STEPS IN APPLYING FOR A NEW BUSINESS

STEP 1
Barangay Clearance
1. Valid I.D of the owner.
2. Php 00 for the payment

STEP 2
DEPARTMENT OF TRADE AND INDUSTRY
1. Accomplished application form
2. Complete and pass all necessary requirements
3. For the business name registration, a registration fee based on
the applied code ( e,g. Barangay code Php.200.00) and
documentary stamp amounting Php.15.00 is required.

STEP 3
1. Fully accomplished application.
2. Barangay Clearance
3. DTI or SEC Registration
4. Pay the corresponding taxes and al regulatory fees.l

Location Clearance
Certification of Occupancy
Health Certificate
Community Tax Certificate ( Cedula )
Fire Safety Inspection Certificate
CENRO Form ( Hygienic Septic Tank )
PESO Form
5. After the assessment of taxes and fees, the treasury office will
receive the payment and release the official receipt.
6. The BPLO will verify, sign, issue the Mayors Permit together with the
business plate/sticker.

STEP 4

1. Fill-up the BIR Form- 1901 for Application for Registration ( for
Sole Proprietor) , BIR Form- 0605- Payment Form
2. DTI Permit
3. Mayors Permit
4. Proof of address such as Contract of Lease (if rented) or
Certificate of Land Owned (if owned)
5. After the assessment and submission of requirements you should
pay the following fees:
Annual registration fee of PHP. 500.00
Books of accounts VAT/NON- VAT of PHP. 100.00
Certification fee of PHP. 100.00
Documentary Stamp- PHP. 15. 00
The RDO shall issue the Certificate of Registration and ATP to 2-3 days
of your registration.

REGISTRATION WITH BARANGAY

1. Fill-up application form


2. After the completion of application form submit together with
the following requirements
Valid I.D of the owner
3. Releasing of Barangay Certificate of Business Registration.

DTI Business Registration

Department of Trade and Industry

Requirements:
1.DTI Application Form
2. 1 Valid id
3. List of 3-5 proposed business name, for DTI Business Registration.
4. Reg. Fee depending on territorial jurisdiction
Barangay- PHP.200,
City PHP.500,
Regional-PHP.1,000
National PHP.2,000

Procedure:

1. Obtain application forms (duplicate copy) and fill these up


completely.
2. Meet the following requirements
3. Submit application form to the DTI Processor. The DTI Processor
will check if the Business Name is still available, if yes, you will be
asked to pay the application fee.
4. Pay the required registration and processing fee.
The Department of Trade and Industry (DTI) is now implementing
the following registration fees for business name registration
(original and renewal) depending on the territorial jurisdiction
covered in the application:

a. Barangay: PHP 200.00


b. City / Municipality: PHP 500.00
c. Regional: PHP 1,000.00
d. National: PHP 2,000.00

5. After showing the receipt to the Processor, the Business Name


Certificate will be released

6. Your Business Name Certificate is valid for 5 years from date of


registration.

Acquisition of New Business Permit


Mayors/Business Permit

Requirements:
1. Duly Accomplished Application Form
2. DTI or SEC Registration
3. Barangay Clearance
4. Tax Clearance or Certification of No Real Property
5. Location Clearance
6. Certificate of Occupancy/Clearance from the Local Building
Official
7. Health Certificate
8. Fire Safety Inspection Certificate
9. Community Tax Certificate (Cedula)
10. CENRO Form ( Hygienic Septic Tank)
11. PESO Form ( City Ordinance No.07-2003)

Procedure:
1. Inquire Application Form and secure list of requirements
2. Fill-up the Application Form and Complete all the requirements
3. File your notarized application form and complete the
requirements.
4. Wait for the evaluation and assessment of application.
5. Pay the corresponding business taxes and all regulatory fees
Based on: Capital arealine of the business and number of
employee.
6. Present the official receipt and wait for the final validation,
approval and releasing of Mayors Permit and Business
Plate/sticker
BIR Registration
Procedure:
1. Accomplished BIR Form 1901 for Application for Registration (for
Sole Proprietor),BIR Form- 0605- Payment Form and submit the
same, together with the required attachments to the RDO having
jurisdiction over the registered address of the business
establishments
2. Proceeds to the counter for the assessment and submit the
following requirements:
BIR Forms- 1901,0605
Birth Certificate
Mayors Permit
DTI certification of registration of business name.
3. Pay the following fees:
annual registration fee of PHP. 500.00 at authorized agent banks
of the RDO
Certification Fee of PHP.100.00
Documentary Stamp of PHP.15.00
4. After paying submit the requirements for ATP,BIR Form 1906, and
registration of book of accounts
5. Attend the taxpayer initial briefing conducted by the RDO for new
registrants in order to appraise them of their rights,duties and
responsibilities.
6. Wait for the issuance of Certification of Registration by the RDO.
