INTRODUCTION

1.1 General

This Plan shall apply for the Bandar Abbas Gas Condensate Refinery Project.

It is imperative that all personnel involved in the project, Contractor, subcontractors,


suppliers and vendors, are familiar with the scope of this document.

The Contractor Site Manager will ensure that this document is regularly reviewed at site.

All parties will be informed of subsequent revisions.

The Contractor is responsible for execution of the project work to the highest level of quality
and completion on schedule.

At no time, HSE performance and considerations will be subordinate to quality, cost or


schedule.

Distribution (see below) of this HSE construction Plan shall be controlled by the Contractor
Project Manager.

Changes to this document, generated on site by the Contractor Field HSE Coordinator,
shall be submitted to the Contractor Project HSE Manager for control.

Distribution of this document will be to:

1.2 Scope

This document sets out how HSE issues will be identified and managed during the
Construction phase of the Project.

Emergencies are explained in the Emergency Response Plan for Construction.

This document applies to Contractor, subcontractors, suppliers and vendors, who perform
activities in the project.

The Contractor Site Manager will ensure that all activities included in subcontractors scope
of work at site will be carried out in compliance with this HSE constructions Plan for
Construction.

All subcontractors shall follow this document and their specific HSE Plan; see section no.
8.5, Subcontractor Requirements.

1.3 Leadership and Commitment details

Contractor Project Director and Site Manager accept the ultimate responsibility for the
proper implementation of this HSE Plan for Construction, and shall mantain Contractor
commitment to the continual improvement of HSE culture within the organisation.

Contractor Project Director and Site Manager shall provide full support to the Contractor
Field HSE Coordinator and ensure that field managers and supervisors accept their moral
and legal duties regarding HSE during the life of this project, and that they are held
accountable for the same.

4 HSE POLICY AND OBJECTIVES

4.1 Contractor HSE Policy

The Construction phase, as well as each phase of the Project, will fully comply with
Contractors HSE Policy Statements.

This HSE Policy has been designed to meet the requirements of Environmental
Management Systems ISO 14001, Occupational Health and Safety Assessment OHSAS
18001.

Compliance with the Contractor HSE Policy is also an instrument for implementing and
ensuring compliance with the Employer HSE Expectations, and legislation in order to
provide a safe and environmentally acceptable Project.

A system of identifying and managing HSE risks, inclusive of technical integrity, will be
implemented proactively in the design, construction and methods of hand over to the final
hand users.

4.2 HSE Objectives

In order to satisfy and fully comply with the HSE Policy, the following requirements shall be
followed:

Development of all the necessary HSE Plans by the Contractor Management Team
and Subcontractors;
Definition of methods and activities for the correct application of the HSE Policy
requirements to the Project;
Definition of roles and responsibilities for key HSE personnel. It is required that the
Project Director/Site Manager has responsibility for ensuring all personnel recruited
into Project shall have appropriate HSE skills and experiences;
HSE training implementation; provision in particular of information concerning risk
and environmental management aspects;
Compliance to the applicable HSE standards, procedures and practices;
achievement of HSE targets and objectives specifically tailored for this phase of the
Project;
Perform audits in order to check the implementation of the HSE Construction Plans.

5 HSE ORGANISATION, FUNCTIONS AND RESPONSABILITIES

5.1 Organization and interfaces

The Project is executed by the Contractor under the overall direction of the Project Director.
The Contractor HSE Management System is implemented through a project-based
organizational structure, which refers to the Contractor HSE Department .The system is
also implemented following suggestions and recommendations by Employer, auditing the
site on planned basis, monitoring the effectiveness of non conformities corrective actions
and enforcing Contractor Site organization structure (following Site Manager and FHSEC
reccomandations).

To enforce the Policys principles, the Project HSE Team is appointed to perform all HSE
related activities, including specialist studies, audits, reviews.
The Contractor Project HSE Team Organization during Construction phase, together with
the definition of roles and responsibilities are detailed in the Project Team Organisation, Job
Document No. 3034-00-ED-QM-ORC-AA607.

The Contractor HSE Team, based at the work site, shall include the following:

o Field HSE Coordinator;


o HSE Supervisor;
o HSE Officers;
o Security Supervisor;
o Training Officer.

A functional link between the Contractor HSE Manager and the Field HSE Coordinator is
ensured at all times, to guarantee a uniform approach and implementation of the HSE
Construction Plan as a whole.

The Contractor Site Manager is responsible for all site activities, including HSE, Quality and
performances.

The Contractor Site Manager is responsible for the interfaces with:

Employer
Police, Civil defence and Hospital;
Local Authorities, Municipality;
Subcontractors;
Suppliers and Vendors.

Emergency interfaces are detailed in the Emergency Response Plan for Construction.

5.2 Communications

Contractor Site Manager is responsible for maintaining effective communications with


Employer subcontractors, suppliers and vendors, local emergency services and local
authorities.

Communication between Contractor and other organisations during the Construction phase
will be, in case of emergency by telephone or radio, and generally by mobile telephone,
verbal and by written letter.

In particular mobile telephones are not intrinsically safe the Contractor Site Manager shall
ensure that, prior to entry into a restricted area, persons with a mobile telephone shall leave
it with security outside the restricted area until their exit. In such cases, the person entering
a restricted area shall appoint a competent person to carry out their duties and
responsibilities during this temporary absence in communications.

The Contractor Field HSE Coordinator is responsible for logistical support of HSE
communications for line management.

The Contractor Site Manager shall also ensure that all site based Project personnel
(Contractor and subcontractors) are kept informed of:

HSE developments (e.g. procedures developed and implemented);


HSE disciplinary actions;
HSE performance (e.g. frequency rates and accident analyses);
HSE audit, review and inspections.

The Contractor Field HSE Coordinator will ensure that:

dedicated HSE notice boards are erected as required around the work site,
administration and management offices (facilities) as necessary;
HSE posters, HSE flashes and other notices in project languages are available for
display, and that site notice boards are changed on a monthly basis, or as dictated
by site activities and events;
personnel in management and supervisory roles are provided with a fully functional
mobile telephone and/or radio for the Construction Works.

5.3 HSE Meetings

Mandatory HSE Meetings for the Construction Works shall be:

Site Management HSE Committee;


Site Management HSE Meeting;
Supervisors HSE Meeting;
Workforce Tool Box Meeting;
Daily pre-job talk.

5.3.1 Site Management HSE Committee

Site Management HSE Committee is established by Contractor and chaired by the


Contractor Site Manager.

The Contractor Field HSE Coordinator, subcontractors Site Representative, subcontractors


HSE manager, Client Representative shall attend this meeting.

The Contractor Field HSE Coordinator shall set-up the rules of the committee and record
the minutes.

The committee shall meet on a monthly basis, or more frequently if required.

5.3.2 Site Management HSE Meeting

The Contractor Site Manager will sponsor and co-ordinate this meeting.

The Contractor Site Manager, Field HSE Coordinator, all supervisors (Contractor and
subcontractor) will attend this twice monthly meeting.

Standing items of this meeting will be:

answers obtained to questions unanswered at the last meeting;


accident reports, statistics and analyses;
review of the Contractor HSE developments and on-going items;
staff questions, point of view;
planned safety and health topic relevant to current work.

Contractor Field HSE Coordinator will prepare the agenda and minute the meeting.

5.3.3 Supervisors HSE Meeting

Weekly meetings shall be held to brief supervisors on their HSE responsibilities.

The meetings shall provide a mechanism for supervisors to make their contribution to the
Contractor and Client HSE Policy (special meetings shall be held as required).

The Contractor Field HSE Coordinator shall organise the meetings, prepare an agenda and
a written summary of each meeting highlighting the major discussion items, and unresolved
issues.

Standing items of this meeting will be:

answers obtained to questions unanswered at the last meeting;


accident narrative reports, statistics and analyses;
review of HSE issues;
foremen's questions, point of view;
planned safety and health topic relevant to current work;
ensure those present sign to record their attendance.

Special meetings may be held as necessary.

5.3.4 Workforce Tool Box Meeting

These talks will be topical and weekly.

The subcontractor HSE Manager shall ensure that supervisors and their foremen are:

conducting Tool Box Talks with their crew each week using their own and/or material
furnished by the Contractor Field HSE Coordinator, and that the talk is given in
languages understood by all attendees;
providing employees with the opportunity to ask questions regarding HSE issues
and providing responses to these in a timely fashion;
having all attendees sign the HSE meeting report form during the meeting;
submitting a completed Tool Box report form to the general foreman or lead
supervisor/manager for review and action, then to the superintendent for review and
action if required, and finally to the Contractor Field HSE Coordinator.

Tool Box Meeting shall as minimum include:

answers to questions raised at the last meeting;


new HSE issues;
accidents or near misses occurred in that period;
topic / work for the day;
questions;
specific Hazard Identification / Hazard Analysis.

5.3.5 Daily pre-job talk

This shall be carried out before each job activity (including a change of work or location) to
reinforce how the work is to be performed in a safe and healthy and environmental friendly
manner, and to discuss associated HSE risks.

It should take approximately 5 10 minutes and shall be given by the supervisor or foreman
in sufficient languages to enable all workers to fully understand.

A record shall be maintained of these talks for audit purposes.

5.4 HSE Training

HSE Training for the Construction Works shall include as a minimum:

Project HSE Orientation and Induction;


Subcontractor(s) HSE Training;
Permit to Work training.

5.4.1 Project HSE Orientation and Induction

All personnel at all levels of the project structure (Contractor, subcontractors, suppliers and
vendors) shall receive Project HSE Orientation and Induction and specific training prior to
starting any work activity.

A permanent personal security badge will only be issued upon satisfactory completion of the
Project HSE Orientation and Induction.

The Contractor Field HSE Coordinator shall ensure that all personnel attending induction
receive as minimum, relevant and up to date information about the following:

description of the project as a whole;


introduction to the Project organisation and principal personnel;
employer and employee HSE responsibilities under Iran Law, Contractor and Client
rules;

Contractor HSE Integrated Management System, HSE Policy and objectives;


how to obtain first aid and medical treatment;
information on First Aid facilities;
how to recognise and response to hazardous and emergency situations, and how to
contact relevant personnel;
alarm systems in operation (Evacuation, Hydrocarbon gas leak, H2S gas leak,
Chlorine leak, Ammonia and etc.);
fire prevention;
information regarding toxic and flammable gas hazards;
information regarding fire and explosion hazards;
action to take in the event of a fire or explosion;
action to take in the event of a toxic or flammable gas release;
HSE practices and work procedures (personal issue of handbooks);
road transport and driving;
permit to work systems ,
security;
HSE issues and requirements in accommodation camps (if any);
hazard identification and risk assessments;
environmental aspects identification and impact assessment;
environmental topics related to the use of hazardous substances on the basis of
what suggested in Material Safety Data Sheets (MSDSs) concerning;
information about existing procedures regarding the correct management of wastes,
wastewater, effluents, noise, dust as well as environmental emergencies;
the potential consequences of deviation from these standard operating procedures;
how to recognise and protect against heat stress;
hygiene.

These induction sessions will be documented by the Contractor Field HSE Coordinator.

5.4.2 Subcontractor(s) HSE Training

Subcontractors will also be required to continually reinforce the HSE message to their
employees on general site rules and train them in the HSE Requirements of their particular
work.

As a minimum, Subcontractors shall inform their employees on:

Project wide rules regarding general PPE;


Project traffic rules;
Project hygiene rules;
The need to report unsafe conditions and practices;
Policies and procedures for:

o Accident and illness reporting;


o Permit to work operations;
o Fall prevention;
o Housekeeping;
o Lockout and tagging.

As a minimum, Subcontractors shall inform their employees on their own procedures carried
out in compliance with Contractor procedures:

Material handling;

Waste management;
Dust reduction measures;
Noise reduction measures;
Electrical equipment;
Welding and cutting.

Subcontractors shall also:

Inform their employees of the safe work methods to be adopted in their particular
work including the use of PPE;
Inform their employees of any special HSE rules for working near operating plant;
Inform their managers, supervisors and personnel of the emergency procedures so
that they may provide the leadership and control required.

These training sessions shall be documented and the minute of meeting will be send to the
Contractor Field HSE Coordinator.

The Contractor Field HSE Coordinator shall monitor subcontractors' education and training
activities to ensure that satisfactory standards are maintained.

5.5 Responsibilities on site

5.5.1 Contractor Site Manager (SM)

The SM shall be responsible for all activities including overall HSE, Quality and
performance.

The SM shall:

assume a leadership role in the HSE process;


ensure that all line management and supervision conduct their duties in a manner
which supports the Zero Accident Tolerance philosophy;
ensure the safe and healthy completion of all construction activities for Construction
phase;
set standards for safe and healthy execution of construction work, and ensure full
dedication by all personnel to Zero Accident Tolerance;
ensure that all site personnel are competent, qualified and adequately trained to
perform duties given to them during the course of the Construction phase;
establish and maintain a direct line of communication with the Contractor Field HSE
Coordinator;
ensure sufficient resources are available to effectively manage HSE issues;
implement an employee incentive scheme;
ensure the provision of adequate fire prevention and protection measures and
equipment.

The SM for the Emergency Response Plan shall:

o ensure that basic site medical facilities (first aid) and medical support are in place
before and throughout Construction Works;
o ensure that the Emergency Response Plan is fully implemented;

The SM for the HSE Meeting shall:

attend and participate in relevant HSE Meetings (Site Management Safety


Committee and Site Management Safety Meeting);
ensure that Contractor line management and supervisors participate, contribute
actively and exercise leadership in their respective individual and collective project
safety meetings;
ensure that HSE is a regular agenda item at site management meetings;
conduct unscheduled meetings as necessary at his discretion or on request of the
Contractor Field HSE Coordinator.

The SM for the Audit and Review shall:

lead investigations into serious accidents or potentially serious incidents;


support accident reporting motivation;
take corrective action with any Department, craft or group having an unsatisfactory
accident record.

The SM for the PPE will:

- ensure the Contractor PPE needs are adequately funded.

The SM for the Hazard Management shall:

ensure that resources and provisions are available for the effective management of
all anticipated hazards on the project;
ensure that every work activity performed by the Contractor and its subcontractors,
vendors and suppliers (if any) is executed in a safe and controlled manner;
ensure that the HSE Management System works efficiently, that sufficient resources
are devoted to it, and that construction activities for Construction phase are in
compliance with the Contractor and Client HSE requirements.

The SM shall:

appoint a person for the role of Permit Coordinator to be responsible for the Permit
to work.

Note: in case the SM is not be available at Site, he will nominate a substitute and
communicate this to all concerned.

5.5.2 Contractor Project HSE Manager (HSEM)

The HSEM (based at the Home Office) manages and supervises the Project HSE activities
throughout the entire development of the Project (Home Office and site activities), in order
to guarantee the correct application of the Project HSE Management System.

The HSEM shall:

provide HSE guidance to the Project;


provide professional direction and guidance to the Contractor Field HSE
Coordinator;
ensure necessary reviews, and updating to this document;
ensure that the project receives the HSE Services that it requires, and maintains
awareness of the Contractor management's and Client evaluation of that service;
coordinate and lead HSE Audit and Review activities for the project (Home Office
and site);
be the prime Contractor contact for HSE matters;

5.5.3 Contractor Field HSE Coordinator (FHSEC)

The FHSEC will operate under the coordination provided by the HSEM in order to support
and monitor the management and implementation of all relevant HSE regulations and the
application of the Project HSE Management System.

The FHSEC responsibilities include:

direct reporting to the SM and functional reporting to the HSEM for the HSE matters;
updating of this document and verifying correct implementation;
promotion of the Zero Accident Tolerance Philosophy;
management of the resources and HSE personnel required for the fulfilment of HSE
tasks in Construction phase;
maintaining contacts with Client and Local Authorities in charge of HSE matters;
liaise with the subcontractors' HSE Manager(s);
notifying the HSEM and the SM, of any HSE related incident/accident involving
personnel, equipment, property or environment;
liaise with Contractor construction departments and management;
liaise with subcontractors, suppliers and vendors on HSE issues;
ensure that reporting of mandatory HSE matters to the appropriate regulatory
authorities is timely and accurate.

The FHSEC for the Emergency Response Plan will:

o report HSE incidents immediately to the HSEM and the SM;


o ensure plans exist for dealing with potential emergency situations;
o ensure subcontractors have appropriate fire prevention procedures in their HSE
documents, and that they apply them;
o investigate injury, damage and near miss, accidents independent of subcontractors
HSE Manager or jointly with them.

The FHSEC for the HSE Meetings shall:

prepare and minute the Site Management Safety Meeting


organise, attend and participate in relevant HSE Meetings (Supervisors Safety
Meeting);
support supervisors and foremen with specific meeting format and content as
necessary;
generate a monthly log of meetings scheduled, meetings held, topics and
attendance and submit the log to the HSEM, the SM and the Client.

The FHSEC shall conduct regular site HSE Audits, Reviews and Inspections. The
objectives are detailed in the following:

check implementation/compliance with the HSE documentation (updating and


reviewing, as required);
conduct regular Inspections;
check the implementation, content, participation and effectiveness of specific
dedicated meetings (see 5.3, 5.4 sections) verifing compliance with Contractor HSE
requirements;
examine accident reports, perform statistical analyses and publicise the results as
necessary;
prepare a monthly HSE progress report for the HSEM and the SM (see par. 8.2,
Safety Statistics Monthly Report Form).

The FHSEC for the Training shall:

ensuring that all personnel involved with Construction activities, including


subcontractors and vendors (if any), are adequately instructed of the Project HSE
Requirements;
ensure training of all site personnel (on HSE matters) at any level of the project
structure (Project HSE Orientation and Contractor Induction);

arranged HSE training sessions (and specific training prior to start any work activity).

The FHSEC for PPE will:

- ensure compliance with local law or more stringent standards, contract, procedures,
Client standards relating to the provision and use of PPE.

The FHSEC for the Hazard Management shall:

ensure that all personnel are aware of the HSE requirements of their activities;
manage the waste streams, compile and issue of documents and reports required in
accordance with the Waste Management;
plan and carry out, when necessary, studies and/or hazard analyses.

Note: should the FHSEC not be available at Site, he will nominate a competent person to
carry out his duties and responsibilities, and communicate this to all concerned.

5.5.4 Contractor HSE Supervisor

The HSE Supervisor shall:

take ownership of Health, Safety, Environment issues within areas under their direct
control and influence;
ensure that a safe and healthy workplace and works are maintained at all times;
ensure that all personnel under his charge are suitably qualified, experienced and
competent to perform tasks requested of them in a safe manner;
ensure that proper instruction, training and supervision is provided to all personnel
under his charge;
assess training needs for personnel under his charge, and request to provide the
necessary courses;
take the lead role in accident or incident investigations relating to personnel or
property under his charge, and ensure that the necessary reports are submitted in
good time;
conduct regular Health, Safety, Environment inspections and risk assessments;
attend all required meetings;
ensure that suitable personal protective clothing is issued as and when required,
and that personnel under his charge wear the same;
ensure the compliance with the identified environmental protection requirements
during the construction activities;
ensure that all construction wastes, including wastes from construction camps, are
properly segregated, stored, transported and disposed of in accordance with the
Construction Environmental Management Plan and Client guidelines as a minimum;
keep and control records of wastes generated, stored and disposed of;
carries out professional safety and occupational health duties as required by the
FHSEC;

provide the professional oversight, guidance and resources to line management and
supervision to ensure Contractor achieves safety objectives and target of Zero
Accidents/Incidents;
reports and follows up implementation of review findings and recommendations to
FHSEC.

5.5.5 Contractor Safety Officers

They will:

report directly to the FHSEC and Contractor HSE Supervisor;


understand the requirements of Contractors Safety Plan and Client Policy and
Requirements;
inspect the work site daily to report and correct unsafe methods and conditions;
set a personal example.

5.5.6 Contractor Training Officer

The Training Officer shall ensure that personnel are aware of:

Contractor and Client organisation and principal personnel;


employer and employee HSE responsibilities under Contractor and Client rules;
project HSE Policy and objectives;
education and training programme;
Health, Safety and Environmental practices;
foreman's tool box talks;
HSE information communication programme;
HSE incentive programme;
HSE discipline programme;
Reporting hazardous conditions;
other matters as necessary, housekeeping and sanitation procedures;
Risk assessments.

The Training Officer shall also develop and implement a training programme covering
aspects such as abrasive wheels, enclosed spaces, breathing apparatus, harnesses,
driving, etc.

5.5.7 Contractor Security Supervisor

The Security Supervisor shall:

report directly to the Site Manager / Human Resources;


ensure the proper application of the Site Security and Access Plan;
assist the FHSEC in the provision and dissemination of adequate security training
material and information;
verify that the Site Security and Access Plan is fully implemented and complied with,
performing frequent inspections to ensure efficiency and performance of all security
related aspects;
establish gate control whenever/wherever necessary and ensure accurate control of
vehicular transit, including goods and equipment and as well provide and verify the
personnel identification, through identification system (personal pass or badge) and
recording at check points;

provide routine control of efficiency of communications devices;


ensure that adequate protection of premises/camps/sites with fencing & gates are in
place;
provide and monitor the round-the-clock watchman/guards surveillance services for
the project installations and all temporary facilities;
ensure effective information & communications (posters, bulletins, etc.) and shows
in different languages to all personnel involved;
traffic and pedestrian control;
provide any support on security issues to Contractor and Client as far as reasonably
practicable;
carry out investigations and reporting of any security related incident;
ensure that security lights are operational and report any maintenance requirements
to FHSEC.

6 RISK EVALUATION AND MANAGMENT

6.1 Risk Assessment

Contractor line management shall ensure that every work activity performed by Contractor
and its subcontractors, vendors and suppliers is executed in a safe and controlled manner.

All tasks to be performed by Contractor and subcontractors shall be subject to a written


Method Statement, a Risk Assessment and Job Safety Analysis (JSA) carried out by
competent persons, and for the submitted for prior approval by the Engineer, Supervisor
and FHSEC.

A Risk assessment shall be required for each Subcontractor, and mitigation measures shall
be implemented before work commences.

Contractor construction/HSE team will check the Subcontractors Risk Assessments to verify
together the Subcontractors the applicability of the work method statement proposed to
reduce the risks analyzed step by step on the JSA.

Further guidance may be found in the Attachment N. 10.1 (HSE Procedures for
Construction phase).

6.1.2 Risk from existing refinery

Personnel entering inside the refinery where there is the possible risk of a toxic or
flammable gas cloud shall receive appropriate instruction and training by the Refinery and
shall be equipped by the belonging company with the required PPE and gas detector.
Personnel shall be respect all refinery procedures especially work-permit procedure.

For further details of risks from external hazards, mitigation measures and actions to be
taken in case of alarm please refer to the Emergency Response Plan for Construction.

6.1.3 Underground electrical and gas network

Before excavation works commence on the construction area all information concerning
electrical and gas networks shall be collected from Contractors and Subcontractors using all
possible information channels: client, Iranian electrical governmental authority, Iranian gas
governmental authority, existing refinery.

6.2 Procedures for hazard control

Contractor shall ensure that work method statements (otherwise known as work
procedures, work instructions) are available for all activities.

These shall include, but not restricted to, the following:

The procedures may be modified and expanded (by the FHSEC), as required, throughout
the duration of the Construction phase.

For all hazardous materials used by the subcontractors, the FHSEC shall ensure that:

an MSDS (material safety data sheet) is obtained before delivery to site of any
hazardous material;
there exist appropriate facilities for proper unloading and storage;
information on the hazards of the materials is properly communicated to the users;

employees shall be trained in the safe use of the materials, including personal
protective equipment and emergency procedures;
a written procedure exists for their use and disposal.

The FHSEC shall also ensure that an inventory is kept and made available on demand.

The following are among those which should be in the inventory:

Paints, coatings, thinners, solvents and cleaning agents;


Insulating materials such as fibre glass and ceramics;
Sandblasting materials;
Compressed gasses such as oxygen, nitrogen, argon, helium, acetylene;
Greases, oils, and other lubricants;
Fuel such as gasoline, diesel, kerosene;
Epoxy resins / sealant;
Bulk containers of household products and disinfectants;
Cement.

6.3 Emergency Response Procedures

Emergency procedures are found in the Emergency Response Plan for Construction
These are the contents of the Emergency Response Plan:

Layout of Work Site and camp;


Emergency service provisions;
Access/escape provisions;
Alarms and assembly points;
First aid and medical treatment;
Emergency phone numbers;
Nomination of persons responsible for implement the Emergency Response Plan;
Nomination of persons responsible for management of an emergency situation;
Development of procedures for declaring, communicating and controlling during the
emergency;
Evacuation provisions of site working areas and facilities area;
Monitoring, testing and maintenance of alarm systems and early warning devices;
Designation of emergency teams and the definition of duties of other key personnel;
Liaison with external services (Police, Ambulance services, Hospitals, Fire Station
and other governmental agencies);
Training and realistic exercises for emergencies;
Effect the rescue and treatment of casualties;
Contain and bring the incident under control.

It is imperative that every employee working on site is familiar with the scope of the
Emergency Response Plan.

Basic emergency actions shall be comprehensively addressed in the compulsory Project


HSE Orientation and Induction training for all site personnel.

6.4 Personal Protective Equipment

Contractor and subcontractors shall ensure that all their personnel are issued with, trained
in their use, and use suitable PPE as SAIPEM Corporate, Contractor standard requirements
and standards of the country.

The minimum PPE required for all personnel during the Construction Works on site will be:

Safety helmet;
Safety glasses;
Safety shoes.

Additional PPE shall be issued dependant on specific work activities and/or a specific work
area, and where required shall be specified in Permits to Work.

Such additional PPE may include, but not be limited to the following:

o Coveralls;
o Protective gloves;
o Safety goggles;
o Welding/grinding protection;
o Hearing protection;
o Respiratory protection;
o Fire proof Coveralls.

For employees working in a Toxic gas cloud Risk Area they shall also be issued with:

Escape masks;
Portable Toxic gas detectors.

The FHSEC will assist subcontractors HSE Manager in:

- critically examining work methods to eliminate the use of additional PPE wherever
possible;
- determining their PPE needs;
- selecting and procuring the PPE;
- educating and training their workforce in the proper use and maintenance of PPE;
- motivating proper use of issued PPE.

The FHSEC shall:

specify PPE requirements when its use is unavoidable;


assist procurement in locating and obtaining PPE appropriate to the project needs;
agree with Contractor management on a procedure for the issue, re-issue and
charging for replacement of, and discipline actions related to PPE;
ensure that PPE users are trained in the use and maintenance of PPE.

6.5 Conditions for suspending work

Conditions for suspending work (e.g. weather, adjacent site operations, etc.) during
Construction Works may be:

in the event of a release of toxic or flammable gas from other areas;

7 STANDARDS AND PROCEDURES

7.1 HSE Standards and legislation

The Contractor and its subcontractors shall comply with the requirement of all relevant
legislations, SAIPEM CORPORATE and SNAMPROGETTI procedures and other
specifications including but limited to:

all Iranian Local Statutory regulations applicable listed in paragraph no. 2


References;
all Contractual documents listed in paragraph no. 2 References.

7.2 Procedures

For the general HSE procedures, refer to this Safety Plan for construction comprehensive of
the all procedures and the subcontractors specific HSE procedures.

7.3 Contractor and subcontractors equipment standards

The SM shall ensure that equipment brought to site is of a high standard and conforms to
Iranian laws and regulations, codes of practice and guidelines for equipment.

The SM shall ensure that arrangements are in place and implemented for the issue of
certificates for all equipment in accordance with Client Requirements.

The FHSEC shall ensure that a register of all equipment is kept on site and review the
equipment schedule with the subcontractors.

Subcontractors are wholly responsible for the provision and maintenance of equipment
brought to site, specifically they will assign responsibility within their organisation for the
control and maintenance of their equipment and issue certificates of competence for their
operators and ensure that only persons competent are employed on site.

The following basic equipment has been identified for the Construction Works:

o trucks and cars;


o crane;
o excavators, graders and compactors;
o cement mixer and vibration tools;
o hand tools and power tools;
o welding units.

A list of equipment shall be provided daily on site by the subcontractor to the SM.

7.4 Occupational Health procedures

Contractor and subcontractors shall put into place work methods and procedures that
address the following occupational health issues identified in the Construction Activities as:

Construction works may involve the storage and use of a limited variety of material and
chemical at various stages of the process.

Some of these materials may have inherent hazardous properties.

Examples are solvents, polymer materials, acids and caustic materials, disinfectants, anti-
mould and anti-fungus compounds, materials of construction, oils.

Employees who conduct construction work and who manually handle and use hazardous
substances will receive training on chemical handling and on proper use of personal
protection equipment (gloves, goggles, chemical masks, etc.).

Safety showers will be provided in the proximity of the chemical storage and handling areas.

Safety Material Data Sheets will be provided to supervisors to inform concerned employees
on the potential health risks and means of protection.

Contractor and subcontractors shall protect exposed employees by complying with the
Control of Substances Hazardous to Health Regulations (COSHH).

Contractor and subcontractors shall carry out Occupational Health Risk Assessments and
implement the necessary measures for each activity before it starts.

First Aid Kits shall be provided in the Contractor and Subcontractors Area

To prevent heat stress sufficient number of sun-sheds shall be provided at work area for
employees to rest.

Cool water shall be available for all Contractor and subcontractor employees.

Dust suppression programme shall be implemented.

Dust sampling shall be carried out on Monthly basis to verify the dust silica content.

Workers shall be provided with the appropriate Hearing Protection Devices (HPD) to protect
them against high level noise induced by heavy trucks and construction machines.

The HPD shall be inspected on regular basis by subcontractors Safety Officers and
replaced as required.

Portable toilets and Washing facilities shall be provided at site.

8.1 Safety

8.1.1 Objectives

Contractors objective is to complete the Construction Works fully embracing its zero
accident tolerance philosophy and achieving its target of zero fatalities, zero injury or illness
cases whether lost time or not, and zero incidents of a nature that may damage the
environment, property, equipment or reputation.

8.1.2 Safety Statistic Reports

The FHSEC shall complete and provide the Health & Safety Statistics Monthly Feedback
(Table no. A) at the close of each month.

Each subcontractor HSE Manager shall complete and submit the same form to the FHSEC
five days before the end of each month.

8.1.3 Safety and Health Statistics Monthly Feedback Definitions

SAFETY INDICATORS

Average Manpower: average number of personnel on duty, present within the Operating
Unit during the reported period (the value does not include the personnel mobilized but who
is not on duty in the particular period due to various reasons (i.e. illness).

Total Hours Worked (or Total Worked Manhours): are the total number of hours of direct
working activities within the Site, including paid overtime and training but excluding leave,
sickness and other absences.

Fatal Cases (Death): term to define a worker death resulting from a Work Related Injury.

Days Away Cases (before called LWC - Lost Work Days Case): all work-related injuries
or illness that results in 1 or more days away from work.

Days Away from Work (before called LWD - Lost Workdays): the number of calendar
days the employee was unable to work (include weekend days, holidays, vacation days,
etc.). The day of the injury or illness is not counted as a day away.

Job Transfer or Restricted Work Cases (before called RWC - Restricted work Case):
cases of work-related injury or illness that result in 1 or more days of Job transfer or
restricted work activity but do not result in any days away from work.

Days of Job Transfer or Restriction: the workdays on which, because of the work related
Injuries or illness, the employee is assigned to a Job Transfer or a restricted work activity.
The count of days of job transfer or restriction must be done in the same way of the count of
days away from work.

Medical Treatment Case (MTC): Cases of work-related injury or illness that result in the
management and care of a patient to combat disease or disorder.

First Aid Cases (FAC): cases of work-related injury or illness that result in:
using a non-prescription medication at non-prescription strength (for medications
available in both prescription and non-prescription form, a recommendation by a
physician or other licensed health care professional to use a non-prescription
medication at prescription strength is considered medical treatment for
recordkeeping purposes);
administering tetanus immunizations (other immunizations, such as Hepatitis B
vaccine or rabies vaccine, are considered medical treatment);
cleaning, flushing or soaking wounds on the surface of the skin;
using wound coverings such as bandages, Band-Aids, gauze pads, etc.; or using
butterfly bandages or Steri-Strips (other wound closing devices such as sutures,
staples, etc., are considered medical treatment);
using hot or cold therapy;
using any non-rigid means of support, such as elastic bandages, wraps, non-rigid
back belts, etc. (devices with rigid stays or other systems designed to immobilize
parts of the body are considered medical treatment for recordkeeping purposes);
using temporary immobilization devices while transporting an accident victim (e.g.,
splints, slings, neck collars, back boards, etc.).

using eye patches;
removing foreign bodies from the eye using only irrigation or a cotton swab;

removing splinters or foreign material from areas other than the eye by irrigation,
tweezers, cotton swabs or other simple means;
using finger guards;
using massages (physical therapy or chiropractic treatment are considered medical
treatment for recordkeeping purposes); or
drinking fluids for relief of heat stress.

Loss of Consciousness: All work-related cases involving loss of consciousness must be


recorded. The length of time the person is unconscious is irrelevant.

Total Recordable Incident (TRI): An injury or illness is recordable if it results in one or


more of the following:
Death;
Days away from work;
Restricted work activity;
Transfer to another job;
Medical treatment beyond first aid;
Loss of consciousness;
Significant injury or illness diagnosed by a PLHCP.

Other Recordable Cases (Significant Diagnosed Injury or Illness by a physician or


other health care professional): the following work-related conditions must always be
recorded at the time of diagnosis by a PLHCP:
Cancer;
Chronic irreversible disease;
Punctured eardrum;
Fractured or cracked bone or tooth.

Near Miss: it is a hazardous Event which, under slightly different circumstances, could have
caused an Accident affecting even people, environment or assets.

Road Traffic Accident (RTA) / Any Accident involving a Snamprogetti or Contractor/Sub-


Contractors vehicle during transportation of personnel or goods as per Snamprogetti
contract. RTA has to be reported in the Monthly Safety Statistical Report regardless the
vehicle occupant of the sustained an injury or not. When one or more vehicle occupants
sustain a personal injury, the RTA has to be recorded once as mentioned above (as RTA)
and the relevant Event (i.e. Days Away Case) in the specific section of the form.

Km Driver: Kilometers driven by the Snamprogetti and Contractor and Sub-Contractors


vehicles for work scope within the Site. Transportation from accommodation to the working
Site has to be counted (if applicable) only when the accommodation is provided by
Snamprogetti.

PRO-ACTIVE INDICATORS

Note: Pro Active Indicators may be reported only if properly documented.

HSE Training Hours: include the safety & environmental training courses provided to
Snamprogetti and Contractor personnel on Snamprogetti facilities and on external facilities,
agreed by the Snamprogetti. HSE Training Hours have to be considered as Contact hours
and it is calculated by multiplying the number of attendees with the duration of the training
session.
In the calculation of HSE Training hours shall not be included Safety Induction Training
such as offshore arrival, neither Emergency Drills.

HSE Management Visit: a planned, methodical, recorded but not exhaustive activity
performed by management which consists in making a tour of a site in order to observe the
state its in as far as health, safety & environment are concerned, and to what extent the
HSE policy is effective.

Toolbox Talks (TBT): Brief (10 15 minutes) meetings, focused on particular safety
issues, conducted prior to work commencing by a supervising person whose responsibility
is to assure that the appropriate information is given to promote awareness and
understanding of all the potential hazards which may affect the safe and efficient job
completion.

Job Safety Analysis (JSA): is a procedure used to identify, analyze and record the steps
involved in performing a specific job, the existing or potential safety hazards associated with
each step and the recommended action(s) /procedure(s) that will eliminate or reduce these
hazards and the risk of a Work Related injury.

HSE Meeting: any specific HSE Meeting held in work Site, base camp, offices and naval
assets where HSE matters are predominately discussed. It does not include Project
Progress meeting (even if HSE related matters are discussed).

HSE Inspections: a planned, methodical, recorded and exhaustive activity performed by


supervisory or HSE personnel, consisting in verification by examination, interview with
people or functional tests that planned measures are in place.

Safety/Hazard Observation Card It is a generic term, used to identify all the situations,
conditions observed and reported by the personnel employed in a Project or Site.
Observation reported are normally dealt with immediately after notification, solved by means
of short-term action and recorded.

8.2 Environmental

The Project environmental objectives are:

identify potential environmental effects/impacts;


comply with all aplicable environmental legislation and other requirements;
protect the work site and surrounding areas (and facilities) from unacceptable
impacts;
identify mitigation measures to control the environmental impacts produced.

Contractor shall prepare and implement detailed plans/procedures for managing the
relevant environmental aspects.

The main document to be performed during Construction phase to protect soil, groundwater
and human health is the CEMP (doc. N. 3034-00-ED-HS-PCR-AA505) and the following
detailed control plans:
o Waste Management Program for construction and commissioning;
o Spill contingency plan for construction

The main activities to control continuously, waste production, emissions and waste water
discharges and than to reduce, minimize the possible impacts on site, are:

o implementation of all the identified mitigation measures;


o control of the effective implementation of site environmental procedures by
means of audits, inspections;
o improving efficient use of energy and natural resources;
o assuring the environmental compatibility of plants and facilities;
o monitoring of the main environmental components (i.e. dust, noise);
o training activities planning;
o implementation of the soil and groundwater investigation activities (by
Employer).

8.2.1 Waste Management Program

The purpose of the Waste Management Program for Construction and commissioning is to
ensure that the wastes produced during Construction Works are managed as prescribed in
the contractual requirements and in all relevant national and applicable international laws in
order to prevent hazards to the health and safety of employees and to minimize the impact
on the environment.

In the Waste Management Program Contractor shall identify, classify and estimate all solid
and liquid waste produced during site activities and provide guidelines in order to collect,
storage, handle, transport and dispose of the same.

The waste minimization process shall include a strategy for managing wastes based on the
hierarchical principles of:
Reduction;
Reuse;
Recycle;
Disposal.

In details, the plan shall include the following information:

name, chemical composition, classification and estimated amount, waste source, waste
collection;
guidelines in order to collect, storage, handle, transport and dispose of the wastes;
recommended methods and principles of recycling and reducing the wastes.

Disposal facilities used by Contractor / subcontractor(s) shall be subject to approval by


Employer.

For further details refer to the Waste Management Program N. 3034-00-ED-HS-PCR-


AA507.

8.2.2 Spills prevention and response

Contractor and Subcontractors shall make every efforts in order to assure the protection of
environment and workers health and safety against accidental spillage that may occur
during site activities.

In particular, it shall be ensured that during work execution all operations are conducted in
order to avoid the risk of a spill situation or, whenever an accident occurs, implement
measures and actions to prevent its escalation.

Within this context, a dedicated plan/procedure shall be developed by Contractor with the
purpose of protecting the environment and workers against potential risks related to
accidental spills.

Starting from the identification of the main situations in which a spill of pollutants may occur,
that plan/procedure shall outline strategies for spill prevention relevant to the site activities
and describe procedures for the control and limitation of the releases.

The basic scope of this document is to:

identify all hazardous substances on site and provide the relevant MSDSs;
identify the types of spill and the scenarios which could possibly occur and lead to
pollution;
identify the prevention strategies and the response actions to be adopted in case of
pollutants release;
describe the organization that shall deal with the problem, both during prevention and
response phases;
describe the procedure to be adopted for alerting Contractor, Employer as well as other
parties involved in emergency management.

8.3 Permit to Work System

The Permit to Work system is designed to ensure that a safe environment is achieved by
providing management control over the various work activities, which may be potentially
hazardous.

During the Construction Works, all site works shall require a Contractor Permit to Work
system application.

The applicable permits depend on project status and work activities carried out.

Contractor Permit to Work System shall be issued by Contractor, prior to work inside the
Construction fenced areas.

The Contractor Permit to Work System is designed to identify potentially conflicting work
tasks both on a geographical and time basis.

It also sets limits to the duration and extent of the works.

The SM shall appoint someone to fulfil the role of Permit Coordinator before the
Construction Works starts.

The Permit to Work issuer has the following responsibilities:

to ensure that it is safe to perform the work in the particular area;


to ensure that the conditions are satisfactory for the work to proceed;
to ensure that each item on the permit is accurately filled out and covered;
to ensure that receiver understands the requirements on the permit and any unusual
conditions connected with the job;
to ensure the continue monitoring of the work area conditions and to stop the work if
unusual operating conditions arise which may affect safety of the personnel, the
environment and/or equipment;
to convey pertinent information about the work to the receivers;
to assign a person from his department to closely monitor the condition and the
work. The person shall have the authority to stop the work, if the condition warrants
it.

The Permit to Work receiver has the following responsibilities:

o to ensure that the workers understand what, where, and how work is to be
performed;
o to ensure compliance with the conditions stipulated on the permit;
o to use proper tools and equipment for the job;
o to follow safe working practices;
o to inform the issuing authority of any change in the condition of the job or the
surrounding area, or any delay in starting the work;
o ensure housekeeping and mechanical clean-up of the area and equipment upon
completion of work (the Receiver of the Permit to Work shall ensure that the work
permit is displayed at a visible location near the worksite);
o notify issuing authority upon completion of work.

More details shall be given in the Permit to Work Procedure.

8.4 Subcontractor requirements

Subcontractors have responsibility for the HSE of their operations irrespective of whether
carried out by themselves or others.

Subcontractors shall either:

issue and keep up to date their own HSE Construction Plan that shall be in
compliance with this document. All subcontractors shall comply with the most
stringent of this document and their HSE Construction Plan; or
fully adopt and comply with the requirements of this HSE Construction Plan as if
their own and otherway improve with comments;

provide specific HSE procedures for the works using procedures in att. 10.1 (HSE
procedures for Construction phase) generally for compliance but if agreed with
Contractor only for direction and guidelines if not completely applicable.

Subcontractors must ensure the requirements of this Hse Construction Plan are cascaded
to their subcontractors and so forth.

Subcontractors and suppliers will be advised of their specific HSE obligations within their
contract.

Contractor will monitor subcontractors and suppliers so that they operate in accordance with
the Contractor HSE Policy and Client Requirements.

8.5 Incident Investigation

All accidents/incidents on the Construction Works Area must be reported to the SM and
FHSEC as soon as practicable.

The SM or FHSEC shall report all accidents/incidents to the Client Representative in


compliance with project Requirements.

All accidents/incidents must be reported to Persian Gulf Oil Company as soon as


practicable.

The immediate field supervisor (supported by the FHSEC) shall lead investigations of any
accident/incident that occurs within his area of control, and ensure that all required reports
are completed and submitted as necessary.

In the case of serious injury or major damage, a Client Representative will head the
investigation.

The SM shall review the accident/incident reports and ensure that all recommendations and
corrective action has been carried out, and that he has addressed any items that may
require action on his part.

The FHSEC shall keep an up to date register of the accident/incident reports and the
consequent recommendations and actions.

The main reasons for conducting an accident/incident investigation are:

o not to apportion blame;


o to address causes so that similar accidents may be prevented;
o to determine the point at which unplanned events took over from the planned
sequence of events;
o to recommend corrective action that should be taken.

It is vital that accident scenes remain undisturbed until an investigation has been
conducted.

Contractor shall ensure that an immediate oral report is made to the Client Representative
in the case of all:

fatal Injuries;
injuries requiring medical attention, which may or may not result in lost time;
damage, in any amount, to Client equipment or property;

damage and near misses to cranes and heavy equipment.

It is the duty of subcontractors to notify the FHSEC of any accident or incident immediately.

For accidents involving fatalities, lost time injury, medical treatment case, restricted work
case, or damage to Client equipment or property, a preliminary written report shall be
submitted within 24 hours, followed by a detailed written report submitted within three days,
to the Client Representative.

A final written report shall be prepared and submitted to the Client Representative.

In the case of serious accidents, a detailed account of the circumstances, witnesses


statements and descriptive photographs will be provided.

In addition to the reports, required above, Contractor and subcontractors shall keep a
record of all injuries and damages on a form approved by the Client Representative.

FHSEC will analyze all the information and submits a monthly report to the SM.

Injuries resulting in less than 3 days lost time, non lost time injuries and occupational
disease shall be reported every quarter within 5 days of the end of the quarter to the Client
representative.

It is responsibility of the FHSEC to analyse data and prepare the Monthly Safety Statistical
Report (see paragraph 8.1.2 table no. A) for submission to the SM.

9 AUDIT AND REVIEW

9.1 Programme of Inspections

The SM and the FHSEC will be responsible for monitoring to ensure that Project HSE
requirements are met.

Routine daily HSE inspections shall be a core component of the FHSEC and subcontractor
HSE Manager (or subcontractor Safety Officer) duties.

The scope and areas of the above inspections shall be dictated by the current work
activities.

The FHSEC shall plan the emphasis of these HSE inspections on a weekly, or more
frequent, basis as necessary.

Inspection logs and action registers shall be maintained and submitted to the FHSEC on a
weekly basis.

Distribution of the inspection log shall include the SM and appropriate subcontractors,
supervisors and foremen.

In addition, the HSEM shall organize and manages HSE Audits on site which frequency will
be approximately at 20%, 50% and 75% of the work progress. These HSE audits shall be
carried out by an appointed audit team from Home Office by means of interviews and/or
inspections, in order to verify the correct implementation, by subcontractor personnel, of this
plan and the other relevant Project HSE plans. The auditors team shall also verify the
correct implementation of the above mentioned inspections and operational controls that the
FHSEC or other dedicated Contractor personnel have the duty to carry out on site.

CONTRACTOR shall also participate in Employer HSE audits and shall implement the
actions and recommendations.

Findings of all HSE audits shall be properly communicated to all relevant subcontractors
with target dates for each action item.

Follow-up of inspections shall be conducted as appropriate.

9.2 Lessons Learned form Inspection and Audit Results

Any time the results of an inspection or audit generate information which would enhance the
effectiveness of the HSE requirements, such information shall be disseminated to all
subcontractors, Contractor and Client via HSE meetings and the various communications
programmes available under this document.
