Good day!

This is to formally ask for your permission to grant me an exemption card through the Student Affairs
Office (SAO). I am currently a full-time employee of Synergia BPO Solutions Inc and taking only five (5)
subjects in DWCL. Of these five, two (2) are scheduled every Saturday. I have attached a copy of my
Certificate of employment and a copy of my Green Form (Class Schedule) for your reference.

With these, considering the facts, it will be inconvenient for me to switch between office dress code and
school uniform knowing that I will only be in school for at most two and a half hours daily.

Further, I would like to apologize for this late request due to my work loads and limited time in school.
