Scope of Services
SCOPE OF WORK

Plant Process Control:

 Service Provider shall conduct maintenance and control of the day to day operations of
the Sewage Treatment Plant (Plant). For this purpose, Service Provider shall assign one
(1) personnel for daily monitoring of the twelve (12) hour operation of the Plant, and one
(1) operation supervisory personnel to oversee the overall operations.

 Service Provider shall conduct visual checking, leak testing, lubrication, condition
assessment, and calibration of Plant equipment.

 Service Provider shall conduct routine preventive maintenance of the Plant, which
includes:

o Minor corrective maintenance of Plant equipment to ensure continuous operation

o Predictive maintenance of all Plant equipment to prolong service life.

 Service Provider shall attend to service and repair calls as well as provide corrective action
for unexpected equipment trouble, prepare service reports based on actual condition of
equipment before and after each service call, which shall include accurate measurement
of information for process control decisions.

 Service Provider shall supply the proper dosage and application of Hypo-chlorite
(Chlorine).

 Service Provider shall be responsible for general clean-up of the Plant.

Records Keeping and Compliance Reporting

 Service Provider shall keep accurate records for ongoing process management,
compliance reports, and establishment of historical trends for operational adjustments,
which shall include sampling analysis results, logs, diaries, and results of routine bench
tests, which shall form part of the report to be submitted to CLIENT and concerned
government regulatory agencies.

 Service Provider shall be responsible for the preparation and submission of the quarterly
Self-Monitoring Report (SMR) and shall prepare and apply for Discharge Permits with the
Department of Environment and Natural Resources (DENR).

 Any fee or penalty to be imposed by the DENR for any violation or failure to meet the
required effluent standards, as set by pertinent government regulatory agencies, shall be
for the account of the Service Provider.
