SUBJECT: Reel Brake Safety Valve for V45HP, 60K, and
Approved KMC
V95HP CT Units

Problem
All V45HP, 60K, and V95HP coiled tubing (CT) units manufactured prior to 2006 will allow the
injector to inject tubing into the well with the reel brake set. This condition, if not detected, could
pull the injector, tubing guide, and wellhead equipment toward the reel until well stack failure
occurs.

Solution
A hydraulic safety valve should be added to the injector brake. This safety device will not allow the
injector brake to release until the reel brake has been released.

Procedure
For all V45HP, 60K, and V95HP CT units built before 2006:

1. Order a reel safety brake valve, SAP Part No. 101501148 (HydraForce PD10-41-6T-N-170 psi).
2. Install the reel safety brake valve as shown in Figure 1.
