Inventory of Used and Unused Receipts and Invoice, all unused receipts/ invoices will be surrendered
Board Resolution approving the transfer of business, applicable to non-individual taxpayers
Photocopy of Barangay Clearance to operate in the new place of business
Photocopy of Mayors Permit from LGU where taxpayer is transferring
Photocopy of Lease Contract presented upon registration
Photocopy of Amended SEC Registration / DTI Certificate, where applicable
Verification Slip and other Transfer Related Docket(TRD)
CMS & Lis RF( collection unit)
