1. Purpose
This procedure is designed to manage the blind insertion, rotation and
removal as part of positive isolation to ensure safety from the system
achieved and as a part of safety and smooth operation of the plant.

2. Scope
This procedure is applicable for blinds management activities (insertion,
rotation and removal) at SAMUR PROJECT.

4. Definition
Blind plates/spades are a standard metal plates which made on circle figure
that installed or inserted in the process pipe line to ensure safety of the
positive isolation is adequate; in order to protect people or process from the
fluid passing through.

5. Responsibility
5.1. Commissioning Manager
- Accountable to implement this procedure to entire SAMUR Project but not
limited to the construction team and OWNER.
5.2. Pre-commissioning Coordinator
- Assist Commissioning Manager to implement this procedure and ensure
all people under his/her subordinate comply with.
5.3. HSE Manager
- Responsible to do audit and improvement for implementation of this
procedure.
5.4. Pre-commissioning & Commissioning Shift Supervisor
- Mandatory to understood and comply or obey for the implementation of
this procedure.
- Ensure all operator under his/her subordinate understood and comply or
obey with this procedure.

- Responsible to ensure safety from the system has been adequate before
release permit for inserting/installing and/or removing/rotate blind plates.
- Ensure operator and technician understood and accept jobs sequence or
blind list to avoid mistake.
- Set up general safety.
- Periodically inspection and supervision to avoid incident happen.
- Ensure all jobs carried out safely with PPE and safe guard completed.
- Maintain coordination with supervisor with related concern party including
preparation, starting the jobs until finished.
- Obey with safety regulation, code and standard in place.
5.5. Pre-commissioning & Commissioning Technician
- Maintain and obey with general safety instructed by his superior.
- Follow and obey to the procedure in place.
- Direct supervision during the job commence until finished.
- Hanging the isolation and blind list tag number as per sequence number
and fill the tag completed and signed off.
- Ensure safety has been achieved and ready for further activity.
- Mandatory to understand and comply or obey for implementation of this
procedure.
- Ensure all blinds tag number sizing and rating met with line specification
as per P&ID/isometric drawing.
- To follow the blind sequence number of blind list to avoid mistake.
- Ensure no human error during implementation of this procedure.

6. Procedure
6.1. General Safety Rule
Pre-commissioning and Commissioning Shift Supervisor shall as
authorized person should execute the following;
- Identify the size, rating and material specification shall matches with piping
code specification requirement.
- Identify that blind has resistance to chemical or fluids in piping lines.
- Identify the number of blinds and tags.

- Identify the hazards, dangers and then avoid them with adequate
procedure/method, tool and safe guards.
- Review JHA and compliment.
- Shall prepare the isolation points and blind insertion point on the piping,
vessel or tank according to P&ID and/or isometric drawing and completed
in the Master Blind List form included sequence number of insertion.
- Assign technician and explain all steps of work, general safety and blinds
list and ensure he was understood and accepted with sign off in the
Master Blind List Form.
- Provides supporting document to trace the blind insertion.
- One copy of P&ID and/or isometric drawing completed with blind insertion
point and sequence of insertion or removal.
- Copy of Master Blind List.
- Senior operator/field operator carried out isolating all valve related with the
system for the draining depressurizing, gas freeing, then lock out the valve
with plastic tie wire with tags ‘Do Not Operate’.
- The blind tag in the point of installation that make numbering as per
master blind list then sign off the tag.
- After safety from the system achieved hand over the system to technician
by formal permit to work system then maintenance will install blind
according to Master Blind List and sequence number. If the technicians
found any potential dangers, then stop and return to operation for
discussion.

WARNING!!!

No blind activities to be carried out without the presence of an operator.

- After blind inserted, technicial shall tear-off the blind cards (yellow part), fill
in such information: Tag Number/Valve Number, Valve Size and Blind List
Reference then sign off the blind cards complete with name and date,
make sure the blinds installation properly.
- Both parties; Pre-commissioning and Commissioning teams representative
shall sign off I the Master Blind List after re-ensure that the blind insertion
has performed completely and properly in accordance to Master Blind List,

then return the blind cards and Master Blind List to Pre-commissioning
supervisor.
- Pre-commissioning supervisor shall assure the blind installation has been
corrected and properly in terms of the location based on Master Blind List.
- Do housekeeping and remove tools and people from the workplace.
- Keep the Master Blind List available at CCB.
- Close Permit of Work Installation blind jobs.
6.2. Blinds Insertion Procedure
- Blind insert/remove has been return to Pre-commissioning supervisor.
- Pre-commissioning shall re-ensure the safety from the system still
achieved. Check the vertical line may content remaining fluid or pressure,
temperature etc. should be drained properly.

WARNING
Check to ensure there is no pressure build-up at the line/vessel at either
side blind insertion/installation.
- Prepare work permit for removal blind then submit to technician as
performing authority.
- Pre-commissioning supervisor handover the Master Blind List removal with
sequent number to the maintenance technician to remove the blinds
including blind card yellow color.
- Technicians shall remove the blind only refer to blind cards (Yellow part)
received from Pre-commissioning supervisor according to sequent
number, then after blind removal, return the blind card (yellow part) and
blind tag (red part) back to Pre-commissioning supervisor.
- Technicians shall do housekeeping and cleaning workplace area, retract
all tools, part and personnel from workplace.
- Close work permit for removing blind.
- Pre-commissioning supervisor shall ensure that all blind has been
removed before bring the system ready back to operation by de-isolate all
isolation point.

- In the special case that any blind still utilized/needed for plant operation,
the area supervisor shall install blind tag and record as the remaining blind
the report to all shift supervisor via logbook.
