To whom it may concern,

This correspondence is in regards to Javi Perez a University of Arizona BSN student graduating
in the fall class 2017. I am honored to write this letter of recommendation for Javi, a new
graduate registered nurse applicant. I had the pleasure to work with Javi during this fall 2017
semester as he was a clinical student in NURS 479 Transition to the Professional Nursing Role
as well as mentored academically in NURS 478 Nursing Leadership and Management in Health
Systems.

Javi demonstrated professionalism and accountability as he arrived early every shift to meet with
the preceptor to prepare for the day. Mr. Perez completed 180 practicum hours in the
Perioperative units which included pre-op, operating room and the pacu at St. Marys Hospital in
Tucson Arizona. Javi was prepared and eager to learn and not afraid to take on any challenge
presented to him. Javi demonstrated flexibility as he floated between the perioperative units
acquiring new skills and knowledge. Javi protected patient privacy respecting HIPAA policies.
Mr. Perez made himself available for multiple educational learning opportunities in the
perioperative environment. Javi attended the active shooter, arterial blood gas analysis and
violence in the workplace presentations just to name a few.

Javi demonstrated competency with pre and post-op assessments & skills as he provided care for
multiple patients per shift. Javi inserted 5-7 IVs per shift. Once aware of expectations, Javi
required minimal supervision from the preceptor. Javi increased his efficiency when preparing
patients for surgery as well as increased his knowledgebase with documentation and pacu
protocols.

Safety was a high priority as medications were administered following all rights to medication
administration. Infection control was also a priority as Javi maintained a sterile field as he
assisted with prepping patients in the operating room opening multiple sterile packages.

Javi not only excelled with assessments and skills he was not afraid to demonstrate leadership
qualities. Javi developed an immediate rapport with patients, families, and staff. Javi gave
report to the pacu staff and assisted with discharge teaching each time building his self-
confidence.

Javis caring attitude and passion for nursing were demonstrated throughout the clinical
experience. The University of Arizona College of Nursing has prepared Javi with a strong work
ethic to succeed and learn at the next level. I highly recommend Javi Perez for a registered nurse
position. Javi truly demonstrates professionalism which makes him a perfect candidate for your
