having been sworn to in accordance with law, do hereby depose and state the following:

1. That we personally know MARY JANE LEONARDO NIGOS because she is our
family friend, playmate and she is publicly known in school and in the community
with that name;

2. That MARY JANE LEONARDO NIGOS is the daughter of Spouses EMILIO

ESPIRITU NIGOS and RUAL PINDI PASCUA LEONARDO, as per attached
copy of my NSO Birth Certificate marked herein as Annex “A”;
3. That she showed us her NSO Certificate of Live birth and her father’s middle
name ESPIRITU was inadvertently omitted thereon;

4. That she also showed us her parents’ Marriage Contract and her father’s middle-
name was also omitted thereon, however, her grandmother’s (Emilio Espiritu
Nigos mother) maiden name was correctly registered thereon as ESTANISLA
ESPIRITU, hence, her father’s middle-name is ESPIRITU, as per attached copy
of her parent’s marriage contract marked herein as Annex “B”;
5. That the person named EMILIO ESPIRITU NIGOS and the person named in
NSO Certificate of Live Birth as EMILIO NIGOS is one and the same person;

6. That MARY JANE middle name “L” should have been “LEONARDO” instead of
“L” only;
7. That this Affidavit is to attest to the truthfulness of this fact and to establish the
fact that MARY JANE father’s legal and correct name is EMILIO ESPIRITU
NIGOS and the “L” in MARY JANE middle name should be written as
“LEONARDO” instead of “L” only, and to have the same recorded as such by all
offices concerned, including effecting the same correction in my Certificate of
Live Birth, and for other legal purposes this may serve.

mother’s surname was registered as CAPAGNGAN as per attached Annex
“B”;

and the same person; and

74. That this Affidavit is to attest to the truthfulness of this fact and to establish
the fact that her legal and correct name is MAY ANN CAPAGNGAN
SANTOS and to have the same recorded as such by all offices concerned.

78. That I found out that my middle name registered at the National Statistics
Office as FELIX and my birthdate is January 18, 1959 instead of December
18, 1959, as per attached NSO Certificate of Live Birth marked herein as
Annex “A”;

79. That the person named in the NSO Certificate of Live Birth as ROLANDO
FELIX RAMOS born on January 18, 1959 and the name ROLANDO FELIX
RAMOS born on December 18, 1959 is one and the same person;

80. That this Affidavit is to attest to the truthfulness of this fact and to establish
the fact that my legal and correct name is ROLANDO FELIX RAMOS and
