MINE SURVEYORS CERTIFICATE OF COMPETENCY

APPLICATION FOR ACCEPTANCE

INSTRUCTIONS:
1. Refer to the attached payment options and submit proof of payment for the amount of R50.00 with this application.
2. Telephonic enquiries can be directed to the Secretary at 012 444 3545.
3. Applicants must be 23 years of age or above.
4. The following originally certified copies from South African Police Service (SAPS) not older than three (3)
months must be attached to this application arranged in the following order:
Identity Document
Marriage certificate if applicant married after application was submitted
Letter of evaluation from SAQA, if foreign qualification
Equivalent qualification recognized by the Commission of Examiners
Qualifications: ND in Mine Surveying Degree, COM advance certificates in Mine Valuation and Surveying
Letter of sobriety (good conduct) from Chief Surveyor on a formal letterhead of the mining company with
contact details
Record of Service on mines letterhead with contact details as proof of sufficient surveying experience
NB: Incomplete documents will not be considered

PAYMENT OPTIONS

OPTION 1

Candidates may pay the required amount directly into the Head Office account at any
ABSA Bank, Account Number 406176 9154, with 010 194 26 OT 12 as the reference
number.

The original bank deposit slip must be attached to the application form as proof of
payment.

OPTION 2

Candidates may pay the required amount to the cashier at any Regional office of the
Department of Mineral Resources (DMR). The cashier must be informed that the
payment is for examination purposes.

The original cashier receipt must be attached to the application form as proof of
payment.
