Description of how to implement in the class:
Microsoft forms requires each student to have access to the internet either through a computer,
tablet, or smartphone. Students will then follow a provided link (or scan a provided QR code) to the
online quiz through Microsoft forms. Students will then go through each question on the quiz and
answer the questions as they see best fit. After the students submit the quiz, they will receive
immediate feedback on how they performed and the questions they missed. After taking the quiz,
students can then move on to the next task at hand.

After each student has completed the quiz through Microsoft forms, as the teacher, I will be able to go
in and monitor how each student performed. I will be able to pull up a Microsoft excel spreadsheet
with feedback on the entire class as well as each student individually. I will be able to look at each
individual question in excel and how the class performed on it. Not only that, but on the Microsoft
forms website, I will be able to access a graphic representation of the performance on each question.
After assessing how the class performed, I can make adjustments to any lesson plans and content
being covered in class.
Level of Technology Implementation (LoTi):
Microsoft forms best fits in as a LoTi level 2 – Exploration. The reason it fits best at this level is
because the software is very teacher friendly and centered while providing added benefit and
excitement while assessing the student’s understanding of the content.
Description of the sample spreadsheet and graph/chart provided *:
Viewers will be able to see a breakdown of each question in the Microsoft excel sheet along with how
the students performed on it. Each question will be provided along with each student’s answer. The
excel sheet will enable the teacher to search the sheet as well for a certain answer, question, or even
student.
Other comments about your proposed spreadsheet/graph/chart activity:
For a teacher, it is very feasible to set up and a great method for assessing student’s knowledge. It will
provide immediate feedback for students and teachers. Great tool for quizzes or small assessments!
