Lesson Plan Form:

 Target Audience:
o Patients who are either pre-diabetic or are currently diabetic

 Audience Characteristics:
o Varying ages; primarily >50 years old
o Different heath backgrounds, physical capabilities and life experiences
o May be retired or still working full/part-time

Educational level:
 Varying educational levels from high school diploma to PhD
 Some have never discussed nutrition-related topics in depth before

Concerns: Motivation and willingness of participants to learn and make changes to their
lifestyles to better manage their diabetes.

 Advance arrangements/contacts:
o Intern has Grand View Health Diabetic Diet brochures (1200, 1500 and 1800
calorie options based on patients needs) to review with participants
o Work with participants to create a general diet plan for themselves based on
their individualized food preferences

 Behavioral Objectives:
o For participants to be able to count carbohydrate choices from their
individualized meal plan (15 grams carbohydrate = 1 carbohydrate choice) by
reading food labels and understand the importance of limiting their intake
moving forward.

 Visuals:
o Carbohydrate Controlled Diet brochures
o Additional educational materials made available to patients can be found in
Module 5: Education and Counseling

 Literature and Source (*materials normally used by the facility):


o UpToDate software provided by the hospital
o Academy of Nutrition and Dietetics Journal articles/website

 Pre- and post-test (Use creative approaches; plan method of administering the
pre- and post-test that is appropriate to the audience.)
o Pre-test: Verbally ask participants to explain their understanding of what
carbohydrates are and why it may be important to limit them as a Diabetic
patient. Also, ask them if they have ever followed a carb controlled diet or any
other specific diet in the past/ with or without success.
o Post-test: After creating individual meal plans, review carbohydrate counting
and the meaning of a carbohydrate choice versus 1 gram of carbohydrate.

 Schedule: How long each item will take?


o Introduction and Discussion – 3 minutes
o Brief review of carbohydrate brochures – 2 minutes
o Meal planning using carbohydrate choices – 15 minutes
o Clarify any underlying questions, discussion and review meal plans – 5
minutes

 Attendance / distribution of literature, etc.


o 2 employees, 1 fellow intern and 2 participants attended

 Welcome / Introduction, pre-test


o Introduce myself and review the purpose of Diabetes Management Class
o Pre-test: Ask participants to explain what type of diet they believe they should
be on and explain their worst habits in regards to eating.

 Body of presentation
o Review brochure information and plan to create a sample meal plan using
carbohydrate counting

 Questions and Answers


o To be done both during and at the conclusion of the session

 Conclusion /post-test
o Review with participants why a carbohydrate controlled diet is the best option
as opposed to other diets they may have tried in the past; takeaways from the
presentation; carb choices and brainstorm ideas of what they can be doing
differently moving forward to maintain good health and manage their
Diabetes
