Unloading materials from a 2 3 6 All materials shall be safely loaded onto the vehicle using 2 1 2
flat-bed vehicle timber supports and packing, where applicable.
All materials shall be loaded by competent persons and
inspected by a supervisor prior to departure.

Dropping / loss of load 3 2 6 Operatives to visually inspect the lifting equipment prior to 3 1 3
any lift. All lifting tackle to have current certificate.
Operatives must be certificated banksman / slingers.
Operative to ensure the equipment is suitable for the task.
Ensure the load is secured and carry out a test lift.

Operative falling from trailer 2 2 4 Ensure the trailer is on secure ground prior to beginning 2 1 2
whilst unloading unloading.
All operatives unloading vehicles will be experienced and
qualified – i.e. competent persons.
Where deemed necessary radio contact shall be
maintained between the banksman and the crane driver at
all times.
If appropriate materials shall be bundled to minimise the
number of lifts.
Ladders are provided to allow safe access and egress to
and from the vehicles.

Severity (S): 1-minor 1st aid, 2-Minor hospital referral, 3-RIDDOR (over 3 day), 4-Major injury, 5-fatality
Likelihood (L): 0 - Very Remote, 1 - Remote, 2 - Occasional, 3- Probable, 4 – Frequent
Hazard Rating S x L (0-3 = Low, work can 4-7 = Medium, Work needs additional controls, 8 or more = High, Work shall not commence)
