Dear Kentucky Board of Physical Therapy,

Hello, my name is . I am writing this letter because the application requested that I send an
explanation for a simple misdemeanor I received in July of 2000 in the city of Des Moines, in Polk
County, in the state of Iowa. The charge was false report to a communications entity. I used to have a
cordless home phone and the number 4 button needed to be pushed very vigorously to get the phone
to acknowledge the number was pressed. Well a friend of mine at the time had a phone number that
was: 491-1789.

Now my friend and I, being dumb young men, engaged in ribald conversations quite often and
we were all around jerkfaces to each other as a form of bonding that stupid young men are want to do.
Anyway, when I heard the male (who I swear at the time sounded just like my friend) who answered say
911, what is your emergency, I believed it to be my friend just messing around. I played along with what
I perceived as the joke, making up a story about said friend, etc, etc. I will not go into details but I shall
say it was both inappropriate and really crude.

About two minutes into the conversation it finally dawned on me what had happened and
quickly hung up. About 20 minutes later two very upset police officers arrived at my home and promptly
arrested me. They took me to apologize to the dispatcher and then had me sit in jail until my sister
(who has never let me live it down to this very day) came and bailed me out. To add insult to injury, I
had to stand before the judge the next morning and listen to him repeat the transcript of the call. I plead
guilty and was given a 100$ fine and that was that, except for being made fun of by everyone I knew,
and also having to mention it when I apply for PT licenses, 16 years later.

I certainly did not do it on purpose, I did not wake up that morning and think to myself how fun
it would be to go to jail and pay a fine for no good reason. I am still embarrassed by the whole event and
I hope it doesn’t reflect poorly on my chances of obtaining a license in the good state of Kentucky. Thank

As before, this is a written explanation of a charge I received (based on the question of have I
ever had a record expunged). This one (and the only other one) was from June of 1998 in the town of
Indianola, in Warren county, in the state of Iowa. This charge was possession of Marijuana as an
accommodation. I was found in possession in May of 1998 before my 18th birthday and the police waited
until I was close enough to 18 (I turned 18 on July 4th, 1998) to charge me with the crime.

I really do not have any explanation for my actions other than I was young, stupid, and in with a
crowd at the time for which that was the norm. I do not make excuses for my actions, I was in the wrong
completely and utterly. I was given a differed judgement on the crime, I completed my probation with
no issues and the record was sealed. The charge was a wakeup call for me, I got my life on track as a
result and aside from the previously mentioned simple misdemeanor and a few speeding tickets, I have
never been in trouble since.

I truly hope this does not affect my ability to obtain a license in Kentucky. When I applied for a
license in Mississippi they had me petition a judge to unseal the records from the case who was unable
to do that. From what I gather, the records were lost in a flood about a decade ago. So Mississippi
accepted the judge’s explanation of what occurred in lieu of the records, I believe it took him a week to
respond to that request. I can petition the judge again to do the same for you if you so require, or
perhaps contact Mississippi Board of Physical Therapy to see if they can send you what the judge sent

Dear Kentucky Board of Physical Therapy,

This is an explanation of the gap in my work history from being granted my Iowa PT license in
May 2015 and when I finally began work as a PT (as a traveling PT in Jan 2016 in Mississippi). Shortly
after I received my license, my mother, who I took care of for 13 years, died. This tore me up fairly bad
and I wasn’t in the mindset to begin practicing at that time. Before I was able to fully recover from that,
my sister hit a deer while traveling down to visit me in Des Moines, Iowa from where she lived in
Minneapolis, Minnesota in June 2015.

My sister suffered 14 broken bones, including both legs, and was hospitalized for 3 weeks. When
she was released I went to go stay with her to help take care of her because she was unable to take care
of herself. I lived up there with her for five months, working as an Uber driver to help pay her bills
because she wasn’t able to work. The ease of that job’s hours allowed me to help support her financially
and physically. Once she was back on her feet I decided I wanted to do traveling work so that led me to
Mississippi and now, Kentucky.
