1. INTRODUCTION
Contractor Company Name was contracted by End Client to provide ROV Survey services with
respect to the inspection of offshore platforms in the Field Name field, offshore ……………….
This document provides the details the services in conducting a ROV General Visual Inspection (GVI),
Cathodic Potential Measurements (CP) and Scouring inspection on Platform Name Platform.
The inspection was conducted in the ddh of Month YYYY. The dynamically positioned (DP) vessel
Zakher Pioneer Providence was in charge to conduct the survey and was fitted with XX (Number in
letters) ROV system.
All inspection related works were carried out in accordance with End Client Name’s requirements/
Contractor’s standing work practices.

4. INSPECTION RESULTS
The following section presents the observed events recorded during the Platform Name platform GVI
inspection. A complete listing of all events is included in Appendix C.
4.1.1. Platform Name Platform Verification

Damage inspection was performed during the GVI on all of the platform components and the following
damages to the Platform Name platform observed

The seabed scour inspection was undertaken on the Platform Name platform to establish if any
suspension of horizontal elements, or seabed scouring, has occurred under the mud line external
horizontal members.
Depth readings were used to derive the scouring value, (the vertical distance between the bottom of
the horizontal element and the seabed) measured in Feet. This depth value was derived using the
following equation:

Where:
Riser Face measurement was conducted on the external horizontal members supporting the risers by
mean of clamps and riser guides at every elevation.
The datum of the measurement was at the welding point between every external horizontal member
and leg nodes at leg B2.
The following table and figures show the obtained measurements at each elevation.

Marine growth inspection was performed during the GVI on Platform Name platform, using graded
probe to stab the members for measuring the MG thickness on selected platform components, in order
to estimate the thickness, coverage, type and pattern of marine growth from waterline to mud line to
enable comparison with the as-built assumptions.
In the upper levels the hard growth coverage is the major coverage decreasing toward the mud line,
while the soft coverage decreased towards the sea surface.
XX (Number in letters) Anomalous MG readings were obtained during the MG Inspection. The table
below shows the details of marine growth inspection results
The cathodic protection system inspection on Platform Name platform was performed by ROV using
CP probe.
XX (Number in letters) anodes were found during the GVI & CP inspection with the following depletion:
XX (Number in letters) anodes were found to be slightly depleted (<25% - Grade 1).
XX (Number in letters) anodes were found to be moderately depleted (25% - 50% Grade 2).
XX (Number in letters) anode was found to be heavily depleted (50% - 75% Grade 3).
XX (Number in letters) anode was found to be totally depleted (75% - 100% Grade 4).
XX (Number in letters) anode was found to be disconnected (Grade 5).
XX (Number in letters) anode was found to be totally buried.
Readings should be between -800mV and -1050mV. Any readings outside these limits shall be
considered as anomalous.
The table below shows the details of anodes condition results
Cathodic protection measurements, which were taken on selected locations specified by End Client.
The platform showed a good level of protection, there were XX (Number in letters) anomalous
readings. The range of anodes CP readings were from -630mV to -1010mV.
Cathodic protection calibration was done under water before the inspection. The table below shows
the details of all CP inspection results

All ACFM results showed the welds on the selected nodes to be in good condition, all nodes have
been cleaned before ACFM. The table below shows the details of all ACFM inspection results
A debris survey was performed during the GVI on all of the platform components, in order to establish
nature of debris and to establish if any structural damage of the structure is associated with debris.
Debris which is considered anomalous is metallic debris which is in contact with the platform
elements.
None of the components inspected were affected by anomalous metallic debris that came within the
definitions prescribed in the scope of work. The table below shows the details of all debris inspection
results
