A completed Permission Slip must be taped or paperclipped to the back of the artwork in order to to be included in the exhibit.
I give permission for Mariposa’s Art and the Pajaro Valley Arts Council (PVAC) to include the artwork of the below student in the “Dreaming
Big” exhibit, October 16 and 17, 2010, at the Pajaro Valley Arts Council Gallery. I give my consent for the artwork to be photographed and used
in promotional material by the PVAC and Mariposa’s Art. I understand that my child’s artwork is not insured and that it is my and my child’s
responsibility to ensure proper drop off and pick up of the artwork.
