4. Evaluation of Performance of relevant parties, individuals, and controls is not exercised. (Clause 4.5.1 Performance
Measurement and Monitoring)
5. Temporary electrical systems inspection regime is not implemented in the project site. DBs, cables, portable electric
appliances/equipment, generators etc. must be inspected on a weekly basis and prior to usage at site as need be.
6. Fall Protection Systems such as edge protection, lifelines, etc. is implemented but not thoroughly compliant to the
requirements.
7. Installation of Passenger Hoist for buildings higher than 5m as per DM BCS Periodical Inspection Report is not complied in
the project.
8. Fire Prevention plan is not fully implemented such as removal of combustible materials, cable management and
inspection, adequate fire points with signage, etc.
9. Required PPEs are not strictly adhered to by the ECCs labor force including their sub-contractors such as suitable hand
protection, goggles, mask, etc.
10. Scaffolding safety is not fully administered in the site as evident on unsafe scaffoldings erected and used without tags.
11. Site fence and adjacent asphalt road is not maintained on regular basis by ECC
12. ECCs housekeeping daily regime is not effective and implemented thoroughly.
13. Adequate illumination to enable safe operations must be provided in all floors and buildings.
14. Minimum of two staircases clear of any obstruction for safe access and egress is not provided for some buildings.
15. Safety net fans not provided as per DM regulation stipulated in Code of Construction Safety Practice.
16. Hot works Permit System not thoroughly and strictly implemented.
