I, GILLIAN KATE BACSAL, of legal age, Filipinos, single with

permanent address at Purok 2, Brgy. Canlapwas, Catbalogan City,
Samar, hereby name, appoint and constitute MARIA TERESA F. ANDOQUE,
likewise of legal age, and presently residing at Bacoor, Cavite, to be our
true and lawful attorney, to act on my behalf and in my name, to execute
and perform all or any of the following acts, deeds, matters and things, in
connection with the processing of the documents needed for the
issuance of my Certificate of Live Birth, to wit:

1. To process the correction of the entries in my Certificate of Live

Birth;

2. To demand, claim and receive the requested documents; and

3. To do all acts necessary to give effect to the foregoing authority.

HEREBY GIVING AND GRANTING unto my said attorney full powers and
authority to do and perform all and every act requisite or necessary to
carry into effect the foregoing powers, as fully to all intents and purposes
as we might or could lawfully do if personally present, with full power of
substitution and revocation, and hereby ratifying and confirming all that
our said attorney or her substitute shall lawfully do or cause to be done by
virtue hereof.
