3. Acknowledgements; Assumption of Risks: By signing this form, I acknowledge, on my own behalf and that of my
scholar, and understand that there are inherent risks associated with the field trip, and that the field trip is voluntary – my
scholar is not required to participate. I also acknowledge and I agree that my scholar and I are ASSUMING ALL RISKS
associated with my scholar’s attendance and participation in the field trip, unless clear negligence on part of the school. I
have read, understand, and fully agree to the terms of this form, including the waiver. I acknowledge that I have been given
a reasonable opportunity to consider the terms of this form, including the waiver, and to seek advice from any person of my
choosing before signing it. I have signed this form freely, voluntarily, under no duress or threat of duress, without any
inducement, promise or guarantee being communicated to me. My signature is proof of my intention for this form to be a
complete and unconditional waiver and release of all liability of the Releasees with respect to the field trip to the fullest
extent permitted by law. I am 18 years of age or older and mentally competent to sign this form.
