Introducción
Definición
Acumulación anormal de líquido en el espacio pleural
secundaria a múltiples etiologías
Recuerdo de Fisiología
• Dos membranas pleurales cubren a los pulmones, parietal y visceral.
- Normalmente hay 5 a 20mL de líquido interpleural (0.1 a 0.2mL/Kg)
• Dos agrupaciones básicas de causas para un derrame pleural.
- Exudativas si son secundarias a incrementos en permeabilidad capilar.
- Transudativas si son secundarias a incrementos en presión hidrostática.
Generalidades
• Un problema frecuente en la práctica clínica diaria.
- Un 1.5 millones de casos anuales en los Estados Unidos.
- Afecta al 8.4% de los pacientes en Unidades de Cuidados Intensivos.
- Incidencia hasta el 57% de los pacientes con neumonía y 87% en insuficiencia cardiaca.
• Síntomas iniciales son inespecíficos.
- Habitualmente disnea y dolor toráxico
- Evaluación imagenológica siempre recomendable ante la sospecha.
• Múltiples causas, frecuentemente ordenadas en dos aspectos característicos.
- Clásica diferenciación en transudado y exudado.

Conceptos Básicos
• Aspecto clínico característico habitualmente es inespecífico.
- Sintomatología compartida por múltiples otras patologías.
- Matidez y disminución de vibraciones vocales deben buscarse de todas maneras.
• Elementos acompañantes pueden orientar a etiología.
- Ascitis en daño hepático crónico o carcinomatosis.
- Yugulares distendidas y tercer ruido en insuficiencia cardiaca.
• Suele ser necesario confirmar radiológicamente el diagnóstico.
- Signos en radiografía de tórax.
- Ecotomografía pleural con eventual marcación para drenaje (Si >10mm).
- Tomografía computarizada.

Quilotórax
• Resultado de la patología del conducto torácico o sus tributarios.
- Recordar que por esta vía circulan lípidos en forma importante.
- Aspecto lechoso característico.
• Múltiples causas descritas.
- Un 50% son secundarias a neoplasias (Linfoma).
- Un 25% son atribuibles a trauma.
• Debe ser diferenciado del pseudoquilotórax
- Derrames pleurales crónicos que presentan acumulación de cristales de colesterol.
- Causa más frecuente es el secundario a artritis reumatoide crónico.

Tuberculosis
• Es una de las formas presentación de baja población bacilar de la tuberculosis.
- Habitualmente presentará transudados de predominio linfocitario.
• Las baciloscopías son insensibles.
- Sólo un 10 a 20% serán positivas en este escenario.
- Un 25 a 55% de los cultivos son positivos.
- La adición de biopsia pleural incrementa la sensibilidad al 90%.
• Puede utilizarse la adenosindeaminasa (ADA), pero no es muy útil en nuestro medio.
- Países con mayores endemias tienen mayor tasa de falsos positivos.
- Inespecífica, también se eleva en empiema, linfoma y artritis reumatoídea.
- Puede ser normal en pacientes con VIH.

Urinotórax
• Una complicación rara en fenómenos obstructivos de la vía urinaria.
- Derrames ocurren en el mismo lugar que el riñón obstruido.
• Aspecto de transudado con pH bajo.
• Para la confirmación puede usarse la creatinina del líquido.
- Será considerablemente más alta a la plasmática.

Urinotórax
• Una complicación rara en fenómenos obstructivos de la vía urinaria.
- Derrames ocurren en el mismo lugar que el riñón obstruido.
• Aspecto de transudado con pH bajo.
• Para la confirmación puede usarse la creatinina del líquido.
- Será considerablemente más alta a la plasmática.
Artritis Reumatoide
• Compromiso pleural ocurre en un 5% de los pacientes, habitualmente hombres.
- Aspecto del líquido puede variar desde seroso a hemorrágico.
• Examen que asiste en tamizaje es la glucosa del líquido.
- Muy raro si el valor es superior a 30mg/dL.
• Complemento bajo en líquido (C4) es también un hallazgo importante.

Es una patología frecuente que puede denotar enfermedades pulmonares,
pleurales o sistémicas.
Las etiologías pueden dividirse en dos grandes grupos, los exudados y los
transudados.
