tenant should move from premises prior to the expiration date, he/she shall liable for all the rental due until such
time the room/space is occupied by Landlord-approved resident and/or expiration of said time period, whichever
is shorter.

1. This Rental Agreement and/or Lease shall evidence the complete terms and conditions under which the parties
whose signatures appear below have agreed. Landlord/Agent shall refer to “Bank/Manager” and
Tenant(s)/Lessee(s) shall be referred to as “RESIDENTS” as consideration for this agreement, Bank/Manager agrees
to rent/lease to RESIDENT and RESIDENT agrees rent/lease from Bank/Manager for use SOLELY FOR USE OF BED
SPACE AS RESIDENCE, the premises listed above. RESIDENT acknowledges that any false statements found in
RESIDENT’S application shall constitute a breach of this agreement. RESIDENT hereby agrees to complete an
updated application. A GUEST or TRANSIENT may also take RESIDENCE in this property as defined in item nos. 7&8
below.

2. PAYMENTS: Rent and/or other charges are to be paid at UCPB through the Bank Manager, at the dated specified
in E, F, G and H above.

Bank/Manager acknowledges receipt of the First month’s rent of: Php. , and a Security Deposit of Php.
, and delivered to UCPB, Calauag, Quezon, during business hours. Any payment due on a holiday
maybe credited on the following business day, without penalty or charges. Any payment made after due date, shall
