if not already running). If no
gas alarm, proceed to drywell.

6. When alarm clears on Fixed-Wall Gas Detection System control panel, inspector/mechanical staff to take
portable gas detector and check gases in drywell (for at least five minutes prior to entry). When testing gas in a
chamber, you must check at various levels in the chamber as different gases are heavier (e.g. H2S) & lighter
(e.g. CH4) than oxygen.

Appendix 5
Rescue Procedures

1. Low Risk Confined Space:


x In the case of a gas alarm in a low risk confined space, e.g. pump station
drywell, the staff member performs a self rescue.

2. Medium Risk Confined Space:


x All staff to don harnesses prior to confined space entry. Harness to remain
permanently attached to a lifeline. However, where an operative has to
traverse a distance within the confined space of such length that the line-
of-sight between the top-man and the the operative is no longer visible, a
second operative or bottom-man is required to stand at the invert level of
the confined space, permanently attached to the lifeline. The operative
traversing the confined space will remain connected to the bottom man by
means of a lanyard. If in the event that the operative collapses, the
bottom-man (whilst remaining in position) pulls the operative to the bottom
of the lifeline, and then connects the operative (via their harness) to the
hoist for rescue lifing.

x In a drywell it is of paramount importance to avoid the risk of a lifeline


getting caught in the rotating parts of the pumping infrastructure. In
drywells fitted with a staircase, the workers in the drywell can
perform a self rescue when they don their beathing escape sets.
Even if a staircase can be accessed within 30 seconds at any point in the
drywell, it is still advisable that a tripod is setup and rescue line lowered to
near basement floor level in case required for rescuing an operative who
has collapsed. Operatives should still however don a rescue harness
when working in a drywell considered a medium risk confined space area.
If in the event that the operative collapses, and there is no gas alarm, the
operative can be resuced from the drywell using a rescure stretcher. Or, if
there is a gas alarm, the operative can be attached to the rescue winch by
the bottom-man and lifted to safety. The bottom man then performs a
self-resuce via the staircase.

3. High Risk Confined Space:


x In the case of a high risk confined space, there must be adequate
personnel onsite trained to CS/RE/11, i.e. Search & Rescue for Working
in High Risk Confined Spaces.

4. Standard Procedures Common to All Confined Space Risks Above:


x Gas alarm sounding/flashing on the insitu gas monitor, evacuate the
confined space immediately and shout to your collegue(s) Gas and
Move Away.
x Move at once to the designated site assembly point and allow the
confined space to vent for at least 20 minuites. Personel onsite must be
aware of number of collegues onsite and check that they are all present

and accounted for at the assembly point. If any personnel are missing, do
not re-enter the confined space searching for them. Call the emergency
