The undersigned, JUAN DELA CRUZ, hereby applies (or apply) to


have the land hereinafter described brought under the operation of the
Property Registration Decree, and to have the title thereto registered and
confirmed:
AND DECLARE . . . . .
1. That the applicant is the owner of the land (by virtue of
inheritance), together with the building and improvements thereon.
The said land, consisting of 25 hectares parcels are situated,
bounded and described as shown on the plan and technical
descriptions attached hereto and made a part hereof;
2. That said land at the last assessment for taxation was assessed at
P 4,532.10, Philippine currency, and the buildings and other
improvements at P 10,987.60, Philippine currency;
3. That to the best of my knowledge and belief, there is no mortgage
or encumbrance of any kind whatsoever affecting said land, nor
any other person having interest therein, legal or equitable, or in
possession;
4. That the applicant has acquired said land in the following manner:
inheritance from predecessor-in-interest on which has been
occupying the subject land for 56 years starting from 1961 up to
present;
5. That said land is solely occupied by the applicant;
6. That the name in full and address of the owner of the property are
as follows: JUAN DELA CRUZ, 85 General Luna St. Brgy.
Banaba, San Mateo, Rizal;
7. That the applicant is single;
8. That the applicant’s full name, age, citizenship, and residence is as
follows: Juan Dela Cruz, 47 y/o, Filipino, 85 General Luna St.
Brgy. Banaba, San Mateo, Rizal; and
9. That the following documents are attached hereto and made a part
hereof:
a. Plan in tracing cloth of land attested and dult certified by
the Regional Technical Director;
b. Technical descriptions certified by the Regional
Technical Director;
c. Geodetic Engineer’s certificate; and
d. Certificate of the Municipal Assessor of the assessed
value of the land at its last assessment for taxation.

PETITION

PETITIONER, by and through councel, unto this Honorable Court


respectfully aver that:

1. Petitioner, Juan Dela Cruz, is a Filipino, of legal age, single and a


resident of 85 General Luna St. Brgy. Banaba, San Mateo, Rizal;

2. Respondent is the Register of Deeds of San Mateo Rizal which


has an official address at City Hall Compound, Brgy. Guitnang
Bayan San Mateo Rizal where it may be served with summons and
other court processes at the given address;
3. Petitioner is applying for the registration of the unregistered land
located at 85 General Luna St. Brgy. Banaba, San Mateo, Rizal;
4. Petitioner has been occupying the subject land for 56 years starting
from 1961 up to present;

PRAYER

WHEREFORE, it is respectfully prayed that the Register of Deeds of


San Mateo Rizal issue an original certificate of title of the land located at 85
General Luna St. Brgy. Banaba, San Mateo, Rizal registered in the name of
the petitioner.
Other just and equitable reliefs are likewise prayed for.
